【摘要】：目的:比較加速康復(fù)方案指導(dǎo)下的早期前交叉韌帶重建術(shù)和晚期前交叉韌帶重建術(shù)的術(shù)后僵硬風(fēng)險(xiǎn)分析非接觸性ACL損傷患者術(shù)前雙側(cè)膝關(guān)節(jié)的TT-TG值差異及手術(shù)前后TT-TG值的變化。方法:回顧性分析2011年2月至2013年12月于我院行ACL重建術(shù)的29例患者手術(shù)前后及對(duì)照組33例的MRI資料,于軸位測(cè)量TT-TG值。ACL組患者術(shù)后隨訪一年以上,根據(jù)膝關(guān)節(jié)功能評(píng)分及穩(wěn)定性評(píng)估手術(shù)效果。各組間計(jì)量資料采用獨(dú)立樣本t檢驗(yàn),計(jì)數(shù)資料采用χ2檢驗(yàn)分析。結(jié)果:早期組和晚期組術(shù)后關(guān)節(jié)恢復(fù)情況差異無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);ACL重建術(shù)后,膝關(guān)節(jié)旋轉(zhuǎn)及內(nèi)翻的情況得到明顯改善,術(shù)后TT-TG值為11.19,減小至健側(cè)膝關(guān)節(jié)水平。結(jié)論:在術(shù)后加速康復(fù)方案的指導(dǎo)下,早期前交叉韌帶重建術(shù)與晚期前交叉韌帶重建術(shù)相比,術(shù)后僵硬風(fēng)險(xiǎn)的差異無(wú)統(tǒng)計(jì)學(xué)意義。ACL組患側(cè)膝術(shù)前的TT-TG值大于健側(cè)膝及對(duì)照組,提示較大的TT-TG值可能是前十字韌帶損傷的解剖學(xué)病因之一;患側(cè)肢體前十字韌帶重建術(shù)后的TT-TG值減小,提示ACL重建術(shù)可重建脛股關(guān)節(jié)的自然狀態(tài)關(guān)系,TT-TG值可為ACL重建術(shù)后膝關(guān)節(jié)穩(wěn)定性的評(píng)價(jià)提供準(zhǔn)確的量化指標(biāo),具體量化方法仍需后續(xù)研究進(jìn)一步確定。
[Abstract]:Objective:. Comparison of postoperative stiffness risk between early anterior cruciate ligament reconstruction and late anterior cruciate ligament reconstruction under the guidance of accelerated rehabilitation; differences in TT-TG values of bilateral knee joints in patients with non-contact ACL injury before operation; Changes of TT-TG before and after operation. Methods:. The MRI data of 29 patients undergoing ACL reconstruction in our hospital from February 2011 to December 2013 and 33 patients in the control group before and after operation were analyzed retrospectively. The patients in the TT-TG group were followed up for more than one year after operation. The results of operation were evaluated according to the knee joint function score and stability. The measured data of each group were measured by independent sample t-test. The count data were analyzed by 蠂 2 test. Results: there was no significant difference in postoperative joint recovery between early group and late group (P 0.05). After ACL reconstruction, the rotation and varus of knee joint were obviously improved, and the TT-TG value was 11.19, which was reduced to the level of healthy knee joint. Conclusion: under the guidance of postoperative accelerated rehabilitation program. There was no significant difference in postoperative stiffness risk between early anterior cruciate ligament reconstruction and late anterior cruciate ligament reconstruction. The preoperative TT-TG value of affected knee in ACLgroup was higher than that in healthy knee and control group. It is suggested that the larger TT-TG value may be one of the anatomical causes of anterior cruciate ligament injury. The TT-TG value decreased after anterior cruciate ligament reconstruction, suggesting that ACL reconstruction can reconstruct the natural state of tibiofemoral joint. The TT-TG value can provide an accurate quantitative index for the evaluation of knee joint stability after ACL reconstruction.
