【摘要】：文章對社區(qū)居民在就診選擇方面考慮的主要因素和行為傾向做了調(diào)查和探討,對于醫(yī)務(wù)人員、醫(yī)療過程及醫(yī)療設(shè)備及環(huán)境三部分和居民就診意愿的關(guān)系,針對三種因素對就診選擇的具體關(guān)系得出分析結(jié)論,針對結(jié)論有層次的給出基層居民引流的建議。通過文獻(xiàn)研究方法和問卷調(diào)查方法,對基層居民在就診選擇上進(jìn)行研究;在樣本的基本情況和相關(guān)政策認(rèn)知情況方面,利用描述性統(tǒng)計方法進(jìn)行統(tǒng)計和分析;將經(jīng)過反復(fù)論證的分類問卷進(jìn)行梳理,擇適當(dāng)?shù)淖兞拷⒔Y(jié)構(gòu)方程模型,對影響因素進(jìn)行分析。經(jīng)過分析得出結(jié)論,社區(qū)居民自身的基本情況對其選擇就診機(jī)構(gòu)影響顯著;基層相關(guān)機(jī)構(gòu)自身造血能力的提升是指引社區(qū)居民就診流向的根本;與醫(yī)務(wù)人員和醫(yī)療過程相比,醫(yī)療設(shè)備及環(huán)境對社區(qū)居民就診意愿影響更大。醫(yī)療設(shè)備與環(huán)境部分中,藥品報銷比例等相關(guān)制度如果完美落實,對就診的社區(qū)居民在選擇就醫(yī)機(jī)構(gòu)中能夠選擇社區(qū)就診有著正面的影響。因此從三方面給出了對應(yīng)的建議:一是從居民需求方面,加強(qiáng)在慢性病預(yù)防及治療普及上做好知識普及,依據(jù)居民的具體情況,適當(dāng)開放上門服務(wù),方便本社區(qū)居民居家養(yǎng)老和慢性病照看;二是從社區(qū)建設(shè)方面,在完善首診制和雙向轉(zhuǎn)診制的實施和普及的前提下,提升機(jī)構(gòu)長期的技術(shù)水平和醫(yī)療設(shè)備;三是從政府幫扶方面,在實施零差藥品的前提下,保證藥品的類別覆蓋全面和細(xì)化,繼續(xù)完善基本醫(yī)療制度在社區(qū)醫(yī)療方面的報銷傾斜,真正實現(xiàn)引流作用。
[Abstract]:This paper investigates and discusses the main factors and behavioral tendencies considered by community residents in the selection of medical treatment, and discusses the relationship between the three parts of medical staff, medical process, medical equipment and environment, and residents' willingness to seek medical treatment. According to the analysis of the relationship between the three factors and the selection of medical treatment, the conclusions are analyzed, and the suggestions of drainage for grass-roots residents are given in the light of the conclusions. Through the methods of literature research and questionnaire survey, this paper makes a study on the selection of medical treatment for grass-roots residents, and makes use of descriptive statistical methods to carry out statistics and analysis on the basic situation of samples and related policy cognition. The structure equation model is established by sorting out the classification questionnaire which has been proved repeatedly, and the influencing factors are analyzed. After analysis, it is concluded that the basic situation of community residents has a significant impact on their choice of medical institutions; the improvement of hematopoietic ability of grass-roots related institutions is the basis to guide the flow of community residents' visits; compared with medical personnel and medical process, Medical equipment and environment have greater influence on community residents' willingness to seek medical treatment. In the part of medical equipment and environment, if the related system such as the proportion of drug reimbursement is implemented perfectly, it has a positive effect on the community residents who can choose the community visit in the medical institution. Therefore, the corresponding suggestions are given from three aspects: first, in terms of the needs of the residents, we should strengthen the popularization of knowledge on the prevention and treatment of chronic diseases, and properly open the door service according to the specific conditions of the residents. The second is to improve the long-term technical level and medical equipment of the institution under the premise of perfecting the implementation and popularization of the first consultation system and the two-way referral system, which is convenient for the residents in this community to live in their homes and care for chronic diseases, and second, to improve the implementation and popularization of the first consultation system and the two-way referral system in the community. Third, from the government support, under the premise of the implementation of homodyne drugs, to ensure the comprehensive and detailed coverage of the drug category, continue to improve the basic medical system in community medical reimbursement tilt, truly achieve the role of drainage.
