麥粒灸治療惡性腫瘤高凝狀態(tài)的療效分析與機(jī)制探討

【摘要】:目的:觀察麥粒灸療法對(duì)惡性腫瘤患者凝血相關(guān)指標(biāo)及血清白介素-1β(IL-1β)、腫瘤壞死因子-α(TNF-α)的影響,以及其對(duì)Lewis肺癌裸鼠一般情況、瘤體大小、生存率及IL-1β、核轉(zhuǎn)錄因子-kB(NF-kB)表達(dá)等的影響,探討麥粒灸對(duì)惡性腫瘤高凝狀態(tài)的治療作用。方法:(1)將55例確診為惡性實(shí)體腫瘤患者隨機(jī)分兩組:治療組(27例)和對(duì)照組(28例)。兩組均接受扶正抗腫瘤藥物治療,治療組在藥物治療基礎(chǔ)上,加用麥粒灸足三里治療,1次/天,10天為1個(gè)周期,兩個(gè)周期后觀察患者凝血相關(guān)指標(biāo)及血清IL-1β、T NF-α表達(dá)水平。(2)將30只雄性BALB/c裸鼠隨機(jī)分為三組:模型組、治療組和對(duì)照組,每組10只。其中對(duì)照組為空白對(duì)照,模型組裸鼠予皮下注射Lewis肺腺癌細(xì)胞懸液建立模型,治療組在此基礎(chǔ)上加用麥粒灸后三里(雙側(cè))治療,每個(gè)穴位灸3壯,1次/天,共治療10天。三組裸鼠每天同時(shí)固定在鼠板上,模型組和對(duì)照組不予麥粒灸干預(yù)。觀察各組裸鼠的一般情況、瘤體大小和生存率;采用酶聯(lián)免疫吸附法(ELISA)方法測(cè)定血清IL-1β、采用免疫印跡法(Western Blot)方法測(cè)定瘤體中NF-kB表達(dá)水平。結(jié)果:1、臨床試驗(yàn)結(jié)果顯示:(1)治療前患者血清IL-1β與凝血酶原時(shí)間(PT)、活化部分凝血活酶時(shí)間(APTT)成負(fù)相關(guān),與纖維蛋白原量(FIB)、D-二聚體(D-D)成正相關(guān),差異具有統(tǒng)計(jì)學(xué)意義(P0.05);血清TNF-α與APTT、PT成負(fù)相關(guān),與FIB、血小板計(jì)數(shù)(PLT)及D-D成正相關(guān),差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。(2)兩組APTT、PT較前升高,FIB、PLT較前降低,差異具有統(tǒng)計(jì)學(xué)意義(P0.05),且在升高PT,降低PLT、FIB等方面,治療組明顯優(yōu)于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。同時(shí)治療組D-D也較前降低,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。(3)治療組血清IL-1β、TNF-α均較前明顯降低,對(duì)照組血清TNF-α較前明顯升高,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);同時(shí)在降低血清IL-1β、TNF-α方面,治療組明顯優(yōu)于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。2、動(dòng)物實(shí)驗(yàn)結(jié)果顯示:在體重和生存率方面,治療組與模型組相比,差異無統(tǒng)計(jì)學(xué)意義(P0.05);在瘤體大小方面,治療組在成瘤第8天以后明顯小于模型組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);在血清IL-1β和組織NF-kB表達(dá)水平方面,治療組和模型組與對(duì)照組相比明顯升高,并且治療組明顯低于模型組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:麥粒灸可通過下調(diào)腫瘤患者IL-1β、TNF-α的表達(dá),調(diào)節(jié)機(jī)體炎癥微環(huán)境,改善異常凝血功能,進(jìn)而改善腫瘤高凝狀態(tài)。麥粒灸能改善Lewis肺癌裸鼠瘤體一般情況、抑制腫瘤生長(zhǎng)、降低死亡率,其機(jī)制可能與麥粒灸通過降低血清IL-1β水平,進(jìn)一步下調(diào)NF-κB在瘤體中的表達(dá),改善血液高凝狀態(tài),延緩腫瘤的生長(zhǎng)、轉(zhuǎn)移。這為麥粒灸在腫瘤高凝狀態(tài)治療中的臨床運(yùn)用完善了實(shí)驗(yàn)依據(jù)。
[Abstract]:Objective: to observe the effect of grain moxibustion therapy on blood coagulation related indexes and serum interleukin-1 尾 -1 尾, tumor necrosis factor- 偽 TNF- 偽 in patients with malignant tumor, as well as the general situation and tumor size of nude mice with Lewis lung cancer. The survival rate and the expression of IL-1 尾 and nuclear transcription factor NF-kB were studied in order to explore the therapeutic effect of grain moxibustion on hypercoagulable state of malignant tumor. Methods 55 patients with malignant solid tumor were randomly divided into two groups: treatment group (n = 27) and control group (n = 28). Both groups were treated with Fuzheng antitumor drugs. On the basis of drug therapy, the treatment group was treated with moxibustion of wheat grain Zusanli once a day for 10 days as one cycle. After two cycles, 30 male BALB/c nude mice were randomly divided into three groups: model group, treatment group and control group, 10 rats in each group. The model group was established by subcutaneously injection of Lewis lung adenocarcinoma cell suspension, and the treatment group was treated with wheat grain moxibustion for 3 li (bilateral). Each acupoint moxibustion was treated once a day for 10 days. The three groups of nude mice were fixed on the mouse board at the same time every day, while the model group and the control group were not treated with moxibustion. The general condition, tumor size and survival rate of nude mice in each group were observed. Serum IL-1 尾 was determined by Elisa and the expression of NF-kB was detected by Western blotting. Results the clinical trial results showed that the serum IL-1 尾 was negatively correlated with prothrombin time and activated partial thromboplastin time (APTT) before treatment, and positively correlated with fibrinogen content and D-dimer. Serum TNF- 偽 had a negative correlation with APTTT PT, and a positive correlation with FIBT, platelet count (PLT) and D-D. The difference was statistically significant (P < 0.05). The difference was statistically significant (P 0.05), and the treatment group was better than the control group in increasing PTT and decreasing PLT FIB, and the difference was statistically significant (P 0.05). At the same time, the D-D of the treatment group was also lower than that of the former, the difference was statistically significant (P 0.05). The serum IL-1 尾 -TNF- 偽 of the treatment group was significantly lower than that of the previous treatment group, and the serum TNF- 偽 level of the control group was significantly higher than that of the former group, and the difference was statistically significant (P 0.05), at the same time, the serum IL-1 尾 -TNF- 偽 was decreased. The results of animal experiments showed that there was no significant difference in body weight and survival rate between the treatment group and the model group, and there was no significant difference in the size of the tumor in the treatment group compared with the model group. The level of serum IL-1 尾 and tissue NF-kB in the treatment group was significantly higher than that in the control group, and it was significantly lower in the treatment group than in the model group, and the difference was statistically significant between the treatment group and the model group after the 8th day after tumorigenesis, and the expression of serum IL-1 尾 and tissue NF-kB in the treatment group was significantly lower than that in the model group. The difference was statistically significant (P 0.05). Conclusion: grain moxibustion can regulate the inflammatory microenvironment, improve abnormal coagulation function and improve tumor hypercoagulability by down-regulating the expression of IL-1 尾 -TNF- 偽 in tumor patients. Grain moxibustion can improve the general condition of Lewis lung cancer in nude mice, inhibit tumor growth and reduce mortality. The mechanism may be that grain moxibustion can further down-regulate the expression of NF- 魏 B in the tumor and improve the hypercoagulability by reducing the serum IL-1 尾 level. To delay the growth and metastasis of the tumor. This provides an experimental basis for the clinical application of granulated moxibustion in the treatment of tumor hypercoagulability.
