【摘要】：研究背景:隨著社會和經(jīng)濟的發(fā)展,人類平均壽命普遍延長,人口老齡化日益明顯,在經(jīng)濟、文化、人體素質等方面給人類社會帶來了一定的影響,并隨著年齡的增大,老年人機體各重要器官的生理功能和免疫功能等有不同程度的衰退,使老年人身體虛弱、精神不振、一人多病、易發(fā)生并發(fā)癥、藥物不良反應多,導致老年人晚年生活質量下降。因而老齡化已成為全世界關注的時尚話題,關于如何實現(xiàn)老年保健及其疾病治療的討論與研究,是我們當代廣大醫(yī)護人員及社會各界人士不容忽視的重任。研究目的:本研究運用藏醫(yī)按摩等外治(溫治)療法對“隆”性老年人保健及其疾病治療具有最佳療效為理論依據(jù),研究老年保健與疾病治療的最佳外治療法作為主要研究目的。首先通過藏、西醫(yī)文獻研究與臨床調(diào)查,了解“老年”的含義、老年保健與疾病治療的總體方案和思路;其次通過三種不同類型的臨床調(diào)查研究,了解臨床上發(fā)病率最高的常見老年病及治療方案,調(diào)查放血、溫灸、熱敷等五種藏醫(yī)特殊外治療法對老年保健與疾病治療效果最佳的治療方法;最后通過基本理論及臨床技能兩方面介紹老年保健與疾病的外治療法及其治療依據(jù),特殊臨床操作規(guī)程為目的,以臨床上老年人群發(fā)病率最高的一例典型疾病為例,說明此病的外治療法、治療依據(jù)及其特殊臨床操作,總結老年保健與疾病治療的最佳外治療法。總之,研究藏醫(yī)外治對老年保健與疾病治療的方法,提高老年人晚年的生活質量為主要研究目的,并補充現(xiàn)階段藏醫(yī)學對老年的認知及其治療方法為奮斗目標。研究方法:本研究采取了理論研究與臨床研究相結合的方法,綜合運用了文獻研究和臨床調(diào)查、問卷調(diào)查、實地考察、疑難問題討論等研究方法。查閱了《月王藥診》、《傷寒論》、《西醫(yī)略論》等古今藏、中、西醫(yī)學經(jīng)典書籍、論著、期刊等相關文獻進行整理和分析,將藏醫(yī)五種特色外治療法對老年保健與疾病療法的新思維為側重點,對以上相關文獻進行相互比較、分析、歸納、總結,進一步解析“老年”之詞的含義、老年保健與疾病治療的總體治療方案及其治療思維;實地考察了青海省藏醫(yī)院、西藏自治區(qū)藏醫(yī)院、西藏山南地區(qū)藏醫(yī)院、UCLA Medical Plaza(美國洛杉磯加州大學附屬醫(yī)院)和Santa Monika KEN EDWARDS CENTER(三塔莫尼卡海邊養(yǎng)老院)等國內(nèi)外先進的醫(yī)院和養(yǎng)老院,通過病歷查閱、問卷調(diào)查、疑難問題討論等三種研究方法,調(diào)查了發(fā)病率最高的常見老年病及其治療方案,總結了在臨床上運用哪些老年保健與疾病治療的藏醫(yī)外治療法及其特殊臨床操作方法等。研究結果:文獻研究的主要結果包括:1.對“老年”之詞的解析方面,藏醫(yī)學認為“老年”是人體的五源和精、氣、神三要素的功能逐漸下降,七大正精和消化功能、三身、五官和主感覺運動等功能減退,導致身體虛弱、器官功能減退、易患病等;2.老年人年齡劃分標準方面,《四部醫(yī)典》云:“從出生到十六歲為童年,從童年后直至七十歲,則機體正精、官竅、精光、體力等都充盈發(fā)達,稱為成年,正精開始衰退稱老年”,這與現(xiàn)代世界衛(wèi)生組織新規(guī)劃的老年人年齡標準相符合,且與當今老年人社會生活的實際情況相符合;3.衰老的表現(xiàn)和治則方面,藏醫(yī)人體7種體質中,老年人屬“隆性體質”,所以衰老的癥狀大多表現(xiàn)有“隆”偏升或紊亂而引起的身體虛弱、器官功能衰退等表現(xiàn);4.老年保健與疾病治療的思路方面,因為老年人屬“隆性體質”,所以老年保健與疾病治則始終考慮治療“隆”病的飲食起居、藥物治療及外治治療;5.老年保健與疾病治療的藏醫(yī)外治療法方面,在《四部醫(yī)典》、《月王藥診》和《杜德桑都忠邁》等藏醫(yī)經(jīng)典著作及相關文獻記載有藏醫(yī)放血、溫灸、熱敷、藥浴及按摩療法,并詳細說明對老年保健與疾病治療有各自的特殊操作方法。臨床調(diào)查研究的主要結果包括:1.藏區(qū)發(fā)病率最高的常見老年病方面,本研究調(diào)查結果顯示藏區(qū)老年人發(fā)病率最高的常見病有:白脈病、腰椎間盤突出、關節(jié)炎、高血壓、心臟病和消化不良六種;2.臨床上對老年保健與疾病治療常用的藏醫(yī)外治方法有五種療法,即藏醫(yī)放血、溫灸、熱敷、藥浴和按摩療法;3.臨床醫(yī)生對老年保健與疾病開展治療過程中外治療法運用率最高,并且老年患者評價最好的外治療法主要有藏醫(yī)溫灸、熱敷及按摩療法3種;4.藏醫(yī)外治的療效方面,(1)從治療方式看,內(nèi)服藏藥和外治綜合治療的療效高于口服藏藥的單一療效;(2)從年齡分布看,較為年輕老人的療效尤為明顯;(3)從治療項目和療程看,較少(1-2項)的外治項目分段治療在較長(2-3)的療程(1個療程一般為7天),此療效高于較多(3-4項)的外治治療項目完成在較短(1-2)的療程。5.藏醫(yī)外治的特殊療法方面,因老年人特殊生理和體質,藏醫(yī)外治對老年保健與疾病治療的方法不同與成年人群,在老年保健與疾病治療時,藏醫(yī)五大外治療法有各自的特殊操作方法。
[Abstract]:Background: with the development of society and economy, the average life span of human beings is generally prolonged, and the aging of the population is becoming more and more obvious. It has brought a certain influence on the human society in the aspects of economy, culture and human body. With the increase of age, the physiological function and immune function of the important organs of the old people have different degrees of decline. Elderly people are weak, deformed, one person is ill, one is prone to complications, and there are many adverse drug reactions, which lead to a decline in the quality of life in the old age. Therefore, aging has become a popular topic of concern all over the world. The discussion and Research on how to realize the health care and the treatment of disease is the vast majority of our medical staff and the community of society. The purpose of this study is to study the purpose of the study. This study is based on the theoretical basis for the best therapeutic effect of Tibetan massage (Wen Zhi) therapy on the health of "long" old people and the treatment of disease. The best external treatment for the elderly health care and disease treatment is the main purpose of the study.
