公立醫(yī)院組織文化對(duì)綜合績(jī)效的影響研究

【摘要】:我國(guó)非常重視文化領(lǐng)域的大發(fā)展、大繁榮,作為社會(huì)主義文化的一個(gè)重要分支,醫(yī)院文化已不只是一個(gè)概念存在,而是現(xiàn)代醫(yī)院管理的新趨勢(shì)。目前公立醫(yī)院改革已進(jìn)入攻堅(jiān)克難的階段,如何在遵從新醫(yī)改整體目標(biāo)基礎(chǔ)上實(shí)現(xiàn)自身綜合績(jī)效的提高和可持續(xù)發(fā)展,如何正確發(fā)揮文化的助推作用以促進(jìn)改革進(jìn)程,是迫切需要解決的問(wèn)題。醫(yī)療衛(wèi)生領(lǐng)域?qū)ξ幕c績(jī)效關(guān)系的研究不多,且大部分的研究從社會(huì)效益或經(jīng)濟(jì)效益單方面著手,進(jìn)行綜合績(jī)效研究的很少。本研究旨在剖析公立醫(yī)院組織文化對(duì)綜合績(jī)效的影響,探究醫(yī)院文化影響綜合績(jī)效的具體路徑,并提出提升公立醫(yī)院綜合績(jī)效的醫(yī)院文化建設(shè)措施,為醫(yī)改提供理論支撐。研究成果將進(jìn)一步拓寬醫(yī)院文化及綜合績(jī)效的立論觀點(diǎn)及研究視角,為各地公立醫(yī)院實(shí)際推行文化建設(shè)及醫(yī)藥衛(wèi)生體制改革提供參照,進(jìn)一步推動(dòng)新醫(yī)改進(jìn)程。基于上述研究目的,本次公立醫(yī)院組織文化對(duì)綜合績(jī)效的影響研究包括以下幾個(gè)方面:(1)文獻(xiàn)綜述,對(duì)公立醫(yī)院組織文化及綜合績(jī)效相關(guān)研究進(jìn)行系統(tǒng)整理。(2)理論分析,從結(jié)構(gòu)上理清公立醫(yī)院文化如何影響綜合績(jī)效,以基因—文化協(xié)同理論、委托代理理論、利益相關(guān)者理論為基礎(chǔ),并結(jié)合文獻(xiàn)綜述,提出公立醫(yī)院組織文化對(duì)綜合績(jī)效影響的模型與假設(shè)。(3)實(shí)證研究,通過(guò)問(wèn)卷調(diào)查法及SEM(結(jié)構(gòu)方程模型)對(duì)所提假設(shè)進(jìn)行驗(yàn)證。調(diào)查樣本為省內(nèi)南京、無(wú)錫、蘇州、泰州、南通等地具備一定文化規(guī)模的二級(jí)及以上公立醫(yī)院14所;調(diào)查對(duì)象包括公立醫(yī)院的醫(yī)務(wù)人員、管理人員、政府衛(wèi)生部門的管理人員,高校相關(guān)專家等,共獲得有效問(wèn)卷289份;統(tǒng)計(jì)工具為SPSS19.0軟件(檢驗(yàn)數(shù)據(jù)信度、效度)和Lisrel8.70軟件(構(gòu)建結(jié)構(gòu)方程模型)。(4)結(jié)論建議,對(duì)研究結(jié)果進(jìn)行討論,并以提升公立醫(yī)院綜合績(jī)效為目的,提出醫(yī)院文化建設(shè)措施。經(jīng)過(guò)研究總結(jié),表明公立醫(yī)院組織文化中凝聚協(xié)調(diào)文化特質(zhì)、激勵(lì)約束文化特質(zhì)、外部適應(yīng)文化特質(zhì)均對(duì)綜合績(jī)效產(chǎn)生影響,即科學(xué)先進(jìn)的醫(yī)院文化建設(shè)能促進(jìn)公立醫(yī)院綜合績(jī)效的提升。由此提出提升公立醫(yī)院綜合績(jī)效的醫(yī)院文化建設(shè)的建議,即發(fā)揮公立醫(yī)院凝聚協(xié)調(diào)的文化特質(zhì),履行社會(huì)責(zé)任,樹立正確價(jià)值觀、提倡團(tuán)隊(duì)合作,促進(jìn)醫(yī)院內(nèi)部協(xié)調(diào),確保綜合績(jī)效的實(shí)現(xiàn);突出公立醫(yī)院外部適應(yīng)的文化特質(zhì),重視學(xué)習(xí)與創(chuàng)新、以人為本、弘揚(yáng)醫(yī)學(xué)人文精神,共建和諧的文化環(huán)境;重視公立醫(yī)院激勵(lì)約束的文化特質(zhì),樹立競(jìng)爭(zhēng)、持續(xù)發(fā)展意識(shí),加強(qiáng)人才隊(duì)伍建設(shè)、培養(yǎng)醫(yī)務(wù)人員職業(yè)精神,規(guī)范管理制度、適當(dāng)下放權(quán)力,保持發(fā)展活力;并提出提升公立醫(yī)院組織文化建設(shè)戰(zhàn)略地位,發(fā)揮文化的積極作用,推進(jìn)公立醫(yī)院改革。
[Abstract]:China attaches great importance to the great development and prosperity of the cultural field. As an important branch of socialist culture, hospital culture is not only a concept of existence, but also a new trend of modern hospital management. At present, the reform of public hospitals has entered a difficult stage. How to achieve the improvement and sustainable development of their own comprehensive performance on the basis of complying with the overall goal of the new medical reform, and how to give full play to the role of culture in promoting the reform process. It is an urgent problem that needs to be solved. There are few researches on the relationship between culture and performance in the field of medical and health, and most of the studies are carried out unilaterally from the social or economic benefits. The purpose of this study is to analyze the impact of public hospital organizational culture on comprehensive performance, to explore the specific path of hospital culture impact on comprehensive performance, and to put forward the measures of hospital culture construction to improve the comprehensive performance of public hospitals, and to provide theoretical support for medical reform. The research results will further broaden the viewpoint and perspective of hospital culture and comprehensive performance, provide references for the actual implementation of cultural construction and medical and health system reform in local public hospitals, and further promote the process of new medical reform. For the purpose of the above research, this research includes the following aspects: 1) Literature review, and a systematic analysis of the related research on organizational culture and comprehensive performance of public hospitals. Based on the theory of gene-culture synergy, principal-agent theory, stakeholder theory, and literature review, this paper clarifies how public hospital culture affects comprehensive performance from the perspective of structure. An empirical study on the influence of organizational culture in public hospitals on comprehensive performance is presented. The hypothesis is verified by questionnaire survey and SEM (structural equation Model). The survey samples were 14 public hospitals of a certain cultural scale in Nanjing, Wuxi, Suzhou, Taizhou, Nantong and other places in the province. The subjects included medical personnel, management personnel, and administrative personnel of government health departments in public hospitals. A total of 289 valid questionnaires were obtained from relevant experts in colleges and universities. The statistical tools were SPSS19.0 software (test data reliability, validity) and Lisrel8.70 software (construction of structural equation model, etc.). The results of the study were discussed. In order to improve the comprehensive performance of public hospitals, the measures of hospital culture construction are put forward. Through the research and summary, it is shown that the combination and coordination of cultural characteristics in the organizational culture of public hospitals, the stimulation and restraint of cultural characteristics, and the cultural characteristics of external adaptation all have an impact on the comprehensive performance. That is, the construction of scientific and advanced hospital culture can promote the comprehensive performance of public hospitals. Therefore, some suggestions are put forward to improve the comprehensive performance of public hospitals, that is, to bring into play the cultural characteristics of public hospitals' cohesion and coordination, to fulfill their social responsibilities, to establish correct values, to promote teamwork, and to promote internal coordination in hospitals. To ensure the realization of comprehensive performance, highlight the cultural characteristics of external adaptation of public hospitals, attach importance to learning and innovation, people-oriented, carry forward the medical humanistic spirit, build a harmonious cultural environment, attach importance to the cultural characteristics of public hospitals, Set up the competition, keep developing consciousness, strengthen the construction of talents, train the professional spirit of medical personnel, standardize the management system, appropriately devolve the power, maintain the vitality of development, and raise the strategic position of organizational culture construction in public hospitals. Give play to the positive role of culture and promote the reform of public hospitals.
