【摘要】：背景:隨著生活水平的提高,藥品安全越來越受到人們的重視,尤其以妊娠期的藥品安全為最。孕婦特有的生理特點(diǎn)使得她對(duì)藥物的敏感度提升,成為用藥安全最需要注意的群體。其次,隨著藥理實(shí)驗(yàn)的不斷深入,許多常用中藥被檢測(cè)出含有妊娠毒性和遺傳毒性,而一些傳統(tǒng)的妊娠禁忌中藥卻被用于治療妊娠期疾病。妊娠禁忌中藥應(yīng)該依照一個(gè)什么樣的標(biāo)準(zhǔn),才能做到安全用藥,這就需要對(duì)妊娠禁忌中藥進(jìn)行新一輪的整理和分析。目的:整理本草文獻(xiàn)和現(xiàn)代藥理研究中出現(xiàn)的妊娠禁忌中藥,建立數(shù)據(jù)庫(kù),并進(jìn)行四性,五味,歸經(jīng),功效,臨床和藥理實(shí)驗(yàn)分門別類的歸納和數(shù)據(jù)挖掘,以期能得出妊娠禁忌中藥的相關(guān)特點(diǎn),為妊娠禁忌中藥的安全性評(píng)級(jí)提供參考。結(jié)果:共檢索出妊娠禁忌中藥1136味。進(jìn)行數(shù)據(jù)挖掘,發(fā)現(xiàn)妊娠禁忌中藥中藥"性""的分布以溫、寒、平、涼遞減分布,熱性所占比例較小;"味"的分布以苦、辛最多,約占總比例的70%,其次為甘酸咸;歸經(jīng)分布以肝經(jīng)為最多,約占31%,其余肺、脾、胃、腎為分布較多的歸經(jīng),均在10%以上,大腸、心經(jīng)為9%,其余5經(jīng)分布較少。相關(guān)對(duì)應(yīng)分析發(fā)現(xiàn),辛溫、苦寒、苦溫平為出現(xiàn)頻率較高的性味組合。寒肝,溫肝為出現(xiàn)頻率最高的性歸經(jīng)組合�？喔�、辛肝為出現(xiàn)頻率最高的味歸經(jīng)組合毒性對(duì)應(yīng)分析結(jié)果顯示,有毒藥物占妊娠禁忌中藥總和的29%。最常出現(xiàn)的幾種功效為解毒、活血、止痛、祛風(fēng)、清熱、消腫、除濕、散瘀、止血、利濕、殺蟲等,并進(jìn)行了功效與性、味、歸經(jīng)之間的關(guān)聯(lián)分析。臨床實(shí)驗(yàn)文獻(xiàn)總結(jié)中發(fā)現(xiàn)一些傳統(tǒng)妊娠禁忌中藥對(duì)于妊娠高血壓、妊娠糖尿病、妊娠劇吐、宮外孕等疾病具有良好的療效,藥理實(shí)驗(yàn)文獻(xiàn)總結(jié)中以有妊娠毒性、無(wú)妊娠毒性、興奮子宮、遺傳毒性、抗早孕、腫瘤進(jìn)行了分類歸納。結(jié)論:本文對(duì)妊娠禁忌中藥進(jìn)行了整理和歸納,分析了藥物性、味、歸經(jīng)、功效的特點(diǎn)以及相互關(guān)系,分類歸納了臨床和藥理實(shí)驗(yàn)的相關(guān)文獻(xiàn)。為進(jìn)一步研究妊娠禁忌中藥,確立妊娠禁忌中藥的安全性評(píng)級(jí),以及妊娠禁忌中藥的跨領(lǐng)域研究提供了文獻(xiàn)基礎(chǔ),并為臨床安全用藥提供參考。
[Abstract]:Background: with the improvement of living standard, people pay more and more attention to drug safety, especially during pregnancy. The physiological characteristics of pregnant women make her more sensitive to drugs and become the most important group for drug safety. Secondly, with the deepening of pharmacological experiments, many commonly used Chinese medicines have been detected to contain gestational toxicity and genetic toxicity, while some traditional contraindications are used to treat gestational diseases. What kind of standard should be followed in pregnancy taboo Chinese medicine, which requires a new round of collation and analysis of pregnancy taboo Chinese medicine. Objective: to sort out the pregnancy contraindication Chinese medicine appeared in the literature and modern pharmacological research of this herb, to establish the database, and to make the induction and data mining of the four properties, five flavors, meridians, efficacy, clinical and pharmacological experiments. In order to obtain the relevant characteristics of pregnancy contraindication Chinese medicine, and provide a reference for the safety rating of pregnancy taboo Chinese medicine. Results: a total of 1136 kinds of pregnancy contraindications were found. Through data mining, it was found that the distribution of "sex" in pregnancy taboo Chinese medicine was temperature, cold, flat, cool decreasing distribution, the proportion of heat was small, the distribution of "taste" was bitter, the most, about 70% of the total proportion, followed by glycolic acid salt; The distribution of the meridian of the liver was the most, accounting for 31%, the other lungs, spleen, stomach and kidney were more than 10%, the large intestine and heart meridian were 9 parts, and the other 5 meridians were less. Correlation analysis showed that Xin Wen, bitter cold and bitter warm level were high frequency combinations of sexual taste. Cold liver, warm liver is the highest frequency of sexual meridian combination. Bitter liver, xinggan is the highest frequency of taste combination toxicity analysis results show that the toxic drugs accounted for 29% of the total gestational taboo Chinese medicine. The most common effects are detoxification, activating blood circulation, relieving pain, dispelling wind, clearing heat, eliminating swelling, dehumidification, dispersing stasis, hemostasis, dampness, killing insects, etc. In the summary of clinical experimental literature, it was found that some traditional Chinese medicine had good curative effect on pregnancy hypertension, gestational diabetes mellitus, hyperemesis of pregnancy, ectopic pregnancy and other diseases. Excitatory uterus, genetic toxicity, anti-early pregnancy, tumor were classified and summarized. Conclusion: the contraindications of traditional Chinese medicine for pregnancy are summarized and summarized. The characteristics of medicine, taste, meridian, efficacy and the relationship between them are analyzed, and the related literatures of clinical and pharmacological experiments are classified and summarized. It provides a literature basis for further study on pregnancy contraindication Chinese medicine, establishing the safety rating of pregnancy taboo Chinese medicine, and cross-domain research on pregnancy taboo Chinese medicine, and provides a reference for clinical safety drug use.
