【摘要】：目的探討Rh系統(tǒng)新生兒溶血病(Rh-HDN)與IgG抗體亞類的相關(guān)性。方法對50例孕婦產(chǎn)前抗體鑒定為Rh系統(tǒng)同種抗體,患兒出生后抽靜脈血做Rh血型、直接coombs試驗、游離抗體試驗、直抗IgG亞類檢測,將抗-D存在時分為1組與其他抗體組比較血清膽紅素值,將IgG1、3同時存時分為1組與IgG1或IgG3單獨存在時比較血清膽紅素值。結(jié)果 Rh血型系統(tǒng)5種抗原D、C、c、E、e的相應(yīng)抗體均可以引起Rh-HDN,其中抗-D(34%)、抗-E(28%)是最常見的抗體,抗-D較其他Rh抗體引起的HDN嚴重。新生兒直接coombs試驗≥2+,游離抗體試驗W+-3+,直抗IgG亞類主要為IgG1(38%)或IgG1、3(48%),IgG1、3同時存在時較單種IgG引起的HDN嚴重,同時發(fā)現(xiàn)IgG2和IgG4也能通過胎盤,結(jié)合在紅細胞上引起HDN。結(jié)論抗-D、抗-E是最常見的引起Rh-HDN的Rh抗體,抗-D引起的HDN較嚴重;引起HDN的IgG亞類主要為IgG1或IgG1、3,IgG1、3同時存在時引起的HDN較嚴重。
[Abstract]:Objective to investigate the correlation between Rh-HDN and IgG antibody subclass in neonatal hemolytic disease of Rh system. Methods 50 pregnant women with prenatal antibody were identified as Rh homologous antibody. The serum bilirubin level was measured by direct anti IgG subclass detection, and the serum bilirubin was compared with other antibody groups in the presence of anti D. The serum bilirubin level was compared with that of IgG1 or IgG3 alone. Results the corresponding antibodies of five antigens of Rh blood group can cause Rh-HDNs, among which anti-DN34B, anti-EN-28) are the most common antibodies.
