【摘要】：目的:觀察逍遙散加減方治療女性肝郁血虛型慢性便秘的臨床療效及安全性,為臨床治療女性慢性便秘提供參考。方法:采用隨機(jī)分組的方法,將符合納入標(biāo)準(zhǔn)的60例女性肝郁血虛型慢性便秘患者分為治療組30例,予逍遙散加減方,對(duì)照組30例采用聚乙二醇4000散劑(福松)治療,療程為8周。以每周完全自發(fā)排便(SCBM)≥3次的患者比例,慢性便秘主要癥狀積分、中醫(yī)證候療效為評(píng)定指標(biāo),并在療程結(jié)束后進(jìn)行隨訪4周以觀察遠(yuǎn)期療效,結(jié)果采用SPSS 22.0統(tǒng)計(jì)學(xué)軟件處理并分析。結(jié)果:1.每周SCBM≥3次:治療第4周治療組和對(duì)照組的SCBM≥3次患者比例分別為30%和26.7%;第8周兩組SCBM≥3次患者比例為治療組36.7%,對(duì)照組36.6%,均無統(tǒng)計(jì)學(xué)差異(P0.05)。2.主要癥狀:兩組患者的癥狀經(jīng)治療均有不同程度減輕,但在排便費(fèi)力、排便頻率、糞便性狀上兩組無明顯統(tǒng)計(jì)學(xué)差異(P0.05);而治療組較對(duì)照組在排便不盡感方面改善明顯(P0.05),治療組主要癥狀總積分與對(duì)照組比較有統(tǒng)計(jì)學(xué)差異(P0.05)。3.中醫(yī)證候:治療組與對(duì)照組總有效率分別為90%和76.7%,治療組在中醫(yī)證候方面改善效果顯著(P0.05)。4.遠(yuǎn)期療效:療程結(jié)束后半個(gè)月,治療組與對(duì)照組每周SCBM≥3次的患者比例、兩組便秘主癥總評(píng)分比較無統(tǒng)計(jì)學(xué)差異(P0.05),癥狀改善情況相似,治療后第一個(gè)月,治療組患者每周SCBM≥3次的比例為30%,對(duì)照組比例為10%,兩組具有統(tǒng)計(jì)學(xué)差異(P0.05);主癥總評(píng)分對(duì)比,治療組在療效維持上優(yōu)于對(duì)照組(P0.05)。整個(gè)治療過程中,兩組患者均未出現(xiàn)不良反應(yīng)。結(jié)論:逍遙散加減方對(duì)于改善女性肝郁血虛型慢性便秘臨床癥狀效果明顯,尤其在中醫(yī)證候和遠(yuǎn)期療效方面更具優(yōu)勢(shì)。
[Abstract]:Objective: To observe the clinical efficacy and safety of Xiaoyaosan female liver blood deficiency type chronic constipation, provide the reference for the clinical treatment of women with chronic constipation. Methods: a randomized method, will meet the inclusion criteria of 60 cases of female Ganyu deficiency type chronic constipation patients were divided into treatment group of 30 patients treated with modified Xiaoyaosan, the control group of 30 patients with macrogol 4000 powder (Forlax) treatment, treatment for 8 weeks. The weekly complete spontaneous bowel movements (SCBM) more than 3 times the proportion of patients, the main symptoms of chronic constipation score, TCM efficacy as evaluation index, and in the 4 weeks after the end of treatment were followed up to observe the long-term effect, the SPSS 22 statistical software processing and analysis. Results: 3 times a week 1. SCBM higher than that after fourth weeks of treatment, the treatment group and the control group of SCBM is more than 3 times the proportion of patients were 30% and 26.7%; the proportion of eighth weeks and two in group SCBM is more than 3 times the patients into the treatment group 36.7%, 36.6% in the control group, there was no significant difference (P0.05.2.) the main symptoms: two groups of patients after treatment, symptoms were relieved in different degree, but in the defecation, defecation frequency, stool on the two groups had no significant difference (P0.05); the treatment group improved significantly than the control group in the endless sense (P0.05), the treatment group total score of main symptoms compared with the control group there were significant differences (P0.05).3. TCM: treatment group and control group, the total effective rate was 90% and 76.7% in the treatment group, TCM syndrome and improve the effect of long term effects of.4. (P0.05): half a month after the end of treatment, the proportion of patients the treatment group and the control group SCBM was more than 3 times a week, no significant differences between the two groups of main symptoms total score of constipation (P0.05), improvement of symptoms similar to that of the first month after treatment, the patients in the treatment group were more than 3 times a week SCBM ratio is 30%, the control group accounted for 10%, two groups With statistical difference (P0.05); the main symptoms total score between the treatment group in the efficacy of maintenance is better than the control group (P0.05). The whole course of treatment, no adverse reactions were observed in two patients. Conclusion: modified Xiaoyaosan for improving women's liver blood deficiency type chronic constipation symptoms effect is obvious, especially in the aspect of traditional Chinese medicine the syndrome and long-term curative effect has more advantages.
