超聲造影、血流動力學參數(shù)、微血管密度在前列腺癌與慢性前列腺炎中的診斷價值

【摘要】:目的:評價微血管密度(microvessel density,MVD)、超聲造影參數(shù)、血流動力學參數(shù)在前列腺癌(prostatic cancer,PCa)與慢性前列腺炎(chronic prostatitis,CP)中的差異及相關性,為PCa與CP的診斷及鑒別診斷、PCa的預后提供幫助。方法:對廣西醫(yī)科大學第一附屬醫(yī)院2014-2015年泌尿外科收治入選的前列腺病患68例行前列腺經(jīng)直腸超聲(Transrectal Ultrasonography,TRUS),獲取血流動力學參數(shù);前列腺經(jīng)直腸超聲造影(contrast enhanced transrectal ultrasound,CETRUS),獲取造影參數(shù);超聲引導下穿刺活檢或手術明確病理類型及格里森(Gleason)評分;進行免疫組化MVD的測定。比較PCa組與CP組、PCa中Gleason評分≤3+4組與Gleason評分≥4+3組的血流動力學、MVD、超聲造影參數(shù)的差異;研究PCa中MVD與血流動力學參數(shù)、超聲造影參數(shù)的相關性。結(jié)果:病理證實前列腺癌患者44例,CP患者24例。PCa組與CP組中的收縮期峰值血流速度(Velocity of flow in systolic,Vs)、阻力指數(shù)(Resistance Index,RI)、初始強度(Basic intensity,B)、曲線下面積(Area under the curve,AUC)、峰值強度(Peak Intensity,PI)、MVD差異均有統(tǒng)計學意義(P0.05);PCa中Gleason評分≥4+3組28例,Gleason評分≤3+4組16例,兩組間Vs、RI、MVD差異有統(tǒng)計學意義(P0.05);PCa中Vs、RI、AUC與MVD具有正相關(P0.05),r分別為0.578、0.688、0.737。結(jié)論:血流動力學、超聲造影參數(shù)、微血管密度為PCa與CP的診斷與鑒別診斷提供幫助;前列腺癌中超聲造影、血流動力學參數(shù)可較好的反映微血管密度;血流動力學參數(shù)、微血管密度對PCa患者預后的判斷有一定價值。
[Abstract]:Objective: to evaluate the microvessel density (MVDV) of microvessel density (MVD), and to evaluate the parameters of contrast-enhanced ultrasound (CEUs).
