【摘要】：目的:評(píng)價(jià)微血管密度(microvessel density,MVD)、超聲造影參數(shù)、血流動(dòng)力學(xué)參數(shù)在前列腺癌(prostatic cancer,PCa)與慢性前列腺炎(chronic prostatitis,CP)中的差異及相關(guān)性,為PCa與CP的診斷及鑒別診斷、PCa的預(yù)后提供幫助。方法:對(duì)廣西醫(yī)科大學(xué)第一附屬醫(yī)院2014-2015年泌尿外科收治入選的前列腺病患68例行前列腺經(jīng)直腸超聲(Transrectal Ultrasonography,TRUS),獲取血流動(dòng)力學(xué)參數(shù);前列腺經(jīng)直腸超聲造影(contrast enhanced transrectal ultrasound,CETRUS),獲取造影參數(shù);超聲引導(dǎo)下穿刺活檢或手術(shù)明確病理類(lèi)型及格里森(Gleason)評(píng)分;進(jìn)行免疫組化MVD的測(cè)定。比較PCa組與CP組、PCa中Gleason評(píng)分≤3+4組與Gleason評(píng)分≥4+3組的血流動(dòng)力學(xué)、MVD、超聲造影參數(shù)的差異;研究PCa中MVD與血流動(dòng)力學(xué)參數(shù)、超聲造影參數(shù)的相關(guān)性。結(jié)果:病理證實(shí)前列腺癌患者44例,CP患者24例。PCa組與CP組中的收縮期峰值血流速度(Velocity of flow in systolic,Vs)、阻力指數(shù)(Resistance Index,RI)、初始強(qiáng)度(Basic intensity,B)、曲線下面積(Area under the curve,AUC)、峰值強(qiáng)度(Peak Intensity,PI)、MVD差異均有統(tǒng)計(jì)學(xué)意義(P0.05);PCa中Gleason評(píng)分≥4+3組28例,Gleason評(píng)分≤3+4組16例,兩組間Vs、RI、MVD差異有統(tǒng)計(jì)學(xué)意義(P0.05);PCa中Vs、RI、AUC與MVD具有正相關(guān)(P0.05),r分別為0.578、0.688、0.737。結(jié)論:血流動(dòng)力學(xué)、超聲造影參數(shù)、微血管密度為PCa與CP的診斷與鑒別診斷提供幫助;前列腺癌中超聲造影、血流動(dòng)力學(xué)參數(shù)可較好的反映微血管密度;血流動(dòng)力學(xué)參數(shù)、微血管密度對(duì)PCa患者預(yù)后的判斷有一定價(jià)值。
