兒童甲狀腺大小與影響因素的相關(guān)性研究

【摘要】:目的:本研究旨在探討性別、甲狀腺功能、年齡、尿碘結(jié)果、甲狀腺結(jié)合球蛋白抗體(TGAb)、甲狀腺微粒體抗體(TMAb)對兒童甲狀腺大小的影響。方法:研究對象為自2013年01月至2014年12月因頸部不適而就診我院兒科生長發(fā)育內(nèi)分泌門診的110名患兒,嚴格按照納入標準進行篩選,記錄患兒就診時間,性別,年齡,尿碘結(jié)果,甲狀腺B超結(jié)果,甲狀腺功能檢查結(jié)果,甲狀腺結(jié)合球蛋白抗體(TGAb),甲狀腺微粒體抗體(TMAb)。全部數(shù)據(jù)的統(tǒng)計學處理均使用SPSS17.0統(tǒng)計軟件。計數(shù)資料采用卡方檢驗或者Fisher確切概率法進行檢驗;計量資料,符合正態(tài)分布的可采用均數(shù)±標準差表示,使用t檢驗、方差分析或k-w檢驗;不符合正態(tài)分布的可采用中位數(shù)、四分位數(shù)間距表示,使用獨立樣本非參數(shù)的秩和檢驗。P0.05認為差異具有統(tǒng)計學意義。結(jié)果:1不同性別兒童的甲狀腺功能構(gòu)成比不存在統(tǒng)計學差異,甲狀腺各部分大小不存在統(tǒng)計學差異。2按照年齡,將研究對象分為如下3組:3~歲組,7~歲組,11~歲組。三組甲功結(jié)果構(gòu)成比進行比較所得P=0.496,甲功結(jié)果構(gòu)成無年齡差異;三組甲狀腺大小進行比較,隨年齡增長,左葉長度呈上升趨勢,三組均具有統(tǒng)計學差異(P0.001);左葉寬度呈上升趨勢,且第3組和第1、2組的差別具有統(tǒng)計學意義(P=0.003);左葉高度呈上升趨勢,三組之間的差異具有統(tǒng)計學意義(P=0.002),第1、2組的左葉高度普遍低于第3組的左葉高度;隨年齡增長,峽部厚度呈上升趨勢,第1組低于第3組,第2組與其它兩組之間暫不能認為有差異(P=0.027);右葉長度呈上升趨勢,且三組之間的差異具有統(tǒng)計學意義(P0.001);右葉寬度呈上升趨勢,第3組與第1、2組的差異有統(tǒng)計學意義(P=0.002);右葉高度呈上升趨勢,第3組與第1、2組的差異具有統(tǒng)計學意義(P=0.001)。3根據(jù)甲功結(jié)果可分為:甲功低于正常組、甲功高于正常組、甲功正常組。左葉長度、左葉高度、峽部厚度、右葉長度、右葉寬度、右葉高度第3組低于第1、2組,差異具有統(tǒng)計學意義(P0.001);左葉寬度三組之間的差別暫不能認為具有統(tǒng)計學意義(P=0.234)。4根據(jù)尿碘結(jié)果分為:100ug/l組,300ug/l組,
[Abstract]:Objective: to investigate sex, thyroid function, age, urinary iodine results, thyroid binding globulin antibody (TGAb). Thyroid microsomal antibody TMAb). Methods: from January 2013 to December 2014, 110 children with neck discomfort were selected from pediatric growth and development endocrine clinic in our hospital. According to the inclusion criteria, the time of visit, sex, age, urine iodine, thyroid B ultrasound, thyroid function, thyroid binding globulin antibody TGAbwere recorded. All the data were processed by SPSS17.0 software. The counting data were checked by chi-square test or Fisher exact probability method. The measurement data can be expressed by mean 鹵standard deviation, t test, ANOVA or k-w test. The nonconforming normal distribution can be expressed as median, quartile spacing. The results showed that there was no statistical difference in the thyroid function composition ratio of children with different sex of 1: 1 using rank sum test (P0.05) of independent sample. There was no statistical difference in the size of each part of thyroid. 2. According to age, the study subjects were divided into three groups: 3: 3 years old group and 7 ~-year old group. The composition ratio of the results of A work in the three groups was 0.496, and there was no age difference between the results of the results of the results of A work and that of the three groups. The size of thyroid gland in the three groups was compared, and the length of Zuo Ye increased with age, and there was a statistical difference among the three groups (P 0.001). The width of Zuo Ye increased, and the difference between group 3 and group 1 was statistically significant. The height of Zuo Ye was on the rise, and the difference between the three groups was statistically significant. The height of Zuo Ye in group 1 and group 2 was generally lower than that in group 3. The thickness of isthmus increased with age, the thickness of isthmus in group 1 was lower than that in group 3, and there was no difference between group 2 and the other two groups. The length of right lobe was on the rise, and the difference between the three groups was statistically significant (P 0.001). The width of right lobe showed an increasing trend, and the difference between group 3 and group 1 was statistically significant. The height of right lobe showed an upward trend, and the difference between group 3 and group 1 was statistically significant. According to the results of nail work, it could be divided into two groups: the work of A was lower than that of normal group, and the work of A was higher than that of normal group. The length of Zuo Ye, the height of Zuo Ye, the thickness of isthmus, the length of right lobe, the width of right lobe and the height of right lobe in group 3 were lower than those in group 1 (P 0.001). The difference between the three groups of Zuo Ye's width cannot be regarded as statistically significant for the time being. According to the results of urine iodine, they can be divided into two groups: one is the 300ugr-1 group, and the other is the 300ugr-1 group.
