【摘要】:研究目的本研究擬通過文獻(xiàn)研究梳理目前醫(yī)用耗材采購管理相關(guān)政策,通過實(shí)地調(diào)研和問卷調(diào)查掌握我國醫(yī)療機(jī)構(gòu)醫(yī)用耗材采購管理現(xiàn)狀,分析目前醫(yī)療機(jī)構(gòu)醫(yī)用耗材采購管理存在的問題,提出規(guī)范醫(yī)療機(jī)構(gòu)醫(yī)用耗材采購管理的建議。研究方法(1)文獻(xiàn)研究法:通過文獻(xiàn)研究,初步了解與此相關(guān)的基本知識(shí),了解國內(nèi)外研究現(xiàn)狀,為后期研究奠定基礎(chǔ)。通過查閱文獻(xiàn),了解相關(guān)研究的主要成果、現(xiàn)狀及不足之處,明確本課題重點(diǎn)研究的問題:(2)調(diào)查研究法:通過實(shí)地調(diào)研、專家訪談等方式了解各級(jí)醫(yī)療機(jī)構(gòu)醫(yī)用耗材采購內(nèi)部管理的現(xiàn)狀,分析其中存在的問題,探尋問題產(chǎn)生的原因;(3)問卷調(diào)查法:通過問卷填寫的形式,分析目前醫(yī)療機(jī)構(gòu)在醫(yī)用耗材采購管理方面普遍存在的共性問題及原因,并試圖找到初步解決方案。研究結(jié)果醫(yī)用耗材的特點(diǎn)有品種規(guī)格多且復(fù)雜、醫(yī)用耗材產(chǎn)品更新?lián)Q代快、不同品種價(jià)值差異很大、安全性要求高、臨床使用突然性和多樣性、進(jìn)口產(chǎn)品占據(jù)國內(nèi)高值醫(yī)用耗材市場的壟斷地位等;采購方式主要包括招標(biāo)采購、競爭性商務(wù)談判、市場摸底談判;目前我國醫(yī)療機(jī)構(gòu)醫(yī)用耗材管理主要存在以下幾方面的問題:品種分類方面,分類方法過于多元化、分類標(biāo)準(zhǔn)不統(tǒng)一、高值耗材與低值耗材界定標(biāo)準(zhǔn)不統(tǒng)一;目錄管理方面,沒有統(tǒng)一的規(guī)范目錄、沒用統(tǒng)一編碼、目錄制定方法不統(tǒng)一;準(zhǔn)入管理方面,準(zhǔn)入方式不統(tǒng) 、準(zhǔn)入審核內(nèi)容不一致、準(zhǔn)入權(quán)限管理較混亂;采購方式方面,采購方式過于多元化、采購計(jì)劃較為欠缺、采購頻次不合理;配存管理方面,配送企業(yè)數(shù)量較多、庫存量管理不科學(xué)、領(lǐng)用后監(jiān)管不力;信息系統(tǒng)方面,信息化程度低、數(shù)據(jù)分析功能欠缺、交互方式落后。研究結(jié)論對醫(yī)療機(jī)構(gòu)醫(yī)用耗材的品種分類、目錄管理、準(zhǔn)入管理、采購方式、配存管理和信息系統(tǒng)等六個(gè)方面進(jìn)行深入的研究和論證,為構(gòu)建能基本在全國范圍內(nèi)普遍適用的醫(yī)療機(jī)構(gòu)醫(yī)用耗材采購管理新模式提出建議。品種分類方面,應(yīng)當(dāng)逐步建立以醫(yī)用耗材標(biāo)準(zhǔn)化分類編碼為基礎(chǔ)的醫(yī)用耗材信息化管理制度和系統(tǒng);目錄遴選管理方面,應(yīng)根據(jù)各類品種的質(zhì)量技術(shù)和價(jià)格信息綜合評價(jià)后進(jìn)行選擇,在保證產(chǎn)品質(zhì)量的前提下,優(yōu)先選擇性價(jià)比高的品種;準(zhǔn)入管理方面,加強(qiáng)對準(zhǔn)入資質(zhì)的管理、審核;采購管理方面,嚴(yán)格按照采購要求和采購計(jì)劃,繼續(xù)推進(jìn)集中采購工作,對于未納入所屬行政區(qū)域省級(jí)或市級(jí)集中采購范圍的,或在集中采購范圍內(nèi)但未有品種中標(biāo)的,醫(yī)療機(jī)構(gòu)可視情況進(jìn)行招標(biāo)采購、詢價(jià)采購、備案采購等特殊方式進(jìn)行采購;配存管理方面,包括對驗(yàn)收入庫管理和出庫管理的要求;信息系統(tǒng)管理方面,應(yīng)采用信息化技術(shù)手段,實(shí)現(xiàn)對醫(yī)用耗材采購的全流程、全方位監(jiān)管。
[Abstract]:The purpose of this study is to sort out the relevant policies of medical consumables procurement management through literature research, and to grasp the current situation of medical consumables procurement management in medical institutions through field investigation and questionnaire survey. This paper analyzes the problems existing in the procurement management of medical consumables in medical institutions, and puts forward some suggestions for standardizing the procurement management of medical consumables in medical institutions. Understand the domestic and foreign research status, lay the foundation for the later research. Through consulting the literature, understand the main results, current situation and shortcomings of the related research, make clear the research method of the key research question of this topic: through the field investigation, Experts interview and other ways to understand the status of medical institutions at all levels of internal management of medical supplies procurement, analysis of the existing problems, to explore the causes of the problems / 3) questionnaire survey method: through the form of questionnaire filling, This paper analyzes the common problems and reasons of medical institutions in purchasing and management of medical consumables, and tries to find a preliminary solution. The value of different varieties is very different, the requirement of safety is high, the clinical use is sudden and diverse, the imported products occupy the monopoly position of the domestic high value medical consumables market, the purchasing methods mainly include bidding and purchasing, competitive business negotiation, etc. At present, there are some problems in the management of medical consumables in medical institutions in our country: the classification methods are too diversified, the classification standards are not uniform, and the definition standards of high value consumables and low value consumables are not unified. In the area of directory management, there is no unified standard catalogue, no unified coding, and no unified method for cataloguing. In access management, access methods are not uniform, the contents of access review are inconsistent, and the management of access rights is relatively confused. In terms of procurement methods, The procurement mode is too diversified, the purchase plan is relatively deficient, the procurement frequency is unreasonable; in the aspect of inventory management, the number of distribution enterprises is more, the inventory management is not scientific, the supervision after receiving the goods is weak; the information system aspect, the informationization degree is low, The function of data analysis is deficient, and the interactive mode is backward. Conclusion the six aspects of medical consumables classification, catalogue management, access management, purchasing method, storage management and information system are deeply studied and demonstrated. In order to set up a new management model of medical consumables procurement and management, which can be widely used in the whole country, some suggestions are put forward. The information management system and system of medical consumables based on standardized classification and coding of medical consumables should be gradually established. In the selection and management of catalogues, the selection should be based on the comprehensive evaluation of the quality, technology and price information of various varieties. Under the premise of ensuring product quality, priority should be given to the selection of varieties with high performance-to-price ratio; access management should strengthen the management and examination of entry qualifications; and procurement management should continue to promote centralized procurement in strict accordance with procurement requirements and procurement plans. For those not included in the scope of centralized procurement at the provincial or municipal level in the administrative region, or in the scope of centralized procurement but not in the bid of a variety, the medical institution may, according to the circumstances, conduct the procurement by bidding, asking for price, filing for purchase, and other special means; In the aspect of inventory management, including the requirement of acceptance and check in and out of storage, and the information system management, information technology should be adopted to realize the whole process of purchasing medical consumables, and to supervise the whole management of medical consumables in all directions.
