【摘要】：目的:制訂確切療效、方便應(yīng)用、科學(xué)規(guī)范的穴位敷貼干預(yù)小兒哮喘中醫(yī)治未病實(shí)踐指南,并提供中醫(yī)指南制修訂的技術(shù)方法。方法:采用文獻(xiàn)研究、三輪專家問(wèn)卷調(diào)查(Delphi法)、專家論證會(huì)、同行征求意見(jiàn)、臨床評(píng)價(jià)(方法學(xué)質(zhì)量評(píng)價(jià)、臨床一致性評(píng)價(jià))、專家指導(dǎo)組審核相結(jié)合的方法制訂"中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘"。文獻(xiàn)研究:遵循循證醫(yī)學(xué)的原則,以網(wǎng)絡(luò)檢索為主結(jié)合手工檢索查詢符合質(zhì)量要求的文獻(xiàn)作為指南的證據(jù),經(jīng)專家共識(shí)法形成指南的推薦建議,匯總、歸納后形成"中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘"的文獻(xiàn)研究總結(jié)。專家問(wèn)卷調(diào)查(Delphi法):基于文獻(xiàn)研究總結(jié),設(shè)計(jì)第一輪專家調(diào)查問(wèn)卷,以郵件形式郵寄給全國(guó)遴選出的40位專家,根據(jù)問(wèn)卷回饋的專家評(píng)分進(jìn)行總結(jié)分析形成下一輪問(wèn)卷內(nèi)容,再次郵寄給回復(fù)積極的專家進(jìn)行第二、三輪問(wèn)卷調(diào)查,經(jīng)統(tǒng)計(jì)分析后形成"中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘"的初稿。專家論證會(huì):對(duì)于指南初稿形成過(guò)程中存在需要專家面對(duì)面討論分析的問(wèn)題,項(xiàng)目工作組邀請(qǐng)12名中醫(yī)兒科、中西醫(yī)結(jié)合兒科、文獻(xiàn)研究等的專家召開專家論證會(huì),專家討論后對(duì)相關(guān)問(wèn)題的解決建議基本達(dá)成一致,形成"中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘"的征求意見(jiàn)稿。同行征求意見(jiàn):為了使指南內(nèi)容更完善,項(xiàng)目工作組向標(biāo)準(zhǔn)化相關(guān)單位的專家發(fā)送指南征求意見(jiàn)稿及征求意見(jiàn)處理表以征求指南修改意見(jiàn),經(jīng)項(xiàng)目工作組討論分析修改后,形成指南的評(píng)價(jià)稿。臨床評(píng)價(jià)(方法學(xué)質(zhì)量評(píng)價(jià)、臨床一致性評(píng)價(jià)):方法學(xué)質(zhì)量評(píng)價(jià)是邀請(qǐng)包括中醫(yī)兒科、文獻(xiàn)學(xué)等專業(yè)的4位專家,應(yīng)用AGREEⅡ工具對(duì)《中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘》評(píng)價(jià)稿進(jìn)行方法學(xué)的質(zhì)量評(píng)價(jià);臨床一致性評(píng)價(jià)是在專家指導(dǎo)組指導(dǎo)下,選取不同地域的9個(gè)醫(yī)療機(jī)構(gòu)對(duì)《中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘》評(píng)價(jià)稿與臨床實(shí)踐進(jìn)行一致性評(píng)價(jià),形成《中醫(yī)治未病實(shí)踐指南穴位敷貼干預(yù)小兒哮喘》的審核稿。專家指導(dǎo)組審核:項(xiàng)目工作組將指南審核稿提交中醫(yī)治未病標(biāo)準(zhǔn)制修訂"兒童人群"專家指導(dǎo)組進(jìn)行審核,并提出修改意見(jiàn),進(jìn)一步完善"穴位敷貼干預(yù)小兒哮喘中醫(yī)治未病實(shí)踐指南",形成指南的送審稿。結(jié)果:通過(guò)文獻(xiàn)研究、專家問(wèn)卷調(diào)查(Delphi法)、專家論證會(huì)、同行征求意見(jiàn)、臨床評(píng)價(jià)(方法學(xué)質(zhì)量評(píng)價(jià)、臨床一致性評(píng)價(jià))、專家指導(dǎo)組審核相結(jié)合的方法,制訂出方便應(yīng)用、科學(xué)規(guī)范的中醫(yī)治未病實(shí)踐指南,臨床一致性評(píng)價(jià)結(jié)果為本指南與臨床實(shí)際操作符合度較高,證明本指南制訂的技術(shù)方法適合中醫(yī)指南的制修訂工作。
[Abstract]:Objective: to establish a scientific and standard guidance for the treatment of children asthma by acupoint application, and to provide the technical methods for the revision of TCM guidelines. Methods: literature study was used. Three rounds of expert questionnaire survey, Delphi method, expert demonstration meeting, peer consultation, clinical evaluation (methodological quality evaluation, Clinical consistency evaluation, expert guidance group review combined with the method of developing "the practice guide of traditional Chinese medicine for preventing disease intervention in children asthma". Literature study: follow the principle of evidence-based medicine, With the combination of online retrieval and manual retrieval of documents that meet the quality requirements, as evidence of the guide, the expert consensus method forms the recommended recommendations of the guidelines and summarizes them. After summing up, it formed the literature research summary of "the practice Guide of TCM treatment and Predisease intervention in Children Asthma". The first round of expert questionnaire was designed based on the summary of literature research. Mail it to 40 experts selected throughout the country, summarize and analyze the next round of questionnaires according to the feedback expert scores, and mail the second and third rounds of questionnaires to the responding active experts again. After statistical analysis, a first draft of the "practical guidelines for the practice of traditional Chinese Medicine in treating and preventing Diseases in Children with Asthma" was formed. The expert demonstration meeting: there are problems that need experts to discuss and analyze face to face in the course of the formation of the first draft of the guide. The project working group invited 12 experts from pediatrics of traditional Chinese medicine, integrated traditional Chinese medicine and western medicine, literature research and other experts to convene an expert demonstration meeting. After discussion, the experts basically reached agreement on how to solve the related problems.
