高氧液聯(lián)合缺血后處理對(duì)家兔腸缺血再灌注致肝損傷的保護(hù)作用

【摘要】:目的探討高氧液聯(lián)合缺血后處理對(duì)家兔腸缺血再灌注所致肝損傷的保護(hù)作用。方法將家兔48只隨機(jī)分為假手術(shù)組(SO組)、缺血再灌注組(IR組)、高氧液組(HO組)和高氧液缺血后處理組(HI組),每組12只。SO組家兔僅手術(shù)顯露腸系膜上動(dòng)脈(SMA);IR組家兔阻斷SMA 60 min,再灌注120 min;HO組家兔在阻斷SMA 60 min和再灌注120 min過程中靜脈輸入高氧液;HI組家兔于阻斷SMA 60 min后,再灌注開始時(shí)靜脈輸入高氧液,同時(shí)進(jìn)行3個(gè)循環(huán)的30 s再灌注/30 s阻斷的缺血后處理。再灌注120 min后采集各組家兔動(dòng)脈血、靜脈血及部分小腸、肝組織,檢測(cè)腫瘤壞死因子-α(TNF-α)、白細(xì)胞介素(IL)-10、丙氨酸氨基轉(zhuǎn)移酶(ALT)、天冬氨酸氨基轉(zhuǎn)移酶(AST)及內(nèi)毒素水平,測(cè)定血清及小腸、肝組織內(nèi)丙二醛(MDA)含量及髓過氧化物酶(MPO)、超氧化物歧化酶(SOD)活性,Chiu 6級(jí)評(píng)分法觀察腸黏膜損傷情況,細(xì)菌培養(yǎng)觀察細(xì)菌移位率,免疫組織化學(xué)觀察肝組織中核因子κB(NF-κB)p65的表達(dá)。結(jié)果 IR組家兔血清中TNF-α、IL-10、內(nèi)毒素水平顯著高于SO組(P0.01);HO組和HI組家兔血清中TNF-α、內(nèi)毒素水平顯著低于IR組(P0.05);HI組家兔血清中TNF-α、內(nèi)毒素水平顯著低于HO組(P0.05,P0.01)。HO組和HI組家兔血清中IL-10水平顯著高于IR組(P0.01),HO組和HI組家兔血清中IL-10水平比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。IR組家兔血清中MPO活性和MDA水平顯著高于SO組(P0.01),HO組和HI組家兔血清中MPO活性和MDA水平顯著低于IR組(P0.01);HI組家兔血清中MPO活性和MDA水平顯著低于HO組(P0.05,P0.01)。IR組家兔血清中SOD活性顯著低于SO組(P0.01),HO組和HI組家兔血清中SOD活性顯著高于IR組(P0.01),且HI組家兔血清中SOD活性顯著高于HO組(P0.01)。IR組、HO組和HI組家兔腸黏膜損傷評(píng)分顯著高于SO組(P0.01),HO組和HI組家兔腸黏膜損傷評(píng)分顯著低于IR組(P0.01),HI組家兔腸黏膜損傷評(píng)分顯著低于HO組(P0.05)。IR組家兔小腸黏膜組織中MPO活性和MDA水平顯著高于SO組(P0.01);HO組和HI組家兔小腸黏膜組織中MPO活性和MDA水平顯著低于IR組(P0.01);HI組家兔小腸黏膜組織中MPO活性和MDA水平顯著低于HO組(P0.05)。IR組家兔小腸黏膜組織中SOD活性顯著低于SO組(P0.01);HO組、HI組血清SOD活性仍顯著高于IR組(P0.01);HI組家兔小腸黏膜組織中SOD活性顯著高于HO組(P0.05)。SO組家兔肝組織均無(wú)細(xì)菌移位,IR組家兔肝臟細(xì)菌移位率100.0%,HO和HI組家兔肝臟細(xì)菌移位率均為83.3%,HO和HI組家兔肝臟細(xì)菌移位率與IR組比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。IR組、HO組、HI組家兔肝組織內(nèi)NF-κB p65陽(yáng)性表達(dá)顯著高于SO組(P0.01),HO組和HI組家兔肝組織內(nèi)NF-κB p65陽(yáng)性表達(dá)顯著低于IR組(P0.01),HI組家兔肝組織內(nèi)NF-κB p65陽(yáng)性表達(dá)顯著低于HO組(P0.05)。IR組家兔肝組織中MPO活性及MDA、ALT、AST水平顯著高于SO組(P0.01);HO組和HI組家兔肝組織中MPO活性及MDA、ALT、AST水平仍顯著低于IR組(P0.01);HI組家兔肝組織中MPO活性及MDA、ALT、AST水平顯著低于HO組(P0.05)。IR組家兔肝組織中SOD活性顯著低于SO組(P0.01),HO組和HI組家兔肝組織中SOD活性顯著高于IR組(P0.01);HI組家兔肝組織中SOD活性顯著高于HO組(P0.05)。結(jié)論高氧液聯(lián)合缺血后處理可以減輕腸缺血再灌注引起的肝損傷。
