【摘要】：目的 探討尿液內(nèi)皮素-1（endothelin-1，ET-1）在腎盂輸尿管連接部梗阻（ureteropelvicjunction obstruction，UPJO）腎積水患兒診斷及術(shù)后隨訪中的作用及其意義。 方法 本研究隨機(jī)選取30例在廣州市婦女兒童醫(yī)療中心小兒泌尿外科就診、確診為單側(cè)UPJO并行腎盂輸尿管成形術(shù)的患兒；30例對(duì)照組兒童，其中包括10例健康兒童，10例膀胱輸尿管返流（vesicoureteral reflux，VUR）的患兒和10例腎結(jié)石的患兒。研究組患兒，分別在術(shù)前留取尿液標(biāo)本、術(shù)中從腎盂抽取尿液標(biāo)本，并在術(shù)后1、3、6、9、12月分別留取尿液標(biāo)本。對(duì)照組兒童也同樣留取尿液標(biāo)本。所有尿液標(biāo)本，均用酶聯(lián)免疫吸附法（ELISA法）檢測(cè)尿液ET-1值，同時(shí)測(cè)定尿肌酐（Cr）。 結(jié)果 研究組中，術(shù)前尿ET-1水平顯著高于其他三個(gè)對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。在患兒術(shù)后隨訪中，尿ET-1值在行腎盂輸尿管成形術(shù)1年后顯著下降。在整個(gè)研究組樣本中，以ET-13fmol/mg Cr為診斷界值，敏感性為76.7%，特異性為90%，總準(zhǔn)確率為83.3%。在8個(gè)嬰兒（≤1歲）的樣本中，以5fmol/mg Cr為診斷界值，則敏感性和特異性均為100%。 結(jié)論 尿液ET-1作為一個(gè)有效的生物學(xué)標(biāo)志物，具有敏感、特異、方便、非侵入性等優(yōu)點(diǎn)，在小兒UPJO的尿生物學(xué)診斷及長(zhǎng)期隨訪中具有較好的應(yīng)用前景。
[Abstract]:Purpose. To investigate the role and significance of urine endothelin-1 endothelin-1 (et 1) in the diagnosis and postoperative follow-up of hydronephrosis in children with ureteropelvic junction obstruction. Method. This study randomly selected 30 children who were diagnosed as unilateral UPJO combined with pyeloureteroplasty in Guangzhou Women and Children Medical Center for urology, 30 children in the control group. The study group included 10 healthy children with vesicoureteral reflux VURs and 10 patients with renal calculi. Urine samples were taken from the renal pelvis before operation in the study group, and urine samples were taken from the renal pelvis during the operation in the study group. Urine samples were collected on December. Urine samples were also collected from children in the control group. All urine samples were detected by Elisa and urine creatinine (creatinine) was measured by enzyme-linked immunosorbent assay (Elisa). Results. In the study group, the preoperative urinary ET-1 level was significantly higher than that in the other three control groups (P < 0.05). During the follow-up, the urinary ET-1 value decreased significantly after pyeloplasty for one year. The sensitivity of ET-13fmol/mg Cr was 76. 7 and the specificity was 90.
