【摘要】：研究背景： 近些年不少兒科醫(yī)師選用口服中成藥“玉屏風顆�！陛o助治療小兒反復上呼吸道感染,不但在臨床上取得較好的療效,又避免了中藥飲片制劑口服方式的不足,因而在臨床上得到不斷地推廣�！坝衿溜L顆�！弊鳛橹兴幊伤幹苿�,治療也應遵循中醫(yī)辨“質”論治理論,但其藥物說明卻缺少相應的中醫(yī)體質類型,古往今來玉屏風散是治療氣虛外感的代表方劑,而氣虛體質在小兒反復上呼吸道感染中最為常見,所以觀察玉屏風顆粒防治氣虛體質小兒反復上呼吸道感染的療效是急需解決的問題。 研究目的： 通過觀察玉屏風顆粒防治療氣虛體質小兒反復上呼吸道感染的療效,以期為提高玉屏風顆粒臨床用藥的療效和安全性提供依據。 研究方法： 選取符合本研究納入標準的小兒120例,根據隨機方法將其分為試驗組和對照組,其中每組各60例,試驗組和對照組均給予“對癥治療”,包括：如退熱處理、止咳化痰、補液等治療,在有細菌感染的情況下,可給與抗生素治療(常規(guī)抗生素的使用對本病的病程并無影響《兒科學》)；試驗組在取得患兒家長知情同意的情況下,在對癥治療的基礎上(上呼吸道感染恢復期)加用玉屏風顆粒(廣東環(huán)球制藥有限公司,國藥準字Z10930036)聯(lián)合治療。玉屏風顆粒(參考玉屏風顆粒說明書和文獻資料)擬用法用量[1-3]如下：1-3歲,每次口服1／2包,3歲,每次口服1包,均3次／日,用開水沖服,連續(xù)服用1個月。治療開始前進行基線資料分析,包括臨床資料(性別、年齡、體重)、入院前病程、發(fā)病次數、免疫方面(IgA、IgG、IgM)以及證候積分等方面。療效主要從患兒服用玉屏風顆粒1個月之后半年內的臨床病程、發(fā)病次數、證候積分及癥狀等進行觀察和判斷,觀察結束后進行統(tǒng)計數據的處理,此次統(tǒng)計學分析所用軟件為SPSS16.0。 研究結果： 1、通過統(tǒng)計分析,試驗組和對照組的總有效率都得到了較理想的結果(分別為96.67%和83.33%),相比較而言,試驗組的總有效率更高,兩組經統(tǒng)計學分析后有明顯差異(p0.05)； 2、兩組中醫(yī)證候積分方面的差異也具有統(tǒng)計學意義(p0.01)； 3、兩組在患病的次數、病程,癥狀體征的改善方面差異顯著(p0.01)； 4、另外免疫功能方面(特別是IgA、IgG)兩組在治療后差異也是比較明顯的(p0.01),說明玉屏風顆粒在免疫系統(tǒng)的調節(jié)方面也發(fā)揮了重要作用。 研究結論： 1、玉屏風顆粒聯(lián)合對癥治療對氣虛體質小兒反復上呼吸道感染的療效優(yōu)于單純對癥治療；2、玉屏風顆粒具有提高機體免疫功能的作用；3、玉屏風顆粒在此次研究過程中無明顯不良反應,安全性良好,適合臨床推廣應用。
[Abstract]:Background:. In recent years, many pediatricians have used oral Chinese patent medicine "Yuping Feng granule" to assist in the treatment of recurrent upper respiratory tract infections in children, which has not only achieved good clinical results, but also avoided the shortage of oral oral methods of Chinese herbal medicine. Therefore, it has been popularized in clinic. As a traditional Chinese medicine preparation, the treatment of Yupingfeng granule should also follow the theory of "quality" of TCM, but its drug description lacks the corresponding TCM physique type. Yupingfeng Powder is a representative prescription for the treatment of Qi deficiency, and Qi-deficiency constitution is most common in children with recurrent upper respiratory tract infection. So it is urgent to observe the curative effect of Yupingfeng granule in preventing and treating recurrent upper respiratory tract infection in children with deficiency of qi. Objectives of the study:. In order to improve the efficacy and safety of Yupingfeng granule in the treatment of recurrent upper respiratory tract infection in children with deficiency of qi, the therapeutic effect of Yupingfeng granule was observed. Research methods:. 120 children who met the criteria of this study were randomly divided into two groups: the experimental group (n = 60) and the control group (n = 60). The experimental group and the control group were given "symptomatic treatment", including: antipyretic treatment, cough and phlegm control. In the case of bacterial infection, antibiotics can be given (the use of routine antibiotics has no effect on the course of the disease; the experimental group has obtained the informed consent of the child's parents. Add Yupingfeng granules (Guangdong Global Pharmaceutical Co., Ltd.) on the basis of symptomatic treatment (upper respiratory tract infection recovery period), The dosage of Yupingfeng granules (with reference to the description of Yupingfeng granules and literature) [1-3] was as follows: 1 to 3 years old, 1/2 packets per oral for 3 years old, 1 packet for each time, 3 times a day, with boiling water, Baseline data were analyzed before treatment, including clinical data (sex, age, body weight, course of disease before admission, frequency of onset). The curative effect was mainly observed and judged from the clinical course, frequency of onset, syndromes and symptoms of the children within half a year after taking Yupingfeng granule for one month. The statistical data were processed after observation. The software used in the statistical analysis was SPSS 16. 0. Results of the study:. 1. Through statistical analysis, the total effective rate of the test group and the control group were all satisfactory (96.67% and 83.33, respectively). Compared with the control group, the total effective rate of the test group was higher, and there was a significant difference between the two groups after statistical analysis. 2, the difference of TCM syndromes integral between the two groups also has statistical significance. (3) there was significant difference between the two groups in the improvement of the frequency, course of disease, symptoms and signs of the disease (p 0.01). 4. In addition, the difference of immunological function between the two groups (especially IgA + IgG) after treatment is also obvious, indicating that Yupingfeng granule also plays an important role in the regulation of immune system. The study concluded that:. 1. The curative effect of Yupingfeng granule combined with symptomatic treatment on Qi-deficiency children with recurrent upper respiratory tract infection is better than that of simple symptomatic treatment. Yupingfeng granule has the effect of improving the immune function of the body. Yupingfeng granule is in the course of this study. There were no significant adverse reactions, It is safe and suitable for clinical application.
