【摘要】:目的:前床突(ACP)是前、中顱窩的分界點,又是視神經(jīng)、頸內(nèi)動脈外側(cè)的保護標(biāo)志,因此,前床突有著重要的解剖意義。其位置重要,對于該區(qū)域的病變,需要考慮如何通過磨除前床突獲得手術(shù)操作空間是近幾年來研究的熱點和難點。為取得國人此方面的資料,為臨床眶上裂及海綿竇區(qū)域病變的顯微外科手術(shù)入路提供解剖學(xué)參數(shù)依據(jù),本研究通過顯微鏡下對尸體頭顱的解剖、測量,明確前床突與周圍結(jié)構(gòu)的解剖關(guān)系,并對高度個體化的骨性結(jié)構(gòu)、神經(jīng)、血管、硬膜等的解剖關(guān)系進(jìn)行分析統(tǒng)計,我們設(shè)計了此項研究。 方法:應(yīng)用10%福爾馬林充分固定的國人成人尸頭濕標(biāo)本10例20側(cè),無法確切地判定年齡和性別,為更準(zhǔn)確地區(qū)分動靜脈,維持血管正常粗細(xì)和提高拍攝質(zhì)量,所有頭顱濕標(biāo)本均在動脈系統(tǒng)灌注混有紅色染料的乳膠,靜脈系統(tǒng)灌注混有藍(lán)色染料的乳膠。漂白的國人成人顱骨干標(biāo)本15例30側(cè),供骨性結(jié)構(gòu)的觀察和測量。選擇前外側(cè)方手術(shù)最有代表性的額顳開顱(翼點)入路進(jìn)行詳細(xì)研究,在手術(shù)顯微鏡下,模擬手術(shù)入路逐層解剖,結(jié)合影像解剖學(xué),對解剖結(jié)構(gòu)進(jìn)行精確測量和拍攝,所得數(shù)據(jù)均經(jīng)SPSS處理,以平均數(shù)(測量值范圍)的形式表現(xiàn)。 結(jié)果:1.ACP前外下方為眶上裂,前外與蝶骨小翼連接,前內(nèi)與視神經(jīng)管頂部后緣及視柱相連結(jié),內(nèi)側(cè)有頸內(nèi)動脈通過,外下側(cè)有海綿竇。ACP狀如錐形,其長、寬、厚分別為9.82±1.24(7.54-12.52)mm,12.77±2.51(8.78-17.84)mm,5.69±1.39(3.36-9.74)mm?赏ㄟ^硬膜內(nèi)外聯(lián)合法磨除,注意其下緣緊貼著海綿竇外側(cè)壁內(nèi)的顱神經(jīng)。 2.床突間隙(clinoid process space)是磨除前床突后人為形成的一錐形腔隙,尖端指向后方,毗鄰組織與前床突存在時大致相同。其空間大小與前床突及周圍組織結(jié)構(gòu)的構(gòu)成、范圍和邊緣的大小有很大的關(guān)系,其中全頸內(nèi)動脈(ICA)
[Abstract]:Objective: anterior clinoid process (ACP) is the dividing point of anterior and middle cranial fossa as well as the protective marker of optic nerve and lateral internal carotid artery. Therefore the anterior clinoid process has important anatomical significance and its location is important. For the lesions in this area, it is a hot and difficult point to study how to obtain the operation space by removing the anterior clinoid process in recent years. In order to provide anatomic parameters for microsurgical approach of supraorbital fissure and cavernous sinus lesions, the anatomical relationship between anterior clinoid process and surrounding structure was determined by microscopical anatomy of cadaveric head. The anatomical relationships of osseous structures, nerves, blood vessels and dura dura were analyzed and analyzed. Methods: 10 cases and 20 sides of wet head specimens of Chinese adult cadavers which were fully fixed with 10% formalin were used. It was impossible to determine age and sex accurately, so as to distinguish artery and vein more accurately. To maintain normal vascular thickness and improve shooting quality, all head wet specimens were infused with red dyed latex in the arterial system. The venous system was infused with blue dye emulsion. Bleached adult skull specimens were collected from 15 cases (30 sides). To select the most representative frontotemporal craniotomy (pterional) approach for detailed study, under the surgical microscope, simulate the surgical approach layer by layer anatomy, combined with image anatomy. The anatomical structure was accurately measured and photographed, and the obtained data were processed by SPSS and represented as the average (range of measured values). Results 1. The superior orbital fissure was located below the anterior and outer part of ACP, the anterior and outer part was connected with the pterygoid of the sphenoid bone, the anterior and outer part was connected with the apical and posterior edge of the optic canal and the optic column, the internal carotid artery passed through the medial side. The lateral cavernous sinus. ACP shape is conical, its length, width and thickness are 9.82 鹵1.247.54-12.52 mm respectively. 12.77 鹵2.51g 8.78-17.84mm. 5.69 鹵1.39n3.36-9.74mmm. it can be milled by the combination of epidural and extradural methods. Note that the lower margin is close to the cranial nerve in the lateral wall of the cavernous sinus. 2. Clinoid process space is a conical space formed by grinding the anterior and posterior clinoid processes, with the tip pointing to the rear. The size of the adjacent tissue is similar to that of the anterior clinoid process, and the size of the space is closely related to the structure of the anterior clinoid process and the surrounding tissue, the range and the size of the margin, in which the total internal carotid artery (ICA)
