【摘要】： 目的對(duì)腹主動(dòng)脈下段的解剖關(guān)系進(jìn)行研究,找出腹主動(dòng)脈下段阻斷的最佳途徑及部位,為低位腹主動(dòng)阻斷術(shù)的順利完成提供解剖方面的資料。方法對(duì)27具尸體標(biāo)本進(jìn)行解剖,找出腹主動(dòng)脈下段的主要分支,測(cè)量分支間的距離及與腹主動(dòng)脈下段相鄰的重要組織器官的距離,進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果腸系膜下動(dòng)脈到主動(dòng)脈分叉最低點(diǎn)的距離為4.27±0.8cm。腸系膜下動(dòng)脈到第三腰橫動(dòng)脈的距離為0.46±0.52cm。第三、四腰橫動(dòng)脈的距離為2.18±0.48cm。第四腰橫動(dòng)脈到骶正中動(dòng)脈的距離為1.09±0.62cm。骶正中動(dòng)脈到腹主動(dòng)脈分叉最低點(diǎn)的距離為0.53±0.37cm。第三、四腰橫動(dòng)脈水平腹主動(dòng)脈壁至相應(yīng)側(cè)輸尿管的距離為:第三,左2.6±0.37cm,右3.56±0.3;第四,左2.4±0.38cm,右3.34±0.35cm。腹主動(dòng)脈與下腔靜脈相鄰壁間距離為:第三腰橫動(dòng)脈水平0.73±0.34cm,第四橫動(dòng)脈水平0.53±0.32cm。結(jié)論第三、四腰橫動(dòng)脈間的距離最寬,適合在此進(jìn)行低位腹主動(dòng)脈阻斷術(shù),第三、四腰橫動(dòng)脈間的中點(diǎn)至主動(dòng)脈分叉最低點(diǎn)距離為:2.7±0.62cm,阻斷點(diǎn)宜在此處;腸系膜下動(dòng)脈至主動(dòng)脈分叉最低點(diǎn)的距離為:4.27±0.8cm,不應(yīng)超過此距離;腹主動(dòng)脈與下腔靜脈很近,分離時(shí)應(yīng)注意避免損傷靜脈壁。
[Abstract]:Objective to study the anatomical relationship of the lower segment of the abdominal aorta and to find out the best way and location of the inferior segment of the abdominal aorta. Methods 27 cadavers were dissected and the main branches of lower abdominal aorta were found. The distance between branches and the important tissues and organs adjacent to the lower segment of abdominal aorta were measured. Results the distance from the inferior mesenteric artery to the lowest point of aortic bifurcation was 4.27 鹵0.8 cm. The distance from the inferior mesenteric artery to the third transverse lumbar artery was 0.46 鹵0.52 cm. Three. The distance between the four transverse artery and the median sacral artery was 2.18 鹵0.48 cm 路4th cm. The distance from the median sacral artery to the lowest point of the abdominal aortic bifurcation was 0. 53 鹵0.37 cm. Third. The distance between the horizontal abdominal aorta wall of the four lumbar transverse arteries and the corresponding lateral ureter was as follows: third, left 2.6 鹵0.37 cm, right 3.56 鹵0.3; 4th, left 2.4 鹵0.38 cm, right 3.34 鹵0.35 cm. The distance between abdominal aorta and inferior vena cava was 0.73 鹵0.34 cm. 4th horizontal artery level was 0.53 鹵0.32 cm. Conclusion the distance between the third and fourth lumbar transverse arteries is the widest, which is suitable for low abdominal aortic occlusion. The distance between the midpoint of the four lumbar transverse arteries and the lowest point of aortic bifurcation is 7. 7 鹵0. 62 cm, where the blocking point should be. The distance from the inferior mesenteric artery to the lowest point of aorta bifurcation was 4. 27 鹵0. 8 cm, which should not be exceeded. The abdominal aorta is close to the inferior vena cava.
