【摘要】:目的:通過測定血清胰島素樣生長因子I(IGF-I)及胰島素樣生長因子結(jié)合蛋白3(IGFBP-3),探討IGF-I、IGFBP-3與胎兒宮內(nèi)生長發(fā)育和生后生長追趕的關(guān)系及其影響因素。方法:選自2004年12月至2006年2月在我院住院的新生兒病例,共64例,分為兩組:①觀察組:為宮內(nèi)發(fā)育遲緩(intrauterine growthretardation,IUGR)兒,共39例,胎齡在32+1周至41周,其中足月兒28例,早產(chǎn)兒11例。②對照組:為適于胎齡(appropriatefor gestational age,AGA)兒,共25例,胎齡在34周至40+3周,其中足月兒16例,早產(chǎn)兒9例。收集兩組患兒相關(guān)病史,出生時(shí)情況,并對觀察組于生后6~12個(gè)月進(jìn)行隨訪,分別于生后7天及隨訪時(shí)采集靜脈血3毫升,以3000RPM離心取血清于-20℃冰箱冰凍待檢,應(yīng)用酶聯(lián)免疫吸附方法(ELISA)測定標(biāo)本IGF-I和IGFBP-3的濃度。結(jié)果:觀察組新生兒靜脈血IGF-I水平(25.15±1.19ng/dl)及IGFBP-3水平(1.91±0.67ng/dl)明顯低于對照組(27.20±8.66)和(6.40±1.25),同一指標(biāo)兩組間比較差異有顯著性(P0.05);其中早產(chǎn)兒IGF-I水平(28.65±9.81ng/dl)和IGFBP-3水平(1.45±0.26ng/dl)明顯低于足月兒(41.77±10.58ng/dl)和(4.10±1.29ng/dl),同一指標(biāo)兩者比較差異顯著(P0.05);IGFBP-3與IGF-I呈顯著性正相關(guān)(P0.001);IGF-I、IGFBP-3與出生體重、身長和頭圍呈正相關(guān)(P0.01);母親有妊娠高血壓疾病史與無有妊娠高血壓疾病史者比較,其新生兒血清IGF-I和IGFBP-3水平均降低(P0.05);胎兒有臍帶繞頸史及宮內(nèi)窘迫或窒息史者血清
[Abstract]:Objective: to study IGF-I by measuring serum insulin-like growth factor (IGF-I) and insulin-like growth factor-binding protein (IGFBP-3). The relationship between IGFBP-3 and fetal intrauterine growth and postnatal growth catch-up and its influencing factors methods: from December 2004 to February 2006, the neonatal cases in our hospital were selected. 64 cases were divided into two groups: intrauterine intrauterine growth rate (IUGR) group (39 cases). The gestational age ranged from 321 weeks to 41 weeks, of which 28 were term infants. There were 11 premature infants (n = 11) and 25 cases (n = 25) in the control group, which were suitable for gestational age and approved for gestational age AGA (n = 25). The gestational age ranged from 34 weeks to 403 weeks, including 16 full-term infants and 9 premature infants. The related history and birth time of the two groups were collected, and the observation group was followed up from 6 to 12 months after birth.
