【摘要】：目的:通過(guò)對(duì)哮喘患兒的心理行為特征及影響因素進(jìn)行研究調(diào)查,了解哮喘兒童的心理行為特征及容易出現(xiàn)的心理行為問(wèn)題,分析其心理行為特征及問(wèn)題的相關(guān)影響因素,加強(qiáng)患兒及照顧者對(duì)哮喘的認(rèn)識(shí),增強(qiáng)哮喘家庭護(hù)理管理計(jì)劃,同時(shí)為兒童支氣管哮喘診療過(guò)程中進(jìn)行的心理干預(yù)及社會(huì)支持提供可靠的臨床依據(jù)。方法:本研究收集2014年9月至2016年12月在吉林大學(xué)第二醫(yī)院確診的6~11歲支氣管哮喘兒童95例為哮喘組,同期收集在該院兒科門(mén)診體檢的6~11歲健康兒童100例為對(duì)照組,進(jìn)行問(wèn)卷調(diào)查。調(diào)查問(wèn)卷包括自制一般資料調(diào)查表、Achenbach兒童行為量表。將調(diào)查結(jié)果所得數(shù)據(jù)通過(guò)SPSS 21.0統(tǒng)計(jì)軟件進(jìn)行分析處理,采用t檢驗(yàn)、χ2檢驗(yàn)、方差分析、logistic回歸分析等統(tǒng)計(jì)學(xué)方法進(jìn)行統(tǒng)計(jì)分析,對(duì)結(jié)果進(jìn)行分析討論。結(jié)果:1.哮喘患兒在退縮、軀體主訴、社交問(wèn)題、焦慮/抑郁、攻擊性行為及違紀(jì)行為方面得分較正常兒童高,同時(shí)活動(dòng)能力及社交能力得分均低于正常兒童。2.患兒的性別、年齡、病程、病情分級(jí)、家庭習(xí)慣、家庭成員吸煙情況、父母學(xué)歷、家庭類(lèi)型、居住地對(duì)哮喘患兒心理行為特征均有影響。3.哮喘患兒檢出有心理行為問(wèn)題者34例,檢出率為35.79%,明顯高于對(duì)照組(11.00%),差異有顯著統(tǒng)計(jì)學(xué)意義(p0.001)。4.哮喘患兒心理行為問(wèn)題的影響因素包括年齡、家庭子女情況、生活習(xí)慣、母親學(xué)歷、家庭類(lèi)型、居住地、病程和病情分級(jí)。5.logistic回歸分析顯示:哮喘患兒心理行為問(wèn)題的危險(xiǎn)因素有學(xué)齡期、病情為重度;保護(hù)因素有非獨(dú)生子女、母親學(xué)歷為高中、中專(zhuān)和本科及以上。結(jié)論:1.哮喘患兒較正常兒童更容易出現(xiàn)以下心理行為特征:退縮、社交問(wèn)題、軀體主訴、焦慮/抑郁、攻擊性行為及違紀(jì)行為,同時(shí)活動(dòng)能力及社交能力更差。2.哮喘患兒心理行為特征的影響因素:性別、年齡、病程、病情分級(jí)、家庭習(xí)慣、家庭成員吸煙情況、父母學(xué)歷、家庭類(lèi)型、居住地。3.哮喘患兒比正常兒童更容易出現(xiàn)心理行為問(wèn)題。4.哮喘患兒心理行為問(wèn)題的危險(xiǎn)因素為:學(xué)齡期、病情為重度;保護(hù)因素為:非獨(dú)生子女、母親學(xué)歷為高中、中專(zhuān)和本科及以上。
[Abstract]:Objective: To study the factors through the investigation of the psychological and behavioral characteristics and effects on children with asthma, understand the psychological and behavioral characteristics of children with asthma and prone to psychological and behavioral problems and related factors analysis of their psychological behavior characteristics and problems, to strengthen the children and caregivers of asthma, asthma enhanced home care management plan, clinical on the basis of reliable support for social and psychological intervention and treatment of children with bronchial asthma in the process. Methods: This study collected from September 2014 to December 2016 in the second hospital of Jilin University were 6~11 years old children with asthma in 95 cases of asthma group, collected during the same period in the hospital outpatient department of Pediatrics examination 6~11 years old healthy children as a control group of 100 cases, questionnaire the investigation. The questionnaire includes general information questionnaire, Achenbach child behavior scale. The result of investigation data obtained by SPSS 21 .0 statistical analysis software, using t test, 2 test, variance analysis, logistic regression analysis and other statistical methods for statistical analysis, the results are analyzed and discussed. Results: 1. children with asthma in the back, somatic complaints, social problems, anxiety / depression, aggression behavior and delinquent behavior score is higher than that of normal children at the same time, activity ability and social ability scores were lower than normal children.2. children's gender, age, disease duration, disease classification, family habits, smoking status, family education of parents, family type, place of residence on the characteristics of the psychological behavior of children with asthma were detected in children with asthma.3. effects of psychological and behavioral problems in 34 cases, the detection rate was 35.79% that is obviously higher than the control group (11%), the difference was statistically significant (p0.001) effect of.4. in asthma children psychology and behavior problems factors included age, family, life habit, mother Education, family type, residence, history and disease classification.5.logistic regression analysis showed that risk factors of psychological and behavioral problems of children with asthma are at school age, illness is severe; the protective factors were non only child, mother's education for high school, college and Bachelor degree or above. Conclusion: 1. asthmatic children than normal children are more likely to have the following psychological behavior features: retreat, social problem, somatic complaints, anxiety / depression, aggression and antisocial behavior, influencing factors and activity ability and social ability worse.2. the psychological characteristics of children with asthma: gender, age, disease duration, disease classification, family habits, smoking status, family education of parents, family type, place of residence.3. of asthmatic children than normal children are more prone to the risk factors of psychological and behavioral problems of children with asthma.4. psychological and behavioral problems: school age, illness is severe; the protective factors for non only Children, mother's education is high school, secondary school and undergraduate and above.
