海綿竇顯微解剖學(xué)研究

【摘要】: 目的:海綿竇(以下簡(jiǎn)稱CS)位于顱中窩底部,是顱底病變常侵犯的部位,該部位的特殊位置與毗鄰結(jié)構(gòu)的復(fù)雜性使其一直是顱底外科學(xué)研究中倍受重視的部位,也是顱底手術(shù)入路難度較大和手術(shù)并發(fā)癥多見(jiàn)的區(qū)域之一。本研究旨在通過(guò)對(duì)額顳部眶顴入路的觀察、了解該入路對(duì)海綿竇的顯露,同時(shí)對(duì)海綿竇常用三角的界限、大小、及其內(nèi)重要結(jié)構(gòu)的毗鄰關(guān)系進(jìn)行顯微解剖測(cè)量,為海綿竇區(qū)手術(shù)提供解剖學(xué)基礎(chǔ)。 方法:對(duì)15例(30側(cè))成人尸頭標(biāo)本經(jīng)額顳部眶顴入路顯露海綿竇區(qū),在顯微鏡下觀察該入路對(duì)海綿竇結(jié)構(gòu)的顯露,去腦后對(duì)海綿竇區(qū)常用三角及其內(nèi)部結(jié)構(gòu)進(jìn)行顯微解剖學(xué)觀測(cè),所得數(shù)據(jù)應(yīng)用SPSS10.0軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。 結(jié)果:通過(guò)15例(30側(cè))成人濕性頭顱標(biāo)本經(jīng)額顳部眶顴入路充分顯露海綿竇及毗鄰結(jié)構(gòu),在顯微鏡下對(duì)海綿竇相關(guān)三角及內(nèi)部結(jié)構(gòu)進(jìn)行顯微解剖及測(cè)量,獲得海綿竇常用三角在非顱內(nèi)腫瘤疾病情況下的大小范圍數(shù)據(jù),并對(duì)相關(guān)三角的界限、構(gòu)成及其內(nèi)部結(jié)構(gòu)進(jìn)行觀測(cè),進(jìn)一步識(shí)別和保護(hù)海綿竇內(nèi)的重要結(jié)構(gòu),豐富臨床海綿竇區(qū)的顯微解剖學(xué)資料。
[Abstract]:Objective: the cavernous sinus (CS) is located at the bottom of the middle cranial fossa, which is often invaded by the lesions of the skull base. Because of its special position and the complexity of the adjacent structure, the cavernous sinus (CS) has always been an important part in the study of skull base surgery. It is also one of the areas where the surgical approach is difficult and the complications are common. The purpose of this study was to investigate the exposure of cavernous sinus via frontotemporal orbitozygomatic approach, and to make microanatomical measurements of the boundary, size and contiguous relationship of the common triangle of cavernous sinus. To provide anatomic basis for cavernous sinus surgery. Methods: the cavernous sinus region was exposed through frontotemporal orbital zygomatic approach in 15 adult cadaveric heads (30 sides), and the exposure of cavernous sinus structure was observed under microscope. The common triangle and its internal structure in cavernous sinus were observed by microanatomy after brain removal. The data were analyzed by SPSS 10.0 software. Results: the cavernous sinus and its adjacent structures were fully exposed through the frontotemporal orbital zygomatic approach in 15 adult wet skull specimens (30 sides). The related triangular and internal structures of cavernous sinus were dissected and measured under microscope. The size range data of common cavernous sinus triangles in the case of non-intracranial tumor diseases were obtained, and the boundary, composition and internal structure of the related triangles were observed to further identify and protect the important structures in the cavernous sinus. To enrich the microanatomical data of clinical cavernous sinus.
