【摘要】： 【目的】探討犬雙肺序貫性移植時，供肺的采取，保護及受體移植手術(shù)技巧及模型建立。 【方法】首先建立犬雙肺序貫性移植的動物模型：犬16條，，體重差別＜5％的隨機配對，分供體及受體組。進行雙肺序貫移植實驗共8次，移植右肺鉗夾左房時采用了‘v’鉗夾技術(shù)，而后通過對左肺及右肺術(shù)后0.5h、1h、2h各進行PaO2、mPAP、HR、肺水含量的檢測及電鏡觀察肺組織超微結(jié)構(gòu)，了解術(shù)后肺功能情況。 【結(jié)果】因心功能衰竭死亡兩例，而后采取新的移植技術(shù)后良好的解決了在鉗夾左房時因回心血量受阻而引起的心功能不全。mPAP術(shù)前術(shù)后有顯著差異(P＜0.05)，而PaO2、肺水含量術(shù)前術(shù)后并無明顯差異(P＞0.05)，表明移植后肺功能良好。 【結(jié)論】掌握了犬雙肺序貫性移植的全過程，“V”形雙阻斷鉗夾左心房技術(shù)提高了犬肺移植的成功率。
[Abstract]:Objective: to investigate the surgical techniques and models of donor lung transplantation in dogs. [methods] Sixteen dogs were randomly matched and divided into donor group and recipient group. The bilateral lung sequential transplantation was carried out for 8 times. The right lung clamp technique was used when the right lung was grafted into the left atrium. Then, the left lung and the right lung were treated with PaO2 mPAPP HRA at 0.5 h and 2 h after operation respectively. The lung water content was measured and the ultrastructure of lung tissue was observed by electron microscope. To understand the pulmonary function after operation. [results] two patients died of heart failure, After adopting the new transplantation technique, the cardiac insufficiency caused by the obstruction of left atrial volume in the left atrium was well solved before and after operation (P < 0.05), but there was no significant difference in Pao _ 2 and lung water content before and after operation (P > 0.05), which indicated that the cardiac insufficiency caused by the obstruction of left atrium volume was not significantly different after operation (P > 0.05), which indicated that there was no significant difference in Pao _ 2 and lung water content after operation (P > 0.05). After transplantation, the lung function is good. [conclusion] the procedure of sequential bilateral lung transplantation was mastered, and the success rate of canine lung transplantation was improved by "V" double clamping of left atrium.
