【摘要】：目的:采用瀉肝安神湯加減方治療肝旺脾虛型老年性皮膚瘙癢癥,通過臨床療效的觀察,客觀評(píng)價(jià)瀉肝安神湯加減方治療老年性皮膚瘙癢癥(肝旺脾虛型)的臨床療效,并探討其療效機(jī)理,旨在為老年性皮膚瘙癢癥的中醫(yī)辨證論治提供理論依據(jù)。方法:將135例符合納入標(biāo)準(zhǔn)的老年性皮膚瘙癢癥患者,分為兩組:治療組68例,對(duì)照組67例。兩組均予以基礎(chǔ)治療:保濕劑外擦,治療組口服瀉肝安神湯加減方,對(duì)照組口服依巴斯汀片,兩組均治療4周。治療結(jié)束時(shí)和治療結(jié)束后1月各隨訪1次,研究結(jié)束。結(jié)果:治療組66例完成試驗(yàn),對(duì)照組66例完成試驗(yàn),兩組患者在性別、年齡、病程等方面經(jīng)統(tǒng)計(jì)學(xué)處理無明顯差異(P0.05),具有可比性。臨床療效比較:治療組:治愈1例,顯效11例,有效46例,無效8例,復(fù)發(fā)8例,總有效率87.88%,復(fù)發(fā)率12.12%,對(duì)照組:治愈0例,顯效1例,有效34例,無效31例,復(fù)發(fā)19例,總有效率53.03%,復(fù)發(fā)率28.79%,兩組總有效率相比,P0.05,表明兩組療效指數(shù)差異具有統(tǒng)計(jì)學(xué)意義。不良反應(yīng)比較:兩組患者在治療過程中未出現(xiàn)頭昏、嘔吐、腹瀉、嗜睡等不良反應(yīng)。結(jié)論:瀉肝安神湯加減方治療肝旺脾虛型老年性皮膚瘙癢癥療效顯著,肝旺脾虛是老年性皮膚瘙癢癥的病機(jī)之一,為老年性皮膚瘙癢癥的中醫(yī)辨證論治提供了一定的理論依據(jù)。
[Abstract]:Objective: to use Xiegan Anshen decoction to treat senile skin pruritus with deficiency of liver and spleen, and to evaluate objectively the clinical effect of Xigan Anshen decoction on senile skin pruritus. The aim of this study was to provide a theoretical basis for the treatment of senile pruritus by differentiation of symptoms and signs. Methods: 135 patients with senile pruritus were divided into two groups: treatment group (n = 68), treatment group (n = 68), and treatment group (n = 68). 67 cases in the control group were treated with basic treatment: external rubbing of moisturizer, oral decoction of Xiegan Anshen decoction in the treatment group and Ebastine tablet in the control group. The two groups were treated for 4 weeks. Each group was followed up once at the end of the treatment and one month after the end of the treatment. Results: 66 patients in the treatment group and 66 patients in the control group completed the test. Comparison of clinical efficacy: 1 case was cured, 11 cases were markedly effective, 46 cases were effective, 8 cases were ineffective, 8 cases were relapse, the total effective rate was 87.88%, and the recurrence rate was 12.12%. In the control group, 0 cases were cured. There were significant effects in 1 case, effective in 34 cases, ineffective in 31 cases, recurrence in 19 cases. The total effective rate was 53.03 and the recurrence rate was 28.79. The total effective rate in the two groups was higher than that in the control group (P 0.05), which indicated that the difference of curative effect index between the two groups was statistically significant. Conclusion: Xiegan Anshen decoction is effective in treating senile skin pruritus with liver and spleen deficiency, which is one of the pathogenesis of senile pruritus. It provides a theoretical basis for the treatment of senile pruritus based on syndrome differentiation.
