【摘要】：目的通過解剖膝關(guān)節(jié)來確定一個手術(shù)切口的安全區(qū)域和角度,在獲取股薄肌和半腱肌肌腱時盡可能避免損傷隱神經(jīng)及隱神經(jīng)髕下支。方法選擇膝關(guān)節(jié)新鮮標本10例,分離出隱神經(jīng)及其髕下支分支。以髕韌帶在脛骨結(jié)節(jié)止點的內(nèi)緣處定為標志點A。經(jīng)A點做一條水平線,與隱神經(jīng)主干的交點定為標志點C,與隱神經(jīng)髕下支的交點定為標志點B。分別測量標志點A至標志點C的距離AC和標志點A至標志點B的距離AB。通過分析和統(tǒng)計學計算得出手術(shù)安全區(qū)范圍和切口角度。結(jié)果手術(shù)切口安全范圍:A至隱神經(jīng)(C點)的距離AC為(5.5±0.3)cm,A至隱神經(jīng)髕下支(B點)的距離AB為(3.5±0.2)cm。手術(shù)切口的安全角度:隱神經(jīng)髕下支走向為(50.7±4.8)°,隱神經(jīng)脛骨結(jié)節(jié)下分支走向(43.3±5.4)°。結(jié)論手術(shù)切口的安全范圍為距髕韌帶在脛骨結(jié)節(jié)止點的內(nèi)緣(A點)的內(nèi)側(cè)(3.5±0.2)cm以內(nèi),角度為(50.7±4.8)°。
[Abstract]:Objective to determine the safe area and angle of a surgical incision by dissecting the knee joint, and to avoid damage to the saphenous nerve and the subpatellar branch of the saphenous nerve when obtaining the tendinous and semitendinosus tendons. Methods Ten fresh specimens of knee joint were selected. The saphenous nerve and its subpatellar branch were separated. The patellar ligament was used as the mark point A at the inner edge of the tibial tubercle. A horizontal line was made through point A. The intersection with the main trunk of the saphenous nerve was taken as the marker point C, and the intersection point with the subpatellar branch of the saphenous nerve as the marker point B. the distance between marker A and marker point C was measured by measuring the distance between marker A and marker point C and the distance between marker point A and marker point B. through analysis and statistics, the distance between marker A and marker point B was measured. The range of safe area and the angle of incision were calculated. Results the distance between the safe range of surgical incision from: a to point C of saphenous nerve (AC = 5.5 鹵0.3 cm) to point B of inferior patellar branch of saphenous nerve (AB) is 3.5 鹵0.2 cm. The safe angle of surgical incision is: saphenous nerve. The direction of the subpatellar branch is 50.7 鹵4.8 擄, and the direction of the subtubercular branch of the saphenous nerve is 43.3 鹵5.4 擄. Conclusion the safe range of the surgical incision is within 3.5 鹵0.2 cm of the medial side of the patellar ligament. The angle was 50.7 鹵4.8 擄.
