3D打印技術(shù)輔助外固定架治療足踝畸形

【摘要】:目的:探討3D打印技術(shù)輔助外固定架治療足踝畸形的臨床應(yīng)用。方法:回顧性分析廣西醫(yī)科大學(xué)第一附屬醫(yī)院創(chuàng)傷骨科手外科2014年4月—2017年1月足踝畸形行外固定架矯正治療48例患者(53足),年齡5-60歲,平均年齡16歲,隨訪時(shí)間2—34個(gè)月,平均隨訪時(shí)間17個(gè)月。分為3D打印技術(shù)輔助組和傳統(tǒng)手術(shù)組。3D打印輔助組24足,傳統(tǒng)手術(shù)組29足,采用美國足踝外科學(xué)會(huì)AOFAS(AOFAS Ankle Hindfoot Scale)踝-后足評分進(jìn)行療效評價(jià),所得數(shù)據(jù)用SPSS17.0軟件進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:本研究通過回顧性分析3D打印技術(shù)輔助組和傳統(tǒng)手術(shù)組患者的年齡、性別構(gòu)成比無統(tǒng)計(jì)學(xué)差異。術(shù)后根據(jù)美國足踝外科學(xué)會(huì)AOFAS(AOFAS Ankle Hindfoot Scale)踝-后足評分,總48例(53足)患者中優(yōu)26足,良17足,一般9足,差1足,優(yōu)良率81.1%,復(fù)發(fā)1足;其中,3D打印組患者24足中,優(yōu)14足,良7足,一般3足,差0足,優(yōu)良率87.5%,復(fù)發(fā)0足;傳統(tǒng)手術(shù)組29足中,優(yōu)12足,良10足,一般6足,差1足,優(yōu)良率75.8%,復(fù)發(fā)1足;3D組優(yōu)良率較傳統(tǒng)組高11.7個(gè)百分點(diǎn)。根據(jù)美國足踝外科學(xué)會(huì)AOFAS踝-后足評分(AOFAS Ankle Hindfoot Scale)評分系統(tǒng)對兩組患者術(shù)前術(shù)后評分,3D打印技術(shù)輔助組術(shù)前平均得分48.0±15.5,術(shù)后得分88.0±10.1,傳統(tǒng)手術(shù)組術(shù)前平均得分47.0±14.2,術(shù)后得分80.0±13.5,術(shù)前兩組間患者均數(shù)標(biāo)準(zhǔn)差比較P0.05,術(shù)前患足畸形基數(shù)資料無統(tǒng)計(jì)學(xué)意義;術(shù)后兩組間患者采用均數(shù)標(biāo)準(zhǔn)差比較P0.05,術(shù)后兩組間根據(jù)美國足踝外科學(xué)會(huì)AOFAS(AOFAS Ankle Hindfoot Scale)踝-后足評分有統(tǒng)計(jì)學(xué)意義,即3D打印技術(shù)輔助組患者較傳統(tǒng)手術(shù)患者術(shù)后療效滿意。結(jié)論:3D打印技術(shù)輔助外固定架治療足踝畸形,能更好的分析足踝畸形和模擬手術(shù),從而制定更精準(zhǔn)的術(shù)前計(jì)劃,從而獲得更好的術(shù)后效果,值得推廣應(yīng)用。
[Abstract]:Objective: to explore the clinical application of 3D printing assisted external fixator in the treatment of foot and ankle deformity. Retrospective analysis of 48 patients with ankle deformity treated with external fixator from April 2014 to January 2017 in the Department of Orthopedic Trauma, Department of Orthopedics, first affiliated Hospital of Guangxi Medical University. (. 53 feet. Age 5-60 years, mean age 16 years, follow-up time 2-34 months, average follow-up time 17 months, divided into 3D printing assisted group and traditional operation group .3D printing assisted group 24 feet. The curative effect was evaluated by AOFAS(AOFAS Ankle Hindfoot scale score of the American Society of foot and ankle surgery (AOFAS(AOFAS Ankle Hindfoot scale) in the traditional operation group (29 feet). The data were analyzed statistically by SPSS17.0 software. Results: the age of patients in 3D printing assisted group and traditional operation group was analyzed retrospectively. There was no significant difference in sex composition. The ankle-hind foot score was evaluated according to the AOFAS(AOFAS Ankle Hindfoot scale scale of the American Society of foot and ankle surgery. The total 48 cases (53 feet) were excellent in 26 feet, good in 17 feet, fair in 9 feet, poor in 1 foot, excellent and good rate was 81.1, recurrence in 1 foot; Of the 24 patients in the 3D printing group, 14 were excellent, 7 were good, 3 were fair, 0 were poor, the excellent and good rate was 87.5, and the recurrence was 0; Of 29 feet in the traditional operation group, 12 were excellent, 10 were good, 6 were fair, 1 was poor, the excellent and good rate was 75.8and the recurrence was 1; The excellent and good rate of 3D group was 11.7% higher than that of the traditional group. According to the AOFAS ankle-hind foot score of the American Society of foot and ankle surgery, AOFAS Ankle Hindfoot scale was used. The score system was used to evaluate the patients before and after operation. The mean preoperative score was 48.0 鹵15.5in the 3D printing assisted group, 88.0 鹵10.1 in the postoperative group and 47.0 鹵14.2 in the traditional operation group. The postoperative score was 80.0 鹵13.5.The mean standard deviation between the two groups was higher than that before operation (P0.05). The mean standard deviation was compared between the two groups after operation (P0.05). There was statistical significance between the two groups according to the ankle-hind foot score of the American Society of foot and ankle surgery (AOFAS(AOFAS Ankle Hindfoot scale). Conclusion the treatment of foot and ankle deformity with the help of 3 D printing technique can better analyze foot ankle malformation and simulate operation. In order to make more accurate preoperative plan and obtain better postoperative effect, it is worth popularizing.
