人工體神經(jīng)—內(nèi)臟神經(jīng)反射弧異類神經(jīng)元及盆神經(jīng)節(jié)內(nèi)突觸再生的形態(tài)學(xué)研究

【摘要】:脊髓損傷(SCI)導(dǎo)致的一直是醫(yī)學(xué)界的一大難題,大多數(shù)SCI患者都因肢體癱瘓和大小便功能失控,喪失自立自理能力,生活質(zhì)量極差,排尿障礙更直接威脅生命,給個(gè)人、家庭和社會都帶來極大的精神壓力和經(jīng)濟(jì)負(fù)擔(dān)。既往的一些治療方法如間歇導(dǎo)尿、選擇性骶神經(jīng)根切斷、骶神經(jīng)前根電刺激等療效均不理想。肖傳國教授于1990年提出設(shè)想:利用脊髓損傷平面以下的廢用神經(jīng),建立人工體神經(jīng)—內(nèi)臟神經(jīng)反射弧,形成一種新的經(jīng)體表皮膚控制的神經(jīng)反射路徑以實(shí)現(xiàn)膀胱的可控性排尿。經(jīng)過長期的臨床及基礎(chǔ)研究,均證實(shí)了該理論及新型術(shù)式的科學(xué)性、有效性和實(shí)用性。后來肖教授又將此反射弧理論應(yīng)用于大小便功能障礙的先天性脊柱裂(SB)脊膜膨出病人,亦取得明顯療效。人工體神經(jīng)—內(nèi)臟神經(jīng)反射弧為治療神經(jīng)源性膀胱開辟了新方向,多項(xiàng)研究已基本闡明了人工體神經(jīng)—內(nèi)臟神經(jīng)反射弧的作用機(jī)制,,但“異類神經(jīng)元”的再生以及新生的“異類神經(jīng)元”的軸突是否能夠隨著既往內(nèi)臟神經(jīng)路徑與副交感神經(jīng)節(jié)(盆神經(jīng)節(jié))建立新的突觸連接,目前還缺乏形態(tài)學(xué)證據(jù)。本研究旨在研究術(shù)后大鼠能否產(chǎn)生“異類神經(jīng)元”及突觸,從而提供形態(tài)學(xué)依據(jù),為進(jìn)一步闡述人工體神經(jīng)—內(nèi)臟神經(jīng)反射弧提理論打下基礎(chǔ)。
[Abstract]:Spinal cord injury (sci) has been a major medical problem. Most patients with SCI suffer from limb paralysis and uncontrolled defecation, loss of self-sufficiency, poor quality of life, and more direct threat to life from dysuria. Family and society bring great mental pressure and economic burden. Some previous treatments, such as intermittent catheterization, selective sacral root dissection and sacral nerve anterior root electrical stimulation, were not satisfactory. Professor Xiao Chuanguo proposed in 1990 that a new neural reflex pathway controlled by the skin of the body surface could be formed by using the disused nerve under the plane of spinal cord injury to establish a reflex arc of human somatic nerve and visceral nerve in order to realize the controllable urination of the bladder. After long-term clinical and basic research, the theory and the new operative method are proved to be scientific, effective and practical. Professor Xiao later applied this reflex arc theory to patients with congenital spina bifida (SBB) with dysuria. The reflex arc of human somatic nerve and visceral nerve has opened up a new direction for the treatment of neurogenic bladder. Many studies have basically clarified the mechanism of the reflex arc of human somatic nerve and visceral nerve. However, the regeneration of "heterogeneous neurons" and the ability of axons of new "heterogeneous neurons" to establish new synaptic connections with parasympathetic ganglion (pelvic ganglion) along with the previous visceral nerve pathway are still lack of morphological evidence. The purpose of this study was to study whether "heterogeneous neurons" and synapses could be produced in rats after operation, and to provide morphological basis for the further elaboration of the theory of reflex arc extraction of human somatic nerve and visceral nerve.
