【摘要】： 胰腺位于人體上腹部,毗鄰關(guān)系復(fù)雜:前面借網(wǎng)膜囊與胃相鄰,右側(cè)被十二指腸緊密包繞,左側(cè)與脾及左腎相鄰,胰頭后有膽總管通過。胰腺背側(cè)有許多重要血管配布,如:腸系膜上動靜脈、脾動靜脈、左腎動靜脈、腸系膜下靜脈、門靜脈、腹主動脈、下腔靜脈、胰十二指腸上前和上后動脈、胰十二指腸下前和下后動脈,以及分支供應(yīng)胰的胰背動脈、胰下動脈、胰大動脈等。其中,除由胰十二指腸上前動脈及胰十二指腸下前動脈組成的前動脈弓位于胰頭前方外,其余各血管均位于胰腺背側(cè)區(qū),因此,胰腺背側(cè)區(qū)一直是解剖學(xué)者及外科醫(yī)師研究的重點(diǎn)。胰腺背側(cè)區(qū)各血管走行變化多樣,相互之間的位置關(guān)系復(fù)雜,變異較大,雖然目前對該區(qū)每個(gè)血管已有詳盡的描述,但對各個(gè)血管之間的位置配布關(guān)系尚缺乏相關(guān)的解剖學(xué)資料。目前,臨床上胰腺疾病有逐漸增多的趨勢,各種治療手段也層出不窮,其中尤以手術(shù)及介入治療發(fā)展迅速,由于胰腺手術(shù)大多步驟繁瑣、難度高、風(fēng)險(xiǎn)大,一旦出現(xiàn)意外,其并發(fā)癥多較嚴(yán)重,甚至引起致命的后果,這就要求術(shù)者對胰腺的局部解剖知識有較好的掌握,為此我們展開了相關(guān)的解剖學(xué)研究。本研究通過20具經(jīng)福爾馬林固定的成人尸體標(biāo)本,對胰腺手術(shù)時(shí)所涉及的重要血管進(jìn)行了系統(tǒng)的觀測,主要觀測了重要血管的起止、走行、分支以及相互之間的位置配布關(guān)系,對測得的數(shù)據(jù)進(jìn)行科學(xué)的統(tǒng)計(jì)分析,以期對胰腺手術(shù)所涉及的重要血管有一個(gè)清晰的、系統(tǒng)的認(rèn)識,為臨床上進(jìn)行各種胰腺手術(shù)提供安全可靠的解剖學(xué)依據(jù)。
[Abstract]:The pancreas is located in the upper abdomen of the human body, and the adjacent relationship is complex: the anterior omentum sac is adjacent to the stomach, the right side is tightly surrounded by the duodenum, the left side is adjacent to the spleen and the left kidney. There is a common bile duct behind the pancreatic head. There are many important vessels in the dorsal side of the pancreas, such as: superior mesenteric arteriovenous, splenic arteriovenous, left renal arteriovenous, inferior mesenteric vein, portal vein, abdominal aorta, inferior vena cava. The anterior and superior pancreaticoduodenal artery, the anterior and inferior inferior pancreaticoduodenal artery, as well as branches supplying the dorsal pancreatic artery, the inferior pancreatic artery, the great pancreatic artery, etc. Except for the anterior arch of the anterior pancreaticoduodenal artery and the anterior inferior pancreaticoduodenal artery, the other vessels were located in the dorsal region of the pancreas. The dorsal region of pancreas has always been the focus of anatomists and surgeons. There are various changes of the blood vessels in the dorsal region of the pancreas, and the location relationship between them is complicated and the variation is great. Although there is a detailed description of each vessel in this area, there is a lack of anatomical data on the relationship between the location and distribution of each vessel. At present, there is a growing trend of pancreatic diseases in clinic. A variety of treatment methods are also emerging, especially surgery and interventional treatment rapid development, because most of the procedures of pancreatic surgery is cumbersome, difficult, high risk, once there is an accident, its complications are more serious. Even fatal consequences, which requires the operator to have a good understanding of the local anatomy of the pancreas. In this study, 20 formalin-fixed adult cadavers were used to systematically observe the important blood vessels involved in pancreatic surgery. The starting and stopping of important blood vessels, walking, branching and distribution relationship between each other were observed, and the measured data were analyzed scientifically in order to have a clear understanding of the important blood vessels involved in pancreatic surgery. Systematic understanding provides a safe and reliable anatomical basis for clinical pancreatic surgery.
