【摘要】: 目的:通過解剖學(xué)觀測(cè)和MSCT影像學(xué)研究,探討正?v隔淋巴結(jié)短徑的MSCT正常值范圍。材料和方法:參照改良的ATS淋巴結(jié)分布圖,對(duì)經(jīng)嚴(yán)格選擇的38例成人尸體標(biāo)本和70例無縱隔淋巴結(jié)腫大者分別行解剖學(xué)觀測(cè)和MSCT研究,并由3位在有經(jīng)驗(yàn)的放射科醫(yī)師盲法觀察影像資料。結(jié)果:1.解剖學(xué)觀測(cè)結(jié)果:38例成人尸體標(biāo)本共觀察到824個(gè)縱隔淋巴結(jié)。各區(qū)淋巴結(jié)的形態(tài)以圓形和類圓形為主,約占76%,5區(qū)和7區(qū)其他形狀的淋巴結(jié)相對(duì)較多。各區(qū)最大淋巴結(jié)的方位多呈豎位,但5區(qū)多呈橫位。2.MSCT觀測(cè)結(jié)果:70例病例一共測(cè)得縱隔淋巴結(jié)1521個(gè),平均每例21.73個(gè),平均大小為3.54mm±1.37mmSD。不同區(qū)域淋巴結(jié)的數(shù)目和大小各不相同。2R區(qū)淋巴結(jié)數(shù)目多于2L區(qū)(P<0.05),4區(qū)淋巴結(jié)數(shù)目明顯多于2區(qū)(P<0.01),,4R和4L區(qū)淋巴結(jié)的數(shù)目沒有顯著性差別(P=0.785)。2R區(qū)淋巴結(jié)明顯大于2L區(qū)(P=0.01),10R區(qū)淋巴結(jié)明顯大于10L區(qū)(P<0.01),4區(qū)淋巴結(jié)明顯大于2區(qū)(P<0.01),7區(qū)淋巴結(jié)明顯大于4區(qū)(P<0.01)。結(jié)論:縱隔各區(qū)淋巴結(jié)的大小和數(shù)目不同;正?v隔淋巴結(jié)短徑的MSCT標(biāo)準(zhǔn)是:7和10R區(qū)≤8mm,2L、6和10L區(qū)≤5mm,其余5個(gè)區(qū)均≤6mm。
[Abstract]:Objective: to study the normal range of MSCT of mediastinal lymph node by anatomical observation and MSCT imaging study. Materials and methods: 38 adult cadavers and 70 cases without mediastinal lymphadenopathy were studied by anatomical observation and MSCT study with reference to the modified ATS lymph node distribution. The imaging data were observed by 3 experienced radiologists. The result is 1: 1. Anatomical results: a total of 824 mediastinal lymph nodes were observed in 38 adult cadavers. The shape of lymph nodes in each region was mainly round and round, accounting for more lymph nodes of other shapes in area 765 and area 7. Most of the largest lymph nodes in each region were vertical, but most of the 5 regions were horizontal. 2. The results of MSCT showed that 1521 mediastinal lymph nodes were detected in 70 cases with an average of 21.73 per case, and the average size was 3.54mm 鹵1.37 mm SD. The number and size of lymph nodes in different regions were different. The number of lymph nodes in region 2L was more than that in region 2L (P < 0.05P < 0.05). There was no significant difference in the number of lymph nodes between region 4R and region 4L. Conclusion: the size and number of lymph nodes in mediastinal regions are different, and the MSCT criteria for the short diameter of normal mediastinal lymph nodes are 鈮
