【摘要】：背景:結(jié)直腸癌是目前最常見消化道腫瘤之一,發(fā)病率在國內(nèi)一直呈上升趨勢,盡管近年來結(jié)直腸癌綜合防治水平有所提高,但是各地不平衡,國內(nèi)總平均5年生存率在27%左右,仍然有30-40%病人死于復(fù)發(fā)和轉(zhuǎn)移,結(jié)果仍然不是十分理想。目的:研究脂類代謝與結(jié)直腸腫瘤的相關(guān)性,對提高結(jié)直腸腫瘤的早期診斷和治療水平可能具有重要價值,能夠在結(jié)直腸腫瘤的預(yù)防及形成過程中進行干預(yù),以及對預(yù)后提供更多的治療方法。方法:1、回顧性分析結(jié)直腸癌組、結(jié)直腸腺瘤組與對照組患者的血脂指標及組間差異。2、分別檢測結(jié)直腸腸癌組、結(jié)直腸腺瘤組、對照組患者血清標本中脂肪細胞型脂肪酸結(jié)合蛋白(FABP4)濃度值及血脂相關(guān)指標,分析三組間差別。結(jié)果:1、回顧性分析結(jié)直腸癌組、結(jié)直腸腺瘤組與對照組相比,腫瘤兩組載脂蛋白A1(ApoAI)均低于對照組,差別有統(tǒng)計學(xué)意義(P0.05);結(jié)直腸癌組脂蛋白a(Lp(a))水平高于對照組,差別有統(tǒng)計學(xué)意義(P0.05);結(jié)直腸腺瘤組高密度脂蛋白膽固醇(HDL-C)水平低于對照組,差別有統(tǒng)計學(xué)意義(P0.05)。2、三組間血清標本中FABP4水平比較,差別無統(tǒng)計學(xué)意義(P0.05);腫瘤兩組ApoAI及低密度脂蛋白膽固醇(LDL-C)平均低于對照組,差別有統(tǒng)計學(xué)意義(P0.05);結(jié)直腸癌組Lp(a)水平高于對照組,差別有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:結(jié)直腸腫瘤患者ApoAI較對照組有明顯差別,結(jié)直腸癌患者Lp(a)較對照組有明顯差別,說明腫瘤在發(fā)生及發(fā)展過程中與ApoAI、Lp(a)代謝有一定的關(guān)聯(lián),為今后結(jié)直腸癌的預(yù)防、診斷及治療提供了幫助及新方向;而腫瘤患者FABP4與對照組相比無統(tǒng)計學(xué)差異,仍需進一步的探索。
[Abstract]:Background: colorectal cancer is one of the most common digestive tract tumors at present. The incidence of colorectal cancer has been on the rise in China. Although the level of comprehensive prevention and treatment of colorectal cancer has improved in recent years, it is unbalanced in different regions. The average 5-year survival rate in China is about 27%, and 30-40% of the patients still die from recurrence and metastasis, the results are still not ideal. Objective: to study the correlation between lipid metabolism and colorectal tumors. It may be of great value in improving the level of early diagnosis and treatment of colorectal neoplasms, and can intervene in the prevention and formation of colorectal tumors. And to provide more treatment methods for prognosis. Methods: 1. Retrospective analysis of serum lipids in colorectal cancer group, colorectal adenoma group and control group, and the difference between two groups, respectively, to detect colorectal cancer group, colorectal adenoma group, and colorectal adenoma group. The concentration of FABP4 in serum of the control group and the related indexes of blood lipids were analyzed, and the differences among the three groups were analyzed. Results: 1. The retrospective analysis of colorectal cancer group and colorectal adenoma group was compared with that of the control group. The level of apolipoprotein A1A1ApoAII in tumor group was significantly lower than that in control group (P 0.05), and the level of lipoprotein A (LpP) in colorectal cancer group was higher than that in control group (P 0.05), and the HDL-C level in colorectal adenoma group was lower than that in control group (P < 0 05), and the level of HDL-C in colorectal adenoma group was lower than that in control group (P < 0 05). There was no significant difference in serum FABP4 level among the three groups, and the ApoAI and LDL-C in tumor group were lower than those in control group. The difference was statistically significant (P 0.05), and the level of LpChina in colorectal cancer group was higher than that in control group (P 0.05). Conclusion: ApoAI in colorectal cancer patients is significantly higher than that in control group, and there is a significant difference between the patients with colorectal cancer and the control group. The results indicate that the tumor is related to the metabolism of ApoAI-LpChina in the course of its occurrence and development, which provides a new direction for the prevention, diagnosis and treatment of colorectal cancer, but there is no significant difference in FABP4 between the tumor patients and the control group. Further exploration is still needed.
