【摘要】：為了探討腹內(nèi)壓監(jiān)測在急腹癥患兒診療中的應(yīng)用價值,本研究對2013年5月至2016年5月收治在承德醫(yī)學(xué)院附屬醫(yī)院小兒外科的150例急腹癥患兒的臨床資料進(jìn)行統(tǒng)計分析。結(jié)果顯示,急腹癥組彌漫性腹部疾病患兒的IAP顯著高于腹部局部炎癥患兒(p0.05);彌漫性腹部疾病、腹部局部炎癥患兒的IAP均顯著高于對照組(p0.05),IAP(7 cm H_2O,10 cm H_2O)比例(66.7%;54.7%)均顯著高于對照組(20.0%;0%)(p0.05),不良反應(yīng)發(fā)生率(10.0%)顯著高于對照組(0%)(p0.05),IAP與腹部壓痛、反跳痛、肌緊張等腹膜炎刺激癥狀、WBC均顯著相關(guān)(p0.05),WBC、IAP值診斷急腹癥的靈敏性、特異性、陽性預(yù)測值、陰性預(yù)測值、準(zhǔn)確率一致。因此,腹內(nèi)壓監(jiān)測在急腹癥患兒診療中具有良好的應(yīng)用價值。
[Abstract]:To explore the value of intraabdominal pressure monitoring in the diagnosis and treatment of children with acute abdomen. From May 2013 to May 2016, we analyzed the clinical data of 150 children with acute abdomen treated in pediatric surgery, affiliated Hospital of Chengde Medical College. The IAP of children with diffuse abdominal disease in acute abdomen group was significantly higher than that in local abdominal inflammation group (P 0.05). The IAP of children with diffuse abdominal disease and local abdominal inflammation was significantly higher than that of the control group (P 0.05) and the ratio of 10 cm H _ 2O to 7 cm H _ 2O was 66.7 cm. 54.7%) were significantly higher than that of control group (20.0%). The incidence of adverse reactions was 10.0), which was significantly higher than that in the control group (P 0.05) and abdominal tenderness, rebound pain, muscle tension and other symptoms of peritonitis stimulation. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy rate of WBC in the diagnosis of acute abdomen were significantly correlated. The monitoring of intra-abdominal pressure has a good value in the diagnosis and treatment of children with acute abdomen.
