【摘要】：目的:評(píng)價(jià)三種不同運(yùn)動(dòng)模式的鎳鈦器械聯(lián)合熱牙膠充填技術(shù)用于下頜后牙一次法根管治療的近期療效觀察。方法:將375名于我院就診年齡為19~68歲且被診斷為慢性根尖周炎的漢族患者納為研究對(duì)象,共375顆下頜后牙(包括第一、二前磨牙及第一、二磨牙),隨機(jī)分為3組:WaveOne往復(fù)運(yùn)動(dòng)組,ProTaper Universal連續(xù)旋轉(zhuǎn)運(yùn)動(dòng)組和Twisted File Adaptive自適應(yīng)運(yùn)動(dòng)組,每組125例研究對(duì)象。所有研究對(duì)象均采用冠根向法進(jìn)行根管預(yù)備,3%次氯酸鈉超聲蕩洗根管,干燥根管,熱牙膠垂直加壓技術(shù)進(jìn)行根管恰填。比較三組鎳鈦器械根管預(yù)備平均操作時(shí)間;分析一次法根管治療術(shù)后24h、72h及7d的疼痛情況;評(píng)價(jià)三組患者術(shù)后3個(gè)月的近期效果。結(jié)果:WaveOne組根管預(yù)備平均操作時(shí)間為(2.14±1.07)min,Twisted File Adaptive組根管預(yù)備平均操作時(shí)間為(3.08±1.19)min,ProTaper Universal組根管預(yù)備平均操作時(shí)間為(4.03±1.73)min,三組鎳鈦器械根管預(yù)備平均操作時(shí)間差異有統(tǒng)計(jì)學(xué)意義(P0.001);術(shù)后24h及72h三組行一次法根管治療術(shù)后疼痛率差異有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后7d三組術(shù)后疼痛率無(wú)統(tǒng)計(jì)學(xué)差異(P0.05);比較三組一次法根管治療術(shù)后3個(gè)月的有效率差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:WaveOne組進(jìn)行根管預(yù)備時(shí)間最短,能夠提高預(yù)備效率;Twisted File Adaptive組進(jìn)行根管預(yù)備,可明顯降低術(shù)后疼痛反應(yīng);WaveOne組和Twisted File Adaptive組術(shù)后3個(gè)月進(jìn)行療效評(píng)價(jià),近期臨床效果顯著。新型鎳鈦器械進(jìn)行根管預(yù)備,對(duì)于感染根管而言可實(shí)現(xiàn)一次法根管治療,對(duì)臨床推廣具有一定意義。
[Abstract]:Objective: to evaluate the short-term curative effect of three different movement-mode nickel-titanium instruments combined with hot gum filling technique in the treatment of mandibular posterior teeth with one-time root canal. Methods: a total of 375 mandibular posterior teeth (including first, second premolar and first premolar) were selected from 375 Han nationality patients aged 1968 and diagnosed as chronic periapical periodontitis. Two molars were randomly divided into three groups: ProTaper Universal continuous rotation group and Twisted File Adaptive adaptive motion group. All the subjects were used to prepare the root canal by the method of crown and root direction. The root canal was washed by ultrasonic with 3% sodium hypochlorite, the root canal was dried and the root canal was filled with hot gum vertical pressure technique. The mean operating time of root canal preparation was compared among the three groups, the pain at 24 hours and 7 days after root canal treatment was analyzed, and the short-term effect of 3 months after operation was evaluated in the three groups. Results the mean operating time of root canal preparation was (2.14 鹵1.07) min Twisted File Adaptive group (3.08 鹵1.19) min ProTaper Universal group was (4.03 鹵1.73) min, there was significant difference among the three groups (P0.001). There was significant difference in postoperative pain rate between the three groups at 24 hours and 72 hours after treatment (P0.05), but there was no significant difference in postoperative pain rate in the three groups on the 7th day after operation (P0.05). There was no significant difference in the effective rate between the three groups after 3 months of once root canal therapy (P0.05). Conclusion the root canal preparation time of the 1: WaveOne group is the shortest, which can improve the preparation efficiency. The root canal preparation in the Twisted File Adaptive group can significantly reduce the postoperative pain response and evaluate the curative effect 3 months after operation in both the WaveOne group and the Twisted File Adaptive group. The short-term clinical effect is remarkable. A new type of Nickel-titanium instrument for root canal preparation can be used for the treatment of root canal infection, which is of significance for clinical application.
