【摘要】：本文為國家中醫(yī)藥管理局2015年中醫(yī)臨床診療指南制修訂項目中的"上氣道咳嗽綜合征(制訂)"(課題編號:SATCM-2015-BZ068)的組成部分。本文采取文獻(xiàn)研究、德爾菲法問卷調(diào)查、專家論證會、同行評價、方法學(xué)質(zhì)量評估、臨床一致性測試等方法,開展上氣道咳嗽綜合征診療指南(制訂)的研究。通過網(wǎng)絡(luò)檢索與手工檢索相結(jié)合,通過中國內(nèi)科古代文獻(xiàn)數(shù)據(jù)庫、中國知網(wǎng)平臺、萬方全文數(shù)據(jù)庫、MEDLINE等平臺和相關(guān)中西醫(yī)內(nèi)科教及診療指南等資料,搜集古今中外關(guān)于上氣道咳嗽綜合征的文獻(xiàn),對文獻(xiàn)進(jìn)行整理、評價、總結(jié),為指南制訂提供循證醫(yī)學(xué)的證據(jù)。根據(jù)文獻(xiàn)研究總結(jié)研討形成了德爾菲法調(diào)查問卷,采用電子信函方式向有豐富臨床經(jīng)驗的來自全國各地的30名專家進(jìn)行了三次問卷調(diào)查。對每次回收的問卷的數(shù)據(jù)進(jìn)行分析,結(jié)合專家提出的書面意見,修改形成下一輪的問卷,直到第三輪問卷調(diào)查結(jié)束后形成指南草稿。形成指南草稿后,邀請了 10名專家進(jìn)行專家論證會,對指南草稿進(jìn)行了討論和論證,提出修改意見,形成指南初稿。將指南初稿通過電子信函的方式向全國各地30所單位的專家進(jìn)行同行評價征求意見,16位專家回復(fù),并提出建議,采納部分意見,以此為依據(jù),對指南進(jìn)行修改,形成評價稿。同時,邀請4位專家進(jìn)行指南方法學(xué)質(zhì)量評價,根據(jù)提出意見進(jìn)一步完善指南評價稿。在10家三級甲等醫(yī)院作為評價單位進(jìn)行臨床一致性評價,共收集病例212例,完成病例調(diào)查表和一致性測試報告,并進(jìn)行匯總分析,再次修改、補充相關(guān)材料,形成指南草案。
[Abstract]:This paper is an integral part of "Upper Airway cough Syndrome" (subject number: SATCM-2015-BZ068) in the 2015 revised item of TCM Clinical guidance system of the State Administration of traditional Chinese Medicine (SATCM-2015-BZ068). In this paper, the methods of literature study, Delphi questionnaire, expert demonstration, peer evaluation, methodological quality evaluation, clinical consistency test, etc., were used to develop the guidelines for the diagnosis and treatment of upper airway cough syndrome. Through the combination of network retrieval and manual retrieval, through the Chinese internal medicine ancient literature database, the Chinese knowledge and network platform, the Wanfang full text database and other platforms such as MEDLINE, and related materials such as traditional Chinese and western medicine, science and technology education and diagnosis and treatment guide, etc. The literature on upper airway cough syndrome was collected, reviewed, evaluated and summarized to provide evidence of evidence-based medicine for the development of guidelines. According to the literature review, a Delphi questionnaire was formed, and 30 experts from all over the country with rich clinical experience were investigated by email. After analyzing the data of each questionnaire and combining the written comments of the experts, the questionnaire is revised to form the next round of questionnaires until the end of the third round of questionnaires to form a draft of the guide. After the draft of the guide was formed, 10 experts were invited to conduct an expert demonstration meeting, to discuss and demonstrate the draft guide, to propose amendments, and to form the first draft of the guide. The first draft of the guide was e-mailed to experts from 30 units across the country for peer evaluation and 16 experts responded, and made recommendations and accepted some comments, based on which the guide was revised to form an evaluation draft. At the same time, four experts were invited to conduct a methodological quality evaluation of the guidelines to further refine the draft guidelines based on their comments. Clinical consistency evaluation was conducted in 10 Grade 3A hospitals. 212 cases were collected, case questionnaire and conformance test report were completed, collected and analyzed, revised again, supplemented relevant materials and formed draft guidelines.
