【摘要】：目的：探討先天性心臟病發(fā)病的影響因素,為預(yù)防先心病的發(fā)生提供科學(xué)依據(jù)。 方法：采用病例對照研究方法。選擇湖南省十四個市州45所國家定點(diǎn)三網(wǎng)監(jiān)測醫(yī)院的2007年7月1日--2008年3月31日期間經(jīng)臨床與彩超確診的253名先心病為病例,按照配對條件(圍產(chǎn)兒性別相同,月齡相差2周內(nèi)),選取同期非先心病的正常圍產(chǎn)兒作為對照。病例與對照的父母均采用統(tǒng)一的調(diào)查表,由醫(yī)院婦產(chǎn)科醫(yī)生當(dāng)面詢問填寫,內(nèi)容包括：產(chǎn)婦基本情況、新生兒基本情況、孕婦懷孕前或孕期情況、家族史、丈夫情況、孕婦癥狀自評量表。數(shù)據(jù)采用Epidata3.2進(jìn)行錄入。資料采用統(tǒng)計(jì)描述等方法分析母親和圍產(chǎn)兒的一般情況,用SPSS16.0統(tǒng)計(jì)軟件包進(jìn)行單因素分析和多因素Logistic回歸分析。 結(jié)果：本研究單因素分析結(jié)果顯示孕產(chǎn)婦年齡、孕次、文化程度、職業(yè)、孕產(chǎn)婦職業(yè)接觸危險(xiǎn)因素、患慢性病、孕期感冒、早孕反應(yīng)、孕前或孕期生殖系統(tǒng)感染、居住地有環(huán)境污染源、孕期吃泡菜腌熏類食品、吃魚蝦肉蛋類食品、蔬菜水果類、牛奶豆類、孕期工作、孕產(chǎn)婦異常生育史、抑郁、焦慮及丈夫年齡、患慢性病等與先心病的發(fā)生有關(guān)聯(lián)；多因素條件Logistic回歸分析結(jié)果顯示：母親年齡,文化程度,生殖感染,孕期食肉類、魚、蝦類和蛋類,食蔬菜和水果,抑郁,焦慮與先心病的發(fā)生有關(guān)聯(lián)。 結(jié)論：本研究顯示母親年齡超過35歲,有生殖道感染、情緒抑郁、焦慮是圍產(chǎn)兒先心病的危險(xiǎn)因素。母親文化程度高、孕期食肉類、魚、蝦類和蛋類,食蔬菜和水果有利于降低先心病的患病風(fēng)險(xiǎn)。加強(qiáng)孕婦的健康宣教,增強(qiáng)孕婦的營養(yǎng)補(bǔ)充,保持孕期的心情舒暢,避免感染,有利于降低圍產(chǎn)兒先心病發(fā)生的危險(xiǎn)性。
[Abstract]:Objective: to explore the influencing factors of congenital heart disease and to provide scientific basis for prevention of congenital heart disease. Methods: a case-control study was conducted to select 253 patients with congenital heart disease (CHD) diagnosed by clinical and color Doppler ultrasonography from July 1st 2007 to March 31st 2008 in 45 state designated three nets surveillance hospitals in 14 cities and prefectures of Hunan Province. According to the matching conditions (the perinatal sex was the same and the age difference was within 2 weeks), the normal perinatal infants with non-congenital heart disease in the same period were selected as controls. The parents of both cases and controls used a unified questionnaire, which was filled out by the hospital gynaecologist in person. The contents include: the basic situation of the parturient, the basic condition of the newborn, the condition of the pregnant woman before or during pregnancy, the family history, the condition of the husband, The data were inputted by Epidata3.2. The general situation of mother and perinatal was analyzed by statistical description. Univariate analysis and multivariate Logistic regression analysis were carried out with SPSS16.0 software package. Results: univariate analysis showed that maternal age, pregnancy, education, occupation, occupational exposure risk factors, chronic diseases, cold during pregnancy, early pregnancy reaction, infection of reproductive system before or during pregnancy, Environmental pollution sources include eating pickled vegetables and smoked foods during pregnancy, eating fish and shrimp meat and eggs, vegetables and fruits, milk and beans, working during pregnancy, abnormal birth history of pregnant women, depression, anxiety and husband's age. Multivariate conditional Logistic regression analysis showed that the age of mother, education level, reproductive infection, carnivore during pregnancy, fish, shrimp and eggs, vegetables and fruits, depression, etc. Anxiety is associated with congenital heart disease. Conclusion: this study showed that mothers over 35 years old, with reproductive tract infection, depression and anxiety were risk factors of congenital heart disease of perinatal infants. Eating vegetables and fruits can reduce the risk of congenital heart disease, strengthen the health education of pregnant women, strengthen the nutritional supplement of pregnant women, keep the mood comfortable during pregnancy, avoid infection, and reduce the risk of congenital heart disease in perinatal infants.
