【摘要】：目的研究早期系統(tǒng)化腸道管理對(duì)先天性肛門直腸畸形術(shù)后患兒排便功能的影響。方法研究學(xué)習(xí)國外成熟開展的腸道管理方案,擬定適合我地區(qū)實(shí)際情況的方案,進(jìn)行專家函詢確定其可行性。選擇重慶醫(yī)科大學(xué)附屬兒童醫(yī)院診斷為先天性肛門直腸畸形并行手術(shù)治療的患兒67例,分為對(duì)照組與干預(yù)組。1對(duì)照組實(shí)施常規(guī)護(hù)理健康教育。2干預(yù)組實(shí)施早期腸道管理,通過健康教育、電話隨訪、門診隨訪等方式實(shí)施擴(kuò)肛指導(dǎo)、飲食管理、服藥管理、排便功能訓(xùn)練、心理護(hù)理等。3調(diào)查患兒腸道管理情況及患兒在術(shù)后3-6個(gè)月時(shí)的排便功能結(jié)果。結(jié)果1早期系統(tǒng)化的腸道管理護(hù)理干預(yù)對(duì)患兒排便功能結(jié)果的改善有顯著意義[U=276.0,P=0.038]。2早期系統(tǒng)化的腸道管理對(duì)加強(qiáng)患兒家長嚴(yán)格飲食管理、掌握排便功能訓(xùn)練方法有積極意義。結(jié)論早期系統(tǒng)化腸道管理對(duì)先天性肛門直腸畸形術(shù)后患兒家長實(shí)施飲食管理、排便功能訓(xùn)練、排便功能改善有必要性、可行性及積極意義,值得臨床推廣。
[Abstract]:Objective to study the effect of early systematic intestinal management on defecation in children with congenital anorectal malformation.Methods to study the mature intestinal management scheme developed abroad, to draw up a plan suitable for the actual situation in our region, and to carry out expert inquiry to confirm its feasibility.Sixty-seven children with congenital anorectal malformation diagnosed by Children's Hospital affiliated to Chongqing Medical University were divided into two groups: control group and intervention group .1 the intervention group was given routine nursing and health education and the intervention group was given early intestinal management.By means of health education, telephone follow-up, outpatient follow-up, anus dilatation guidance, diet management, medication management, defecation function training, psychological nursing and so on, the intestinal management and defecation function of the children were investigated 3-6 months after operation.Results (1) early systematic nursing intervention had significant effect on the improvement of defecation function of children. 2 early systematic intestinal management had positive significance for strengthening strict diet management and mastering defecation function training method.Conclusion it is necessary, feasible and positive for parents of children with congenital anorectal malformation to carry out diet management, defecation function training and defecation function improvement by early systematic intestinal management, which is worth popularizing in clinic.
