【摘要】：中國(guó)嚴(yán)峻的人口老齡化趨勢(shì)引起了國(guó)家的重視,政府相關(guān)部門(mén)頻頻通過(guò)出臺(tái)相關(guān)政策以應(yīng)對(duì)老年人不斷增長(zhǎng)的照護(hù)需求,而疾病譜的改變、家庭結(jié)構(gòu)的變化以及人類(lèi)主體意識(shí)的增強(qiáng)等問(wèn)題也引發(fā)了各界專(zhuān)家人士不斷探索應(yīng)對(duì)方法。經(jīng)國(guó)內(nèi)外有關(guān)地區(qū)成功的實(shí)踐經(jīng)驗(yàn)表明,臨終關(guān)懷作為一種旨在滿足臨終者終末期的照護(hù)需求、提升其生命質(zhì)量,及協(xié)助家屬度過(guò)沮喪期的全面照護(hù)手段,能夠有效應(yīng)對(duì)上述問(wèn)題的出現(xiàn),應(yīng)該得到廣泛的應(yīng)用及推廣。據(jù)研究,深圳現(xiàn)有的臨終關(guān)懷服務(wù)無(wú)法滿足臨終者的照護(hù)需求,雖然還未進(jìn)入人口老齡化階段,但隨著腫瘤等慢性疾病的蔓延、四二一家庭的增多以及隨遷老人數(shù)量的增加等情況的出現(xiàn),臨終照護(hù)的需求將越來(lái)越大,迫切需要有志人士通過(guò)探索、研究、學(xué)習(xí)相關(guān)地區(qū)臨終關(guān)懷服務(wù)的成功經(jīng)驗(yàn)來(lái)有效發(fā)展本土臨終關(guān)懷服務(wù)。本研究采用定量與定性相結(jié)合的混合研究方法,主要通過(guò)文獻(xiàn)研究、比較分析了解國(guó)內(nèi)外各地臨終關(guān)懷服務(wù)的發(fā)展現(xiàn)狀及成功經(jīng)驗(yàn),通過(guò)政策分析掌握深圳市居家養(yǎng)老及老齡服務(wù)等相關(guān)政策,通過(guò)問(wèn)卷調(diào)查、訪談及實(shí)地調(diào)查來(lái)了解深圳市的臨終關(guān)懷服務(wù)現(xiàn)狀及存在的問(wèn)題。經(jīng)研究發(fā)現(xiàn),深圳臨終關(guān)懷服務(wù)存在諸如政府不重視、缺乏政策支持、本土化臨終關(guān)懷服務(wù)模式缺失、臨終關(guān)懷機(jī)構(gòu)運(yùn)轉(zhuǎn)資金匱乏、服務(wù)人員專(zhuān)業(yè)素質(zhì)低下、社會(huì)資源欠缺整合等問(wèn)題,為有效應(yīng)對(duì)上述問(wèn)題,筆者有針對(duì)性地提出探索建立長(zhǎng)期醫(yī)療保險(xiǎn)制度、建立社區(qū)——家庭照護(hù)的臨終關(guān)懷形式,運(yùn)用社會(huì)工作手法——個(gè)案管理來(lái)統(tǒng)籌服務(wù)等相關(guān)建議,以此來(lái)推進(jìn)深圳市臨終關(guān)懷服務(wù)在理論和實(shí)踐層面的發(fā)展。
[Abstract]:China's austere aging population has attracted national attention, with government departments frequently adopting policies to cope with the growing care needs of the elderly and changing the disease spectrum. The changes in family structure and the enhancement of human subjective consciousness have also led experts from all walks of life to explore ways to deal with the problem. The successful practical experience in the relevant regions at home and abroad has shown that, As a comprehensive care means to meet the end-stage care needs, improve the quality of life, and assist the family members through the depression period, hospice care can effectively deal with the above problems. It should be widely used and popularized. According to research, the existing hospice care services in Shenzhen cannot meet the care needs of the terminally ill, although they have not yet entered the stage of population aging, but with the spread of chronic diseases such as cancer, With the increase of 421 families and the increase in the number of elderly people moving away, the demand for hospice care will become greater and greater. It is urgent for aspiring people to explore and study. In order to develop the local hospice service effectively by learning the successful experience of the hospice service in the relevant area, this study adopts a mixed method of quantitative and qualitative research, mainly through the literature study. To compare and understand the current situation and successful experiences of hospice care services at home and abroad, to master the related policies such as home pension and aging services in Shenzhen through policy analysis, and to investigate the related policies through questionnaires. Interviews and field investigations were conducted to understand the current situation and problems of hospice care services in Shenzhen. It was found that there are such problems as government neglect, lack of policy support, and lack of local hospice care service mode in Shenzhen. The running funds of hospice care institutions are short of funds, the professional quality of service personnel is low, the social resources are not integrated and so on. In order to effectively deal with the above problems, the author puts forward to explore the establishment of long-term medical insurance system. In order to promote the development of hospice care in Shenzhen City, we should establish the form of hospice care for community and family care, and use social workmanship-case management to coordinate the service and other related suggestions in order to promote the development of hospice care in Shenzhen.
