【摘要】：白血病是兒童時(shí)期常見的惡性腫瘤疾病,在治療過程中患兒承受著生理和心理兩方面的煎熬,隨著現(xiàn)代醫(yī)療技術(shù)的不斷發(fā)展,患兒及其家庭心理社會(huì)層面的治療和輔導(dǎo)越來越受到重視。醫(yī)務(wù)社會(huì)工作的出現(xiàn)不僅滿足了人們這一需要,而且逐漸成為現(xiàn)代醫(yī)療過程中不可或缺的重要組成部分。此次研究結(jié)合本人對(duì)白血病患兒家庭提供的具體服務(wù),對(duì)這一實(shí)踐過程進(jìn)行研究和反思,運(yùn)用嵌入性發(fā)展理論分析醫(yī)務(wù)社會(huì)工作在介入患兒家庭時(shí)面臨的困境和挑戰(zhàn)。研究發(fā)現(xiàn),醫(yī)務(wù)社工在嵌入原有醫(yī)療體系,對(duì)白血病兒童家庭提供服務(wù)時(shí),面臨以下幾方面的困境:一是作為嵌入主體的醫(yī)務(wù)社工存在自身實(shí)務(wù)能力不足、專業(yè)認(rèn)同感較低、社會(huì)知曉度不高等問題;二是作為被嵌入一方的醫(yī)院給醫(yī)務(wù)社工提供的讓渡空間有限,服務(wù)對(duì)象對(duì)現(xiàn)代健康觀念表現(xiàn)出低認(rèn)同等;三是醫(yī)務(wù)社工在嵌入過程中存在選擇性服務(wù)和嵌入失真等現(xiàn)象;四是從嵌入效應(yīng)上看,醫(yī)務(wù)社工服務(wù)介入效果不明顯,且自身發(fā)展的自主性受限等。最后針對(duì)以上困境進(jìn)行探討,提出應(yīng)該從主體層面注重和加強(qiáng)專業(yè)醫(yī)務(wù)社工教育;在對(duì)象層面,優(yōu)化醫(yī)務(wù)社工與醫(yī)生的協(xié)同合作機(jī)制來為患兒家庭提供更全面的服務(wù),并在已有的“社工+志愿者”的模式基礎(chǔ)上試圖探索“醫(yī)院-社工部-社工機(jī)構(gòu)”三位一體的醫(yī)務(wù)社工實(shí)務(wù)模式的應(yīng)用。
[Abstract]:Leukemia is a common malignant tumor disease in childhood. In the course of treatment, the child is suffering from both physical and psychological aspects. With the development of modern medical technology, More and more attention has been paid to the psycho-social treatment and counseling of children and their families. The emergence of medical social work not only meets the needs of people, but also gradually becomes an indispensable part of modern medical process. This study combined with the specific services provided by my family to children with leukemia, this practical process of research and reflection, the use of embedded development theory to analyze the medical social work in the intervention of the child's family difficulties and challenges. The study found that medical social workers are faced with the following difficulties when they are embedded in the original medical system to provide services to leukemia children's families: first, the medical social workers, as embedded subjects, have their own lack of practical ability and low professional identity. The social awareness degree is not high and so on; Second, the hospital as one of the embedded side provides limited space for medical social workers, and the service object shows a low recognition of modern health concept, third, the medical social worker has the phenomenon of selective service and embedded distortion in the process of embedding. Fourthly, from the perspective of embedding effect, the intervention effect of medical social work service is not obvious, and the autonomy of its own development is limited. Finally, in view of the above difficulties, the author puts forward that we should pay attention to and strengthen the education of professional medical social workers from the subject level; At the target level, we should optimize the cooperative mechanism between medical social workers and doctors to provide more comprehensive services for families with children. On the basis of the existing model of "social worker volunteers", this paper attempts to explore the application of the "hospital-social work department-social work organization" three-in-one medical social work practice model.
