社會(huì)工作對(duì)兒童精神分裂癥患者的介入研究

【摘要】:本研究以社會(huì)支持模式作為實(shí)踐框架,以增權(quán)理論和一般系統(tǒng)理論為理論基礎(chǔ),主要運(yùn)用社會(huì)工作專業(yè)的小組工作方法,帶領(lǐng)武漢市精神衛(wèi)生中心10名康復(fù)期兒童精神分裂癥患者進(jìn)行了四期的小組活動(dòng)。小組活動(dòng)以“社會(huì)交往體驗(yàn)”為主題,旨在增加長(zhǎng)期住院的康復(fù)期兒童精神分裂癥患者人際交往體驗(yàn),以探討和了解社會(huì)支持模式小組的社會(huì)工作方法,對(duì)康復(fù)期兒童精神分裂癥患者社會(huì)功能恢復(fù)的作用和意義。 在資料收集的過程中,筆者發(fā)現(xiàn),武漢市精神衛(wèi)生中心兒少科康復(fù)期精神分裂癥患者正處于兒童和青少年階段,對(duì)于游戲和交往有著一定的需求,但長(zhǎng)期住院治療和臨床癥狀使得他們這些需求無從滿足,同時(shí)該群體面臨的主要問題有二:一是否認(rèn)病情,表現(xiàn)出退縮、回避、自卑等消極行為和態(tài)度;二是社交困難,表現(xiàn)出害羞、焦慮等消極情緒。針對(duì)上述問題,本研究以小組工作方法為主,設(shè)計(jì)了娛樂性和反思性兼具的游戲活動(dòng);同時(shí)在小組活動(dòng)的前后分與小組成員進(jìn)行了個(gè)別訪談,以比較小組活動(dòng)前后小組成員情緒、行為和態(tài)度的變化。對(duì)于結(jié)果部分的探討,筆者整理了大量的一手資料,包括小組成員基本信息、參加小組活動(dòng)情況記錄、工作人員觀察記錄、護(hù)士用住院病人觀察量表(NOSIE)、部分患者訪談?dòng)涗浌?jié)選等,通過比較發(fā)現(xiàn):首先,小組成員較小組活動(dòng)之前有了較強(qiáng)的社會(huì)交往意愿;其次,,在日常人際交往方面體現(xiàn)出更多的積極性和主動(dòng)性,康復(fù)治療的積極性也有所提高;另外,小組成員間的互動(dòng)和支持網(wǎng)絡(luò)得以建立,成員間關(guān)系密切;最后,小組成員通過活動(dòng)意識(shí)到“重要他人”對(duì)自己的意義,促使其組外支持系統(tǒng)的建立。
[Abstract]:This research takes the social support model as the practice frame, takes the empowerment theory and the general system theory as the theoretical foundation, mainly uses the social work profession group work method. Led 10 rehabilitation children with schizophrenia in Wuhan Mental Health Center for four stages of group activities. With the theme of "the experience of social interaction", the group aims to increase the interpersonal experience of children with schizophrenia who have been hospitalized for a long time, so as to explore and understand the social work methods of the social support model group. The role and significance of social function recovery in children with schizophrenia during rehabilitation period. In the course of the data collection, the author found that the schizophrenic patients in Wuhan Mental Health Center are in the stage of children and adolescents, and have a certain demand for play and communication. However, long-term hospitalization and clinical symptoms make them unsatisfied with these needs. At the same time, the group faces two main problems: denying their condition, showing negative behaviors and attitudes such as withdrawal, avoidance, inferiority complex, and social difficulties. Show negative emotions such as shyness, anxiety, etc. In order to solve the above problems, the study focused on the group work method, designed both entertaining and reflective game activities, and conducted individual interviews with the group members before and after the group activities. Compare changes in group members' emotions, behaviors, and attitudes before and after group activities. For the results part of the discussion, the author collated a large number of first-hand information, including the basic information of the team members, participation in the group activities record, staff observation records, Nurses used the inpatient observation scale (NOSIEI), excerpts from some patient interview records, and found that: first, the group members had a stronger willingness to socialize than before the group activities; secondly, the group members had a stronger willingness to communicate with each other than before the group activities. Show more enthusiasm and initiative in daily interpersonal communication, improve the enthusiasm of rehabilitation treatment; in addition, the interaction and support network among group members has been established, and the relationship between members is close; finally, Team members recognize the meaning of "important others" to themselves through activities that encourage them to support the system outside the group.
