【摘要】：目的:探索推拿結(jié)合下頸段抗阻運動治療頸型頸椎病的臨床療效以及遠期效果。闡明在頸型頸椎病的治療以及療效的維持方面,推拿結(jié)合下頸段抗阻運動具有顯著的意義,為頸型頸椎病的預(yù)防及治療方面提供新穎的策略和操作方法。方法:本研究以黑龍江中醫(yī)藥大學附屬第一醫(yī)院推拿科門診所選擇60名頸型頸椎病患者為實驗研究對象,隨機數(shù)字表法平均分為治療組與對照組。治療組進行頸部推拿結(jié)合下頸段抗阻運動訓練治療,對照組采用常規(guī)推拿法。應(yīng)用視覺模擬量表、頸椎功能障礙指數(shù)和顱頸角測量對兩組治療前、治療后以及隨訪時的相關(guān)癥狀和體征進行評分并作比較。結(jié)果:1.接受治療后,治療組與對照組的視覺模擬量表評分、頸椎功能障礙指數(shù)均有改善(P0.05),但治療組改變更顯著,結(jié)果優(yōu)于對照組(P0.05)。2.兩組患者在治療后CV angle有顯著差異(P0.05),治療組優(yōu)于對照組。3.全部病例獲得2個月隨訪,治療組無明顯進展病例,對照組2例進展為神經(jīng)根型頸椎病;兩組間VAS評分與NDI指數(shù)均有統(tǒng)計學差異,治療組均優(yōu)于對照組(P0.05)。結(jié)論:1.推拿結(jié)合下頸段抗阻運動治療頸型頸椎病能夠較好的改善患者的臨床癥狀、客觀體征;緩解患者的疼痛和頸部功能障礙。應(yīng)用該方法所得效果明顯優(yōu)于傳統(tǒng)推拿治療組。2.推拿結(jié)合下頸段抗阻運動治療頸型頸椎病能更明顯的糾正患者的錯誤體態(tài);與傳統(tǒng)推拿組對比,前者更趨于正常。3.推拿結(jié)合下頸段抗阻運動治療頸型頸椎病擁有更顯著的遠期療效;與傳統(tǒng)推拿組對比,該方法治療的患者于1個月、2個月的隨訪時頸部疼痛程度較輕、功能障礙較少。
[Abstract]:Objective: to explore the clinical effect and long-term effect of massage combined with lower cervical resistance movement in the treatment of cervical spondylosis. In the treatment of cervical spondylopathy and the maintenance of curative effect, massage combined with lower cervical resistance movement has significant significance, which provides novel strategies and operational methods for the prevention and treatment of cervical spondylosis. Methods: 60 patients with cervical spondylosis were randomly divided into treatment group and control group. The treatment group was treated with cervical massage combined with lower cervical resistance exercise training, while the control group was treated with conventional massage. Visual analogue scale, cervical vertebrae dysfunction index and craniocervical angle were used to evaluate and compare the symptoms and signs before, after and after treatment in the two groups. The result is 1: 1. After treatment, the visual analogue scale score and cervical spine dysfunction index of the treatment group and the control group were improved (P0.05), but the change of the treatment group was more significant, the result was better than that of the control group (P0.05). 2. There was significant difference in CV angle between the two groups after treatment (P0.05), the treatment group was better than the control group. 3. 3. All cases were followed up for 2 months. There was no significant progress in the treatment group and 2 cases in the control group. There were significant differences in VAS score and NDI index between the two groups and the treatment group was superior to the control group (P0.05). Conclusion 1. The treatment of cervical spondylosis with massage combined with lower cervical resistance can improve the clinical symptoms, objective signs and relieve the pain and cervical dysfunction. The effect of this method is obviously better than that of the traditional massage treatment group. 2. 2. The treatment of cervical spondylopathy with massage combined with lower cervical segment resistance movement can significantly correct the wrong posture of the patients, and compared with the traditional massage group, the former tends to be normal. 3. The treatment of cervical spondylopathy with massage combined with lower cervical segment resistance exercise has a more significant long-term effect, compared with the traditional massage group, the patients treated by this method at 1 month, 2 months follow-up, the degree of neck pain is less, and the dysfunction is less.
