【摘要】：目的探討極低出生體質(zhì)量和極早早產(chǎn)與新生兒的生存狀況及相關(guān)影響因素。方法選取2012年4月至2016年1月該院收治的極低出生體質(zhì)量?jī)?VLBWI)和極早早產(chǎn)兒(VPI)142例,選取同期出生的非VLBWI和非VPI 140例,比較入組新生兒的生存狀況及遠(yuǎn)期預(yù)后,根據(jù)生存狀況將VLBWI與VPI分為死亡組和存活組,對(duì)兩組患兒的臨床資料進(jìn)行單因素和多因素Logistic回歸分析。結(jié)果出生體質(zhì)量小于1 500、1 500~2 500、2 500g新生兒的預(yù)后不良發(fā)生率比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。胎齡小于32周、32~37周、≥37周新生兒的預(yù)后不良發(fā)生率比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。死亡組新生兒的胎齡、出生體質(zhì)量和產(chǎn)前使用地塞米松的比例明顯低于存活組,母親年齡、窒息、胎糞吸入、妊娠高血壓綜合征和機(jī)械通氣比例明顯高于存活組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)�！�28周胎齡、≤1 000g出生體質(zhì)量、窒息是影響VLBWI與VPI生存狀況的獨(dú)立危險(xiǎn)因素(P0.05)。結(jié)論 VLBWI與VPI的生存狀況及遠(yuǎn)期預(yù)后不佳,易發(fā)生智力和運(yùn)動(dòng)障礙。
[Abstract]:Objective to investigate the survival status and related factors of very low birth weight, very early birth and newborns. Methods very low birth weight infants (VLBWI) and very early premature infants (VPI) were treated from April 2012 to January 2016 in our hospital. One hundred and forty cases of non VLBWI and non VPI were selected to compare the survival status and long term prognosis of newborns in the same period. According to the survival conditions, VLBWI and VPI were divided into death group and survival group. Univariate and multivariate Logistic regression analysis was carried out on the clinical data of the two groups. Results the incidence of poor prognosis of newborns with birth weight less than 1 500 / 1 500 / 2 500 g was significantly higher than that of control group (P 0.05), and the gestational age was less than 32 weeks or 37 weeks.
