【摘要】：醫(yī)患溝通在醫(yī)患關(guān)系中有著重要的作用,因此醫(yī)患雙方都非常注重交際策略的選擇,其中就包括模糊限制語的使用。本文以100例真實(shí)的醫(yī)患溝通音頻語料為研究對象。語料由華僑大學(xué)部分在校大學(xué)生志愿者利用2013-2014學(xué)年度的寒假,使用手機(jī)、錄音筆等設(shè)備在國內(nèi)不同醫(yī)院、診所錄制而成,并由收集者根據(jù)音頻轉(zhuǎn)寫,筆者進(jìn)行校對。本文采用定量和定性相結(jié)合的研究方法,借助ROST內(nèi)容挖掘軟件5.0版本,并以(不)禮貌理論為理論框架,對語料中出現(xiàn)的醫(yī)患模糊限制語的分布頻率及其作為(不)禮貌策略的語用功能進(jìn)行分析。本文著重探討以下四個(gè)問題:(1)醫(yī)患門診溝通中模糊限制語如何分布?有何特點(diǎn)?(2)從(不)禮貌角度,醫(yī)患門診溝通中交際者是如何使用模糊限制語的?(3)患者(含家屬)是否有可能實(shí)施不禮貌言語行為?如果是,為什么?(4)醫(yī)患雙方在不禮貌言語行為的回應(yīng)策略方面有什么差異?研究發(fā)現(xiàn):(1)醫(yī)患雙方出于不完全相同的動(dòng)機(jī)都會(huì)經(jīng)常使用模糊限制語,比如“一點(diǎn)”“很”;(2)模糊限制語的使用具有雙重功能:一方面,模糊限制語是醫(yī)患雙方用于實(shí)施禮貌策略以維護(hù)對方面子、建立良好醫(yī)患關(guān)系的語言表達(dá)手段;另一方面,模糊限制語的使用既可直接表達(dá)對聽話人的不禮貌態(tài)度,也可在醫(yī)患雙方表明不同意見、威脅對方面子時(shí)起強(qiáng)化作用;(3)醫(yī)生是不禮貌話語的主要發(fā)起者,而患者有時(shí)也使用模糊限制語干涉醫(yī)生的診治活動(dòng),表達(dá)自己的不同意見,威脅醫(yī)生的面子;(4)醫(yī)生既采用攻擊性策略也采用防衛(wèi)性策略來回應(yīng)不禮貌,但是患者通常不作回應(yīng)或接受對方的不禮貌或偶爾采用防衛(wèi)性策略來回應(yīng),極少使用攻擊性回應(yīng)策略。本文對醫(yī)患溝通中模糊限制語的語用分析具有多方面的意義。從理論上來說,模糊限制語可作為不禮貌策略的語用功能,是對Brown和Levinson(1987)將模糊限制語視為消極禮貌策略的補(bǔ)充,并且增強(qiáng)了Culpeper(1996,2005)不禮貌框架理論的解釋力和適用面。從實(shí)踐上來說,本研究所收集到的醫(yī)患門診溝通語料可以豐富醫(yī)患溝通研究的語料范圍;最后但同樣重要的是,本研究有助于醫(yī)患雙方了解模糊限制語在溝通過程中的重要地位,從而改善雙方交際策略,促進(jìn)醫(yī)患關(guān)系。
[Abstract]:Doctor-patient communication plays an important role in the doctor-patient relationship, so both doctors and patients attach great importance to the choice of communication strategies. Among them is the use of hedges. In this paper, 100 cases of real doctor-patient communication audio data were studied. The data were used by some college students volunteers in overseas Chinese University to use mobile phones during the winter vacation in 2013-2014. The recording pen and other equipment are recorded in different hospitals and clinics in China and proofread by the collector according to the audio frequency. This paper adopts the method of combining quantitative and qualitative research, and makes use of the 5.0 version of ROST content mining software. And take the (no) politeness theory as the theoretical framework, This paper analyzes the distribution frequency of doctor-patient hedges in the corpus and its pragmatic function as a politeness strategy. This paper focuses on the following four questions: 1) how to distribute hedges in doctor-patient outpatient communication? What are the characteristics? (2) from the point of view of politeness, how do communicators use hedges in doctor-patient clinic communication? Is it possible for a patient (including a family member) to perform an impolite speech act? If so, why? What are the differences between doctors and patients in response strategies to impolite speech acts? The study found that both doctors and patients often use vague hedges for different motivations, such as "a little" and "a bit".) the use of vague hedges has a dual function: on the one hand, Hedges are language expressions used by both doctors and patients to implement politeness strategies to protect the face of each other and to establish good doctor-patient relations. On the other hand, the use of hedges can directly express the impolite attitude towards the hearer. Doctors are also the main initiators of impolite words, and patients sometimes use hedges to interfere with doctors' diagnosis and treatment activities and express their different opinions. Doctors use both offensive and defensive strategies to respond to impoliteness, but patients often do not respond or accept the other's impoliteness or occasionally respond with defensive strategies. Aggressive response strategies are rarely used. This paper has many implications for pragmatic analysis of hedges in doctor-patient communication. In theory, hedges can be used as pragmatic functions of impolite strategies. It is a supplement to Brown and Levinsonian's view of hedges as a negative politeness strategy, and enhances the explanatory power and application of the theory of impoliteness framework. The data collected in this study can enrich the scope of the research on doctor-patient communication. Last but not least, this study helps both doctors and patients to understand the important role of hedges in the communication process. In order to improve the communication strategies and promote the relationship between doctors and patients.
