【摘要】：目的:通過自擬益肺止咳湯對陰虛肺燥型妊娠咳嗽的患者進(jìn)行臨床治療,評價(jià)其臨床療效,將治療本病相關(guān)經(jīng)驗(yàn)與同行們共享。方法:收集2016年02月至2017年01月于黑龍江中醫(yī)藥大學(xué)附屬第一醫(yī)院婦科門診就診符合本課題研究診斷納入及排除標(biāo)準(zhǔn)的妊娠期咳嗽患者30例進(jìn)行臨床觀察研究�；颊呷虢M后根據(jù)個(gè)體情況差異辨證,組藥開方,分別觀察患者療程均為2周。分別觀察患者0天(入組時(shí))及14天(治療后)中醫(yī)癥狀積分(包括主癥、兼次癥、總分);統(tǒng)計(jì)患者治療2周后臨床咳嗽療效、及其他中醫(yī)癥狀療效改善。結(jié)果:1.共入組30例陰虛肺燥型妊娠咳嗽患者,總有效率達(dá)90.00%,其中痊愈率為10.00%,顯效率為50.00%,有效率30.00%。2.對治療前后中醫(yī)癥候總積分?jǐn)?shù)據(jù)進(jìn)行分析,經(jīng)統(tǒng)計(jì)學(xué)兩組相關(guān)樣本非參數(shù)檢驗(yàn)(Wilcoxon秩和檢驗(yàn))分析,P=0.000＜0.01,可以認(rèn)為治療前后癥狀積分有顯著差異,且治療前癥狀積分明顯高于治療后癥狀積分,具有統(tǒng)計(jì)學(xué)意義。3.對主證、兼次證癥狀治療前后積分進(jìn)行統(tǒng)計(jì),運(yùn)用統(tǒng)計(jì)學(xué)兩組相關(guān)樣本非參數(shù)檢驗(yàn)分析(Wilcoxon秩和檢驗(yàn)),得出各項(xiàng)癥狀的P＜0.01,有統(tǒng)計(jì)學(xué)意義,說明治療前后各項(xiàng)證候積分有顯著差異,且治療前各項(xiàng)癥狀積分明顯高于治療后各項(xiàng)癥狀積分。結(jié)論:1.自擬益肺止咳湯在減輕入組患者主癥積分的同時(shí),對次癥亦有較好的控制作用。2.本病的治療以識(shí)病位、辨病機(jī)、結(jié)合止咳與安胎并舉這一大法為主,在選方用藥方面,應(yīng)注重一藥多用,隨癥加減。3.自擬益肺止咳湯治療陰虛肺燥型妊娠咳嗽的臨床療效顯著,值得在臨床上推廣應(yīng)用。
[Abstract]:Objective: to evaluate the clinical effect of Yifei Zhike decoction (YFZD) on pregnancy and cough with yin deficiency and lung dryness. Methods: from February 2016 to January 2017, the gynecological clinic of the first affiliated hospital of Heilongjiang university of traditional Chinese medicine was collected to meet the criteria of inclusion and exclusion of the research and diagnosis of pregnancy. A clinical observation study was carried out in 30 patients with cough. The course of treatment was 2 weeks. The scores of TCM symptom (including main symptom, secondary symptom, total score) were observed on 0 day (when entering the group) and 14 days (after treatment), and the curative effect of clinical cough after 2 weeks of treatment was counted. Results the total effective rate of 30 cases of pregnancy and cough with yin deficiency and lung dryness was 90.00.The cure rate was 10.00, the effective rate was 50.00g and the effective rate was 30.00.2.The total integral data of TCM symptoms before and after treatment were analyzed. According to the nonparametric test (Wilcoxon rank sum test) of the two groups of related samples, we can conclude that there is significant difference in symptom score before and after treatment, and the symptom integral before and after treatment is obviously higher than that after treatment, which has statistical significance. The scores of symptoms before and after treatment were statistically analyzed, and the Wilcoxon rank sum test was used to analyze the two groups of related samples. The results showed that the symptoms were significantly different before and after treatment (P < 0.01), indicating that there were significant differences in the scores of symptoms before and after treatment. The scores of symptoms before and after treatment were significantly higher than those after treatment. Conclusion: 1. The self-made Yifei Zhike decoction can reduce the main symptom score of the patients in the group, at the same time, it also has a better control effect on the secondary symptoms .2.The treatment of this disease can recognize the position of the disease and distinguish the pathogenesis of the disease. The combination of cough relief and the treatment of fetus is the main method, in the choice of prescription, we should pay attention to the use of one drug, with the syndrome plus or minus .3.The self-formulated Yifei Zhike decoction for the treatment of Yin deficiency and lung dryness type pregnancy cough clinical effect is remarkable, it is worth to be popularized in clinical application.
