SP-A2單體型與早產(chǎn)兒呼吸窘迫綜合征的相關(guān)性研究

【摘要】:目的:本研究主要目的為了探討肺表面活性蛋白A2(SP-A2)單體型與早產(chǎn)兒呼吸窘迫綜合征(RDS)發(fā)病的關(guān)系,以及了解SP-A2單體型在廣西局部地區(qū)人群中的分布情況。方法:研究采用病例對(duì)照研究和人群研究的方法,選取80例患有RDS的早產(chǎn)兒及80例非RDS早產(chǎn)兒,同時(shí)選取足月分娩(包含順產(chǎn)及剖宮產(chǎn))的80例健康嬰兒作為研究對(duì)象;采集他們的靜脈或臍帶抗凝血,使用DNA提取試劑盒提取基因組DNA;應(yīng)用TaqMan(?)實(shí)時(shí)熒光定量聚合酶鏈反應(yīng)(polymerase chain reaction, PCR)方法對(duì)SP-A2基因上的4個(gè)單核甘酸多態(tài)性位點(diǎn)(single nucleotide polymorphism, SNP) (rs1059046、rs17886395、 rs1965707、rs1965708)進(jìn)行基因分型;每個(gè)SNP位點(diǎn)隨機(jī)抽取45例標(biāo)本應(yīng)用聚合酶鏈?zhǔn)椒磻?yīng)(polymerase chain reaction, PCR)和DNA測(cè)序技術(shù)驗(yàn)證TaqMan(?)實(shí)時(shí)熒光定量PCR的基因分型結(jié)果。結(jié)果:1、病例組和對(duì)照組的一般情況:所有研究對(duì)象在性別、胎齡、體重、雙胎妊娠及出生方式等情況在病例組和對(duì)照組比較中差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。2、SP-A2基因上的4個(gè)位點(diǎn)(rs1059046、rs17886395、rs1965707、 rs1965708)在足月兒組中的基因型頻率分布符合哈迪-溫伯格平衡(P0.05)。足月兒主要表達(dá)的單體型為1A0、1A5、1A1,頻率分別為54.2%、21%、 9.7%。3、SP-A2各單體型頻率分布在早產(chǎn)兒病例組和對(duì)照組總?cè)巳罕容^中差異均無統(tǒng)計(jì)學(xué)差異(P0.05)。4、以胎齡32周為界,進(jìn)行分組后(胎齡≥32周及32周),再分別對(duì)病例組和對(duì)照組進(jìn)行比較,發(fā)現(xiàn)SP-A2各單體型頻率分布在兩組中比較差異仍無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:1、未發(fā)現(xiàn)SP-A2的各單體型與早產(chǎn)兒RDS發(fā)病存在關(guān)聯(lián)。2、在廣西局部漢族人群中SP-A2的單體型分布有一定特異性。
[Abstract]:Objective: to investigate the relationship between pulmonary surfactant protein A2SP-A2 haplotype and respiratory distress syndrome (RDS) in premature infants. To understand the distribution of SP-A2 haplotype in local population in Guangxi. Methods: 80 premature infants with RDS and 80 non RDS premature infants were selected by case-control study and population study.
