【摘要】：目的評價(jià)不同維持量枸櫞酸咖啡因治療早產(chǎn)兒原發(fā)性呼吸暫停(AOP)的療效及不良反應(yīng)。方法選取AOP者60例,隨機(jī)分為3組治療:低維持量枸櫞酸咖啡因組[LD組:首劑20 mg/kg,維持量5 mg/(kg·d)]、高維持量枸櫞酸咖啡因組[HD組:首劑20 mg/kg,維持量10 mg/(kg·d)]、氨茶堿組,療程均至呼吸暫停消失1周。比較3組患兒AOP消失時(shí)糾正胎齡、總氧療時(shí)間等指標(biāo),并評估藥物不良反應(yīng)。結(jié)果 HD組及LD組氧療結(jié)束日齡明顯早于氨茶堿組,箱內(nèi)或頭罩吸氧時(shí)間較氨茶堿組明顯減少(P0.05);胎齡34周中,HD組氧療結(jié)束日齡明顯早于氨茶堿組(P0.05),LD組與氨茶堿組差異無統(tǒng)計(jì)學(xué)意義(P0.05);LD組與HD組各指標(biāo)差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論兩種維持量枸櫞酸咖啡因?qū)OP均有治療作用,優(yōu)于氨茶堿;在胎齡34周患兒中,高維持量療效較低維持量略好;未見明顯咖啡因不良反應(yīng)。
[Abstract]:Objective to evaluate the efficacy and side effects of caffeine citrate in the treatment of premature infants with primary apnea. Methods 60 cases of AOP were selected. They were randomly divided into three groups: low maintenance dose citrate caffeine group [LD group: first dose 20 mg / kg, maintenance dose 5 mg/(kg 路d], high maintenance dose citrate caffeine group [HD group: initial dose 20 mg / kg, maintenance dose 10 mg/(kg 路d], aminophylline group, aminophylline group. The duration of treatment was 1 week. The corrected gestational age and total oxygen therapy time when AOP disappeared were compared among the three groups, and the adverse drug reactions were evaluated. Results the end age of oxygen therapy in HD group and LD group was significantly earlier than that in aminophylline group. The oxygen inhalation time in the box or hood was significantly lower than that in the aminophylline group, and in 34 weeks of gestational age, the oxygen therapy in HD group was significantly earlier than that in aminophylline group (P 0.05) and aminophylline group (P 0.05). There was no significant difference in every index between HD group and HD group. Conclusion two maintenance doses of caffeine citrate have therapeutic effect on AOP. It was superior to aminophylline. In 34 weeks of gestational age, the therapeutic effect of high maintenance dose was slightly better than that of low maintenance dose, and no obvious side effect of caffeine was observed.
