【摘要】：目的:通過中醫(yī)基礎(chǔ)理論與臨床研究,探討腰痹寧膏聯(lián)合脊痛消膠囊治療氣滯血瘀型腰椎間盤突出癥的臨床療效,與通絡(luò)骨質(zhì)寧膏聯(lián)合脊痛消膠囊作對比,通過治療前后VAS及JOA評分進行比較,分析臨床療效,為臨床指導氣滯血瘀型腰椎間盤突出癥的治療提供方法。方法:本次研究所有患者均來自于黑龍江中醫(yī)藥大學附屬第一醫(yī)院骨四科門診,收治時間為2016年1月至2017年1月,治療前確診為氣滯血瘀型腰椎間盤突出癥,將符合納入標準的60例患者隨機分為治療組30例和對照組30例,治療組采用腰痹寧膏聯(lián)合脊痛消膠囊治療,對照組采用通絡(luò)骨質(zhì)寧膏聯(lián)合脊痛消膠囊治療,4周為一療程,治療前后采用VAS及JOA評分進行評定,數(shù)據(jù)做統(tǒng)計學分析。結(jié)果:1.治療前兩組患者VAS及JOA評分進行統(tǒng)計學分析,p＞0.05,無統(tǒng)計學意義,兩組數(shù)據(jù)具有可比性。2.治療后兩組患者分別將VAS及JOA評分進行組內(nèi)比較,p0.05,有統(tǒng)計學意義,兩組患者經(jīng)治療后均有效。3.治療2周后、4周后分別將兩組數(shù)據(jù)進行組間比較,均得出p0.05,有統(tǒng)計學意義,治療組在治療效果上明顯優(yōu)于對照組。4.治療4周后,治療組顯效7例、有效22例、無效1例,總有效率96.67%,對照組顯效1例、有效25例、無效4例,總有效率86.67%。分析得出p＜0.05,差異有統(tǒng)計學意義。結(jié)論:1.腰痹寧膏與通絡(luò)骨質(zhì)寧膏相比,能有效的緩解氣滯血瘀型腰椎間盤突出癥患者的癥狀與體征,療效明顯。2.腰痹寧膏治療氣滯血瘀型腰椎間盤突出癥的總有效率明顯高于通絡(luò)骨質(zhì)寧膏。3.腰痹寧膏具有活血化瘀、通絡(luò)止痛的功效,簡單方便,副作用小,治療效果顯著,被廣大患者所接受,值得臨床推廣與運用。
[Abstract]:Objective: through the basic theory and clinical research of traditional Chinese medicine, to discuss the clinical effect of Yaobining plaster combined with Jiitongxiao capsule in treating lumbar intervertebral disc herniation with Qi stagnation and blood stasis type, compared with Tongluoguoning plaster combined with Jiitongxiao capsule. VAS and JOA scores before and after treatment were compared to analyze the clinical efficacy. Methods: all the patients in this study were from the fourth outpatient department of the first affiliated Hospital of Heilongjiang University of traditional Chinese Medicine. From January 2016 to January 2017, the patients were diagnosed as herniation of lumbar intervertebral disc with Qi stagnation and blood stasis before treatment. 60 patients who met the inclusion criteria were randomly divided into treatment group (n = 30) and control group (n = 30). The control group was treated with Tongluogu Ning ointment combined with Jiitongxiao capsule for 4 weeks as a course of treatment, VAS and JOA scores were used before and after treatment. Results: 1. Before treatment, the VAS and JOA scores of the two groups were statistically analyzed (p > 0.05), and there was no significant difference between the two groups (P > 0.05). After treatment, the VAS and JOA scores of the two groups were compared in group P 0.05, with statistical significance. 2. Two groups of patients were effective after treatment. 3. After 2 weeks of treatment and 4 weeks after the data were compared between the two groups, all obtained p0.05, with statistical significance. After 4 weeks of treatment, 7 cases in the treatment group were effective, 22 cases were effective, 1 case was ineffective, the total effective rate was 96.677.The control group showed remarkable effect in 1 case and effective in 25 cases. The total effective rate was 86.67. The difference was statistically significant (p < 0.05). Conclusion: 1. Compared with Tongluo Guoning ointment. It can effectively relieve the symptoms and signs of patients with Lumbar Disc herniation with Qi stagnation and Blood stasis. The total effective rate of Yaobining ointment in the treatment of Qi-stagnation and blood-stasis type lumbar disc herniation is significantly higher than that of Tongluo Guongning plaster .3.Yaobining ointment has the effect of promoting blood circulation and removing blood stasis, and it is simple and convenient. The side effect is small, the curative effect is remarkable, is accepted by the general patient, is worth clinical promotion and use.
