血清S100B蛋白和神經(jīng)元特異性烯醇化酶預測手足口病嚴重程度的價值

【摘要】:目的探討手足口病(HFMD)患兒血清S100B蛋白和神經(jīng)元特異性烯醇化酶(NSE)水平檢測對預測HFMD嚴重程度的價值。方法將90例HFMD患兒分為普通型、重型和危重型3組,每組30例;隨機選取30例健康兒童作為對照組。ELISA法檢測治療前及治療后7 d血清S100B蛋白和NSE水平。采用受試者操作特征曲線(ROC曲線)評價S100B蛋白和NSE對HFMD嚴重程度的預測效能。結果危重型HFMD患兒血清S100B蛋白和NSE水平均較其他3組明顯升高(P0.01);重型患兒血清S100B蛋白和NSE水平均較普通型和對照組明顯升高(P0.01)。與治療前比較,危重型組和重型組治療后血清S100B蛋白及NSE水平明顯下降(P0.05)。當血清S100B蛋白在0.445μg/L時其Youden值最大,為0.611,其對重癥HFMD(包括重型和危重型HFMD)預測的靈敏度為61%,特異度為100%。血清NSE在5.905μg/L時其Youden值最大,為0.533,其對重癥HFMD預測的靈敏度為80%,特異度為73%。兩個指標并聯(lián)檢測的靈敏度為86%,特異度為73%,對預測重癥HFMD的價值最高。結論血清S100B蛋白和NSE有助于判斷HFMD病情嚴重程度及治療效果;二者如能并聯(lián)檢測對重癥HFMD的預測價值更高。
[Abstract]:Objective to investigate the value of serum S100B protein and neuron-specific enolase (NSE) in predicting the severity of HFMD in children with HFMD. Serum S100B protein and NSE levels were measured by Elisa in 30 healthy children before and 7 days after treatment. The predictive effect of S100B protein and NSE on the severity of HFMD was evaluated by using the operating characteristic curve (ROC curve). Results the levels of serum S100B protein and NSE in children with severe HFMD were significantly higher than those in the other three groups, and the levels of serum S100B protein and NSE in severe children were significantly higher than those in the common type and control group. Compared with those before treatment, the levels of S100B protein and NSE in severe children were significantly higher than those in the control group. The levels of serum S100B protein and NSE decreased significantly in the critical and severe groups after treatment, and the Youden value was the highest when the serum S100B protein was at 0.445 渭 g / L. The sensitivity of predicting severe HFMD (including severe and critical HFMDs) was 61 and the specificity was 100. The Youden value of serum NSE at 5.905 渭 g / L was the highest. The sensitivity for predicting severe HFMD was 0. 533, and the specificity was 73. The sensitivity and specificity of the two indexes in parallel detection were 86 and 73, respectively. Conclusion Serum S100B protein and NSE are of the highest value in predicting severe HFMD. Conclusion Serum S100B protein and NSE are helpful to judge the severity of HFMD.
