胰島素對內(nèi)毒素誘導(dǎo)大鼠肝細胞損傷的保護作用及其抗炎機制

【摘要】:研究背景 嚴重創(chuàng)傷、全身感染、肝病以及眾多的危重疾病在其發(fā)生發(fā)展過程中都可發(fā)生腸源性內(nèi)毒素血癥,大量的內(nèi)毒素可直接或者通過激活肝組織中的枯否細胞(Kupffer cell,KC)合成及釋放多種炎性介質(zhì)間接損傷肝細胞,從而加劇肝臟病變,導(dǎo)致肝功能不全,引發(fā)肝衰竭,造成不可逆損傷。臨床研究發(fā)現(xiàn),對于重癥監(jiān)護以及燒傷、膿毒血癥及感染性休克等危重疾病患者給予胰島素強化治療可改善病人癥狀,降低病人死亡率及其合并癥的發(fā)生率,但其作用機制尚不清楚。 近年發(fā)現(xiàn),,胰島素不僅可影響細胞糖代謝,還可抑制氧化應(yīng)激和缺血/再灌注(myocardial ischemia/reperfusion,MI/R)誘導(dǎo)的心肌細胞凋亡,進而發(fā)揮心臟保護作用。我室前期的研究也發(fā)現(xiàn)在離體細胞和整體動物MI/R模型上胰島素可通過抗凋亡作用減輕心肌細胞及內(nèi)皮細胞損傷。新近的研究報道胰島素能夠通過促進肝細胞增殖和抑制肝細胞凋亡,改善肝功能和維持肝臟的完整性。近年來,人們通過對內(nèi)毒素血癥及熱損傷大鼠
