【摘要】：目的觀察托吡酯(TPM)與丙戊酸鈉(VPA)對癲癇患兒體質(zhì)量及糖代謝指標的影響。 方法選擇首次診斷癲癇TPM或VPA單藥治療的患兒各30例,分別于治療前、治療3個月和6個月進行體質(zhì)量、空腹血糖、空腹胰島素、胰島素抵抗(IR)指數(shù)、瘦素、脂聯(lián)素、瘦素／脂聯(lián)素(L/A)測定,行TPM、VPA治療前后自身對照研究。 結(jié)果TPM組：治療3個月的患兒空腹胰島素水平、IR指數(shù)、瘦素水平、L/A較治療前均明顯降低(均P0.01),體質(zhì)量和脂聯(lián)素水平變化不明顯(均P0.05)；治療6個月的患兒空腹胰島素水平、IR指數(shù)、瘦素水平、L/A較治療3個月時亦明顯降低(均P0.01),體質(zhì)量較治療前、治療3個月時均明顯增高(P0.05),但脂聯(lián)素水平較治療前明顯增高(P0.01): TPM治療前后空腹血糖水平無顯著區(qū)別(P0.05)。 VPA組：治療3個月的患兒體質(zhì)量、空腹胰島素水平、IR指數(shù)、瘦素水平、L/A較治療前均明顯升高(均P0.05)但脂聯(lián)素水平較治療前明顯降低(P0.01)；VPA治療6個月的患兒體質(zhì)量、瘦素水平、L/A較治療3個月時亦明顯升高(均P0.05)而脂聯(lián)素水平較治療3個月時下降更明顯(P0.01),空腹胰島素水平、IR指數(shù)較治療前、治療3個月時均明顯降低(均P 0.01); VPA治療前后空腹血糖水平無顯著區(qū)別(P0.05)。 結(jié)論(1)TPM治療后使癲癇患兒體質(zhì)量增長緩慢,而不是真正的體質(zhì)量下降。 (2)VPA治療后使癲癇患兒體質(zhì)量增加。 (3) TPM、VPA治療后可引起糖代謝指標的變化。(4) TPM、PA治療后對血糖水平無顯著影響。
[Abstract]:Objective to observe the effects of topiramate (TPM) and sodium valproate (VPA) on body mass and glucose metabolism in epileptic children.Methods 30 children with epilepsy treated with TPM or VPA for the first time were given body mass, fasting blood glucose, fasting insulin, insulin resistance index, leptin and adiponectin before treatment for 3 and 6 months, respectively.Leptin / adiponectin L / A was measured and self-controlled study was performed before and after treatment with TPM VPA.Results in TPM group, the fasting insulin level and IR index, leptin level and L / A were significantly decreased after 3 months of treatment (all P 0.01), the body weight and adiponectin levels were not changed significantly (all P 0.05), and the fasting insulin level of 6 months after treatment was significantly lower than that of before treatment (P 0.05), and no significant changes were found in the body mass and adiponectin levels (P 0.05).Leptin level was significantly lower than that at 3 months after treatment (all P0.01A, body weight was higher than that before treatment, P0.05A was significantly higher than that before treatment, but adiponectin level was significantly higher than that before and after treatment. There was no significant difference in fasting blood glucose level before and after treatment with TPM.In VPA group, the body mass, fasting insulin level, IR index and leptin level were significantly higher than those before treatment (all P0.05), but adiponectin level was significantly lower than that before treatment.The levels of leptin, L / A and adiponectin were significantly higher than those at 3 months after treatment (all P 0.05), and the levels of adiponectin were significantly lower than those at 3 months after treatment (P 0.01), and the fasting insulin level and IR index were higher than those before treatment.After 3 months of treatment, the level of fasting blood glucose decreased significantly (all P 0.01), and there was no significant difference in fasting blood glucose level before and after treatment with VPA (P 0.05).Conclusion the body mass of epileptic children increased slowly but not the real body mass decreased after TPM treatment.After treatment with VPA, the body mass of epileptic children was increased.(3) TPMN VPA could cause the change of glucose metabolism index after treatment. 4) TPMN PA had no significant effect on blood glucose level after treatment.
