【摘要】：目的：探討一種聯(lián)合術(shù)式治療兒童發(fā)育性髖關(guān)節(jié)脫位(DDH)，觀察其臨床效果，并對相關(guān)問題進(jìn)行討論。方法：選擇合適病例，行聯(lián)合術(shù)式(髖關(guān)節(jié)切開復(fù)位、轉(zhuǎn)子下旋轉(zhuǎn)短縮內(nèi)翻截骨、改良Pemberton髖臼成形術(shù))并輔以術(shù)后功能鍛煉。按周永德、吉士俊提出的DDH療效評定標(biāo)準(zhǔn)對術(shù)后功能、X光片及三維CT表現(xiàn)進(jìn)行評定。分析出現(xiàn)術(shù)后并發(fā)癥的原因，總結(jié)治療經(jīng)驗。結(jié)果：91例115髖應(yīng)用聯(lián)合術(shù)式治療的DDH患兒當(dāng)中，84髖為優(yōu)（30-26分），16髖為良（25-21分），13髖為可(20-16分)，2髖為差（15-11分），，優(yōu)良率為86.95%。結(jié)論：聯(lián)合術(shù)式是治療兒童發(fā)育性髖關(guān)節(jié)脫位的有效、可靠方法，能徹底松解髖臼內(nèi)、外病變的軟組織，糾正髖臼及股骨畸形，恢復(fù)髖關(guān)節(jié)正常的解剖結(jié)構(gòu)。正確的手術(shù)操作及術(shù)后處理是治療的關(guān)鍵，能有效的減少術(shù)后并發(fā)癥。
[Abstract]:Objective: to explore a combined operation for the treatment of developmental dislocation of the hip (DDH) in children, to observe its clinical effect, and to discuss the related problems. Ji Shijun's criteria for evaluating the efficacy of DDH were used to evaluate the postoperative functional X-ray and 3D CT findings. The causes of postoperative complications were analyzed. Results among 91 cases of DDH treated by combined operation, 84 hips were superior 30 to 26 minutes, 16 hips were excellent, 25 to 21 minutes, 13 hips were good, 20 to 16 hips, 2 hips were inferior 15-11 points, and the excellent and good rate was 86.955.Conclusion: the combined operation is effective in the treatment of the hair of the children.
