學(xué)生缺鐵性貧血常用篩查方法比對分析

【摘要】:目的比對干化學(xué)法、血球儀與氰化高鐵法等常用學(xué)生缺鐵性貧血檢測方法的準(zhǔn)確性和重復(fù)性,為學(xué)生缺鐵性貧血的準(zhǔn)確診斷提供參考。方法使用全血樣品,分別采用幾種檢測方法對參加健康體檢的121份成人空腹靜脈血樣本進(jìn)行測定,分析不同方法與金標(biāo)準(zhǔn)氰化高鐵法在判定貧血上的異同。結(jié)果血球儀和干化學(xué)(A)2種方法與氰化高鐵法配對結(jié)果,差值分別為(0.29±2.77)(-0.15±2.80)g/L,差異無統(tǒng)計(jì)學(xué)意義(P0.05);而干化學(xué)法(B)與干化學(xué)法(C)2種方法與氰化高鐵法檢測結(jié)果相比,差值分別平均低(4.97±5.37)(6.85±4.51)g/L(t值分別為10.19,16.71,P值均0.01)。4種檢測方法血紅蛋白檢測值與氰化高鐵法檢測結(jié)果的相關(guān)系數(shù)均在0.98以上(P0.01),連續(xù)檢測20份樣本的標(biāo)準(zhǔn)差或變異系數(shù)均低于行業(yè)標(biāo)準(zhǔn)規(guī)定值。結(jié)論干化學(xué)法(A)的準(zhǔn)確性和重復(fù)性基本可以和血球儀及氰化高鐵法一致,而干化學(xué)法(B)和干化學(xué)法(C)的準(zhǔn)確性與氰化高鐵法存在一定差異。在現(xiàn)場缺鐵性貧血篩查中,可采用方便快捷的干化學(xué)法替代氰化高鐵法,但需要嚴(yán)格的質(zhì)量控制。
[Abstract]:Objective to compare the accuracy and repeatability of the methods of iron deficiency anemia in students, such as dry chemistry method, blood cell analyzer and high iron cyanide method, so as to provide a reference for the accurate diagnosis of iron deficiency anemia in students. The fasting venous blood samples of 121 adults who took part in the physical examination were measured by several methods. The differences and similarities between the different methods and the gold standard high iron cyanide method in the determination of anemia were analyzed. The difference values were 0.29 鹵2.77 ~ 0.15 鹵2.80 g / L, respectively. There was no significant difference (P 0.05), but the results of dry chemical method (B) and dry chemical method were compared with those of high iron cyanide method. The average difference value was 4.97 鹵5.37g / L = 6.85 鹵4.51g / L ~ (t) = 10.19 ~ 16.71g / L ~ (-1) P, respectively. The correlation coefficients of hemoglobin detection value and cyanoferrate test result were all above 0.98 (P < 0.01). The standard deviation or coefficient of variation of 20 consecutive samples were all lower than that of row. Conclusion the accuracy and reproducibility of dry chemical method are basically the same as that of blood cell analyzer and high iron cyanide method. The accuracy of dry chemical method (B) and dry chemical method (C) is different from that of high iron cyanide method. In the field screening of iron deficiency anemia, the convenient and quick dry chemical method can be used to replace the high iron cyanide method, but strict quality control is needed.
