血液病患者出血程度與血小板微顆粒、血管性血友病因子、纖維連接蛋白的關系及臨床意義

【摘要】:目的觀察血小板微顆粒(Platelet microparticle,PMP)、血管性血友病因子(von Willebrand Factor,v WF)、纖維連接蛋白(Fibronecti,FN)在血小板減少的血液病患者中與出血程度的關系;探討PMP、v WF、FN對出血程度預測的臨床意義.方法病例均采集于2016年1月~2016年6月安徽醫(yī)科大學第一附屬醫(yī)院血液內科血小板計數(shù)20×109/L的患者90人作為試驗組,所有疾病的診斷均符合張之南主編的《血液病診斷及療效標準》中的診斷標準,參照WHO關于出血的分級標準,根據(jù)試驗組患者出血程度分為0分組、1分組、2分組、3分組、4分組。以同時期我院體檢中心體檢人50例為對照組,試驗組與對照組在年齡,性別方面均無統(tǒng)計學差異。所有試驗組及對照組人群均無心腦血管疾病,試驗前2周內未服用抗凝及抗血小板藥物,未進行輸血治療。應用流式細胞術(FCM)檢測患者外周血中血小板微顆粒含量變化,應用酶聯(lián)免疫吸附測定法(ELISA)檢測患者外周血中血管性血友病因子、纖維連接蛋白含量變化。結果(1)各組血小板微顆粒、血管性血友病因子、纖維連接蛋白的含量均有顯著差異(P0.001)(2)不同出血程度組之間出血程度越重血小板微顆粒含量越低(1分組與2分組;3分組、4分組和對照組無明顯統(tǒng)計學差異,其余有統(tǒng)計學差異)、血管性血友病因子含量越高(除1分組和2分組外,其余各組之間差異均有顯著統(tǒng)計學意義)纖維連接蛋白含量越高(除0分組與1分組外,其余各組均有顯著統(tǒng)計學差異)。結論血小板微顆粒、血管性血友病因子、纖維連接蛋白的含量對血小板減少的血液病患者出血程度的預測具有臨床意義。
[Abstract]:Objective to investigate the relationship between platelet microparticle PMPN, von Willebrand Factorv WFN, fibronectidine FN in patients with thrombocytopenia, and the clinical significance of PMPV WFFN in predicting the degree of hemorrhage in patients with thrombocytopenia. Methods from January 2016 to June 2016, 90 patients with 20 脳 109 / L platelet count in Department of Hematology, first affiliated Hospital of Anhui Medical University, were selected as trial group. The diagnosis of all diseases was in accordance with the criteria of diagnosis and curative effect of Hematological Diseases edited by Zhang Zhinan. According to the WHO classification standard of bleeding, the patients in the experimental group were divided into 0 groups, 1 group, 2 groups, 2 groups, 3 groups, 4 groups. There were no significant differences in age and sex between the test group and the control group, taking 50 cases of physical examination in the same period as the control group. No cardiovascular and cerebrovascular diseases were found in all the experimental and control groups. No anticoagulant and antiplatelet drugs were taken within 2 weeks before the trial. Flow cytometry (FCM) was used to detect the changes of platelet microparticles in peripheral blood, and enzyme linked immunosorbent assay (Elisa) was used to detect the changes of von Willebrand factor and fibronectin in peripheral blood. Results (1) Platelet microgranule, von Willebrand factor, There were significant differences in the contents of fibronectin (P 0.001) the more severe the degree of hemorrhage was, the lower the platelet microparticle content was. There was no significant difference between group 1 and group 2, group 3, group 4, group 4 and control group. There were statistical differences in the other groups. The higher the content of von Willebrand factor (except group 1 and group 2, the difference between the other groups were statistically significant), the higher the content of fibronectin (except group 0 and group 1). There were significant statistical differences in other groups. Conclusion the contents of platelet microparticles, von Willebrand factor and fibronectin have clinical significance in predicting the bleeding degree of patients with thrombocytopenia.
