【摘要】：目的本研究旨在闡明兒童呼吸道感染EB病毒(EBV)外周血淋巴細(xì)胞、異型淋巴細(xì)胞比值變化,及其與EBV-DNA載量的相關(guān)性。方法選擇2015年3月-2016年6月醫(yī)院確診為呼吸道感染的住院患兒624例,另選擇同期健康兒童100名作為對(duì)照組,采用實(shí)時(shí)熒光定量PCR檢測(cè)其外周血EBV-DNA載量;利用全血細(xì)胞分析儀進(jìn)行血常規(guī)檢測(cè),統(tǒng)計(jì)分析淋巴細(xì)胞檢測(cè)結(jié)果;同時(shí)人工鏡檢外周血異型淋巴細(xì)胞并計(jì)算百分比。結(jié)果健康對(duì)照組兒童外周血未檢出EBV-DNA,624例呼吸道感染患兒共檢出EBV-DNA陽性245例,陽性率為39.26%,EBV-DNA陽性樣本平均EBV-DNA病毒載量為(3.56±0.68)log IU/ml;1~3歲組患兒EBV-DNA陽性率高于1歲及3歲患兒,不同年齡呼吸道感染患兒EBV-DNA檢測(cè)陽性率比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05);呼吸道感染EBV-DNA陽性組患兒外周血淋巴細(xì)胞及異型淋巴細(xì)胞比值,均高于呼吸道感染EBV-DNA陰性組患兒及健康對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.001)。結(jié)論呼吸道感染EBV患兒外周血淋巴細(xì)胞及異型淋巴細(xì)胞比值明顯升高,并與EBV-DNA載量呈正相關(guān),可作為初步快速診斷呼吸道感染EBV的輔助性檢測(cè)指標(biāo),適用于廣大的基層醫(yī)院。
[Abstract]:Objective this study was designed to elucidate the changes of the ratio of peripheral blood lymphocytes and abnormal lymphocytes in children with respiratory tract infection of Epstein-Barr virus (EBV). Methods from March 2015 to June 2016, 624 hospitalized children with respiratory tract infection and 100 healthy children as control group were selected to measure the EBV-DNA load in peripheral blood by real-time fluorescence quantitative PCR. Blood routine examination was carried out by whole blood cell analyzer and the results of lymphocyte detection were statistically analyzed. Results EBV-DNA was positive in 624 children with respiratory tract infection without EBV-DNA in peripheral blood of healthy control group. The average EBV-DNA virus load of EBV-DNA positive samples was 3.56 鹵0.68%. The positive rate of EBV-DNA in 3 years old group was higher than that in 1 year old and 3 year old children. The positive rate of EBV-DNA in respiratory tract infection children of different ages was higher than that in children with respiratory tract infection of different ages. The ratio of peripheral blood lymphocytes and abnormal lymphocytes in EBV-DNA positive group was higher than that in EBV-DNA negative group and healthy control group. Conclusion the ratio of peripheral blood lymphocytes and abnormal lymphocytes in children with respiratory tract infection (EBV) was significantly increased and positively correlated with the EBV-DNA load, which could be used as an auxiliary index for the rapid diagnosis of EBV in respiratory tract infection. It is suitable for basic hospitals.
